Abstract

Latar belakang: TBC menjadi masalah kesehatan dunia, termasuk Indonesia karena kasus baru TB paru terus meningkat. Penelitian ini mengkaji faktor dominant yang mempengaruhi penularan kontak serumah pasien TB paru. Metode: Studi observasional dengan desain analitik cross sectional ini menggunakan populasi seluruh penderita dan anggota keluarga pasien TB paru di Puskesmas Kedundung tahun 2015 2016. Sampel sejumlah 52 orang dihitung menggunakan rumus besar sampel infinith dan diambil secara konsekutif. Variabel penelitian meliputi perilaku pencegahan, Gradasi BTA dan penularan kontak serumah. Pengumpulan data menggunakan lembar wawancara, lembar pengumpul data, sputum pot steril. Data dianalisis menggunakan uji Spearman Rank dan uji regresi logistik binary dengan tingkat signifikansi 0,05. Hasil: Perilaku pencegahan (p=0.093), BTA positive 2+ (p=0.377) tidak berpengaruh terhadap penularan kontak serumah pasien TB paru, sedangkan BTA positive 1+ mempengaruhi penularan kontak serumah pasien TB paru (p= 0,007). Nilai Exp (B) BTA positif (1+) menunjukkan 12,144 artinya pasien BTA positif (1+) memiliki risiko 12,144 kali lebih tinggi menularkan ke kontak serumah dibandingkan dengan BTA positif (3+). Sedangkan pasien BTA positif (2+) memiliki risiko 3,328 kali menularkan ke kontak serumah dibandingkan dengan BTA positif (3+). Kesimpulan: Pasien TB paru dengan pemeriksaan BTA positif (1+) menjadi faktor dominan yang mempengaruhi penularan kontak serumah pasien TB paru. Upaya komunikasi dan edukasi personal higiene, tingkat kepatuhan pengobatan pasien TB paru perlu ditingkatkan untuk menekan kejadian baru TB paru. Kata kunci: Gradasi BTA, TB Paru, Perilaku Pencegahan, Penularan Kontak Rumah Tangga Abstract Background: Tuberculosis has become a global health problem, included in Indonesia, new cases of pulmonary TB increase continuously. This study examined the dominant factors that influenced the transmission of household contacts in pulmonary TB patients. Methods: This observational study was a cross-sectional analytic design which used a population of all patients and family members of pulmonary TB patients at the Kedundung Health Center in 2015 until 2016. A sample of 52 people was counted using the infinith sample size formula and taken consecutively. Research variables include prevention behavior, AFB smear gradation, and household contact transmission. Collecting data used interview sheets, data collection sheets, sterile sputum pots. Data was analyzed by chi-square test and binary logistic regression test with a significance level of 0.05. Results: Prevention behavior (p=0.093), AFB smear-positive 2+ (p=0.377) did not affect on household contact transmission in pulmonary TB patients, whereas AFB smear-positive 1+ affected household contacts transmission of pulmonary TB patients (p=0.007). The value of Exp (B), AFB smear-positive (1+) have a risk of 12.144 times transmitting to household contact when compared to AFB smear-positive (3+). Whereas patients with AFB smear-positive (2+) have a risk of 3,328 times transmitting to household contact when compared with AFB smear-positive 3+. Conclusion: Pulmonary TB patients with AFB smear-positive (1+) was the dominant factor affecting household contact transmission. Communication and personal hygiene education efforts, the level of adherence in the treatment of pulmonary TB patients needs to be increased to suppress the new incidence of pulmonary TB. Abstract

Medical record personnel is one part of the profession in the hospital. Medical record personnel also have animportant role in a patient service. Because it is responsible for the procurement of files beginning from patientregistration, assembling, coding, indexing, filling, internal or external reporting and INACBG’s. There is aproblem concerning the workload of medical records personnel at Surabaya PHC Hospital. A number ofofficers are have been resigning for the last few years, causing a reduction of a number of officers. Theworkload indirectly becomes higher and double jobs occurred. In addition, there are also found several officerswho have 2 jobs such as an outpatient coding officer become a filling officer, an inpatient coding officerbecome an information release officer and an assembling officer become a hemodialysis coding officer. Thepurpose of this study was to analyzed the needs of medical record personnel based on workload in SurabayaPHC Hospital using the WISN (Workload Indicator Staffing Need) method. A quantitative research approachwith a descriptive study design used as this research type. Respondents that used were 1 person reporting, 1person assembling, 1 person coding outpatient and 1 person filling. The results of this research were 12 ofofficers with an educational background in Medical Records. It took the addition of 1 person for the assemblingunit, 6 people for the outpatient coding unit, 2 people for the inpatient coding unit, 3 people for the filling unitand 1 person for the information release unit.

Abstract

One of the obligations of the hospital is carrying out medical records. The implementation of medicalrecords at the Primasatya Husada Citra Surabaya Hospital has used an electronic system. The applicationis an Electronic Medical Record (ERM) with a website-based that runs since September 1, 2018. SinceSeptember 1, 2018, until now there has never been an evaluation of the application. This study aims toevaluate ERM using the Human Organization Technology (HOT) -Fit Model evaluation method. This typeof research is qualitative research with data collection methods using interviews, observation, anddocumentation. The results of the study indicated that the implementation of ERM has been going well,socialization and training have been provided by organizations, the presentation of data are not right, andcomputer facilities are still lacking. The researcher's suggestion for ERM at PHC Surabaya Hospital is totest the reliability and security of the ERM system, add one computing device, and improve thepresentation of data by the system to improve the quality of the ERM system.

Abstract

The development of information technology in the world is very fast, one of which is Mobile technology. Mobile is a cellular communication tool that provides convenience in conveying information quickly and completely so that it is very important for all sectors, especially in the service of government agencies. Indonesia is a country with a fairly high incidence of disasters. This condition cannot be separated from geographical factors, namely archipelagic and volcanic countries. There were 13 disaster events recorded in 2020. Jember Regency is an area where disasters often occur so that special attention is needed in handling them. There were 286 disaster events in 2019 including floods, landslides and earthquakes. There are many researches on making android applications about disasters, but they still focus on mitigation and have not fully touched the real needs of victims. So it is still necessary to develop the android application (Ship, Handle & Drive). This application adopts and develops an online motorcycle taxi system that is applied in disaster management by supporting the features needed in the current digital era, namely victims of natural disasters, the government and officers so that they are practical in use. the hope is that this application can facilitate disaster management easily, quickly, precisely and accurately. The research method uses the waterfall method with a systematic approach through the stages of Analysis, Design, Coding, and Testing.

Abstract

Rekam kesehatan berbasis komputer merupakan rekaman pasien secara elektronik untuk mendukung pengguna mengakses secara lengkap & akurat. Manfaat rekam medis elektronik (RME) sangat penting tetapi terdapat beberapa ancaman yg menjadi perhatian khusus dpt berdampak merugikan. Pencurian data kesehatan mengalami peningkatan menjadi permasalahan serius. Berdasarkan observasi & wawancara ditemukan permasalahan dari aspek keamanan di Rumah Sakit X. Tujuan penelitian untuk menganalisis aspek keamanan data pasien dlm penerapan RME berdasarkan aspek kerahasiaan, integritas, autentikasi, ketersediaan, akses kontrol, nir-sangkal. Penelitian menggunakan pendekatan kualitatif, pengumpulan data melalui wawancara, observasi, dokumentasi dgn subjek penelitian 7 responden. Data dianalisis dg reduksi data, penyajian data & kesimpulan. hasil menunjukkan bahwa keamanan dari aspek kerahasiaan yaitu login menggunakan username & password namun belum melakukan penggantian secara berkala serta belum adanya SOP. Dari aspek integritas terdapat fitur edit data untuk pengguna sesuai tupoksi, edit data dlm jumlah besar tidak dapat dilakukan secara langsung namun harus sesuai SOP. Aspek Autentikasi sudah menerapakan tanda tangan elektronik bersertifikat menjamin keabsahan. aspek ketersediaan RME hanya ddt diakses dilingkungan rumah sakit dengan VPN sehingga mudah diakses. Aspek kontrol akses untuk membatasi hak akses pengguna diterapkan username & password. Aspek Nir-sangkal terdapat riwayat bagi pengguna yg mengakses data pasien.

Abstract

Fraud in healthcare services is any form of deception carried out by various parties in healthcare services to gain personal benefits beyond the profits obtained from normal practices. At RS X, there is a team called the National Health Insurance (JKN) Fraud Prevention Team, which functions to ensure that the quality of healthcare services provided meets the applicable standards. One of the methods to analyze the distribution of abnormal data in a dataset is by using the concept of Benford's Law. The purpose of this research is to detect potential fraud in inpatient JKN claim data at RS X using Benford's Law. The type of research used is descriptive quantitative research. The population in this study is the JKN inpatient claim data for the period from August 2024 to October 2024, consisting of 11,789 rows of data. The use of Benford's Law to examine the differences in the "Hospital Rate" values shows that there is no difference in the pattern between the actual frequency and the expected frequency according to Benford's Law. The hypothesis test using chi-squared where the null hypothesis of the study is accepted, namely that the first digit numbers in the "Hospital Rates" column from August to October 2024 are distributed according to Benford's law.
