Abstract

Background: Various complications caused by causing diabetes mellitus patients to undergo treatment at the Hospital to overcome the complaints that arise. This causes the patient to experience stress physically and psychologically. Objective: To explain the effect of progressive muscle relaxation on the stress response, blood glucose levels, and quality of life in patients with diabetes mellitus. Method: This systematic review used the PRISMA approach. The database used in this study were Scopus, Pubmed, Proquest, and Science Direct in English. Results: Progressive Muscle Relaxation (PMR) could reduce muscle tension, stress, lower blood pressure, increased tolerance for daily activities. Regular exercise can make it easier for patients to concentrate and relax conditions will be more quickly achieved thereby increasing the performance of the adrenal glands to produced sedative hormones which will have an impact on reducing stress and decreasing blood glucose levels so that the quality of life of DM patients increased. Conclusion: PMR influences the stress response, blood glucose levels, and quality of life in patients with diabetes mellitus. Keywords: progressive muscle relaxation; stress; blood glucose level; quality of life; diabetes mellitus ABSTRAK Latar belakang: Berbagai komplikasi yang ditimbulkan menyebabkan pasien diabetes mellitus harus menjalani perawatan di Rumah Sakit untuk mengatasi keluhan yang ditimbulkan. Hal ini menyebabkan pasien mengalami stres secara fisik maupun secara psikologis. Tujuan: Untuk menjelaskan pengaruh progressive muscle relaxation terhadap respon stres, kadar glukosa darah dan kualitas hidup pada pasien Diabetes Melitus. Metode: Systematic review ini menggunakan pendekatan PRISMA. Database yang digunakan adalah Scopus, Pubmed, Proquest, dan Science Direct menggunakan bahasa Inggris. Hasil: Progressive Muscle Relaxation (PMR) mampu mengurangi ketegangan otot, menurunkan stress, menurunkan tekanan darah, meningkatkan toleransi terhadap aktivitas sehari-hari. Latihan yang teratur dapat membuat pasien lebih mudah untuk memusatkan pikiran dan kondisi rileks akan lebih cepat tercapai sehingga meningkatkan kinerja kelenjar adrenal untuk memproduksi hormon penenang yang akan berdampak pada penurunan stres dan penurunan kadar glukosa darah sehingga kualitas hidup pasien DM meningkat. Kesimpulan: PMR memiliki pengaruh terhadap respon stres, kadar glukosa darah, dan kualitas hidup pada pada pasien diabetes melitus. Abstract

Background: Diabetes mellitus is a metabolic disorder characterized by chronic hyperglycemia with impaired carbohydrate, protein and fat metabolism associated with dyslipidemia, which is a risk factor for increased cardiovascular disease and mortality. Educational / physical education programs and activities are alternatives in the management of Type 2 Diabetes mellitus (T2DM). Objective: To evaluate the effect of a single intervention or a combination of educational programs and physical activity on glucose values and lipid profiles in T2DM patients. Methods: The database used in this systematic review was Scopus and PubMed / Medline, in English. The analyzed papers were published between 2016-2020. Total of 20 randomized controlled trials were included that focused on educational program interventions and physical activity in T2DM patients. Results: The selected article has shown that the implementation of physical activity programs (aerobic, resistance, aerobic and resistance combination, as well as Passive Static Stretching (PSS)) and educational programs obtained based on article analysis namely professional Problem Based Learning (PBL), Conversation maps ( CMs), self-management education conducted by nurses, education with video media, family-based, group-based education, community-based multimodal intervention methods, diabetes self-management education (DSME) programs, autonomy support (family and peers). improve glycemic control as well as lipid profile in T2DM patients. Conclusions: educational programs regarding the importance of changing lifestyles and physical activity programs are scientifically expressed as complementary therapy strategies in the treatment and metabolic control of T2DM. Suggestion: Education programs and physical activities independently or modified can be used as alternative non-pharmacological / complementary therapies by taking into account the level of education and medical history of DMT2 patients. Keywords: education; physical activity; biochemistry; type 2 diabetes mellitus ABSTRAK Latar belakang: Diabetes mellitus adalah kelainan metabolisme yang ditandai dengan adanya hiperglikemia kronis dengan gangguan metabolisme karbohidrat, protein dan lemak yang dikaitkan dengan dislipidemia, yang merupakan faktor risiko peningkatan penyakit kardiovaskular dan mortalitas. Program pendidikan/edukasi dan aktivitas fisik menjadi alternative dalam pilar penatalaksanaan Diabetes mellitus tipe 2 (DMT2). Tujuan: Mengevaluasi pengaruh intervensi tunggal atau gabungan program pendidikan dan aktivitas fisik pada nilai glukosa dan profil lipid pada pasien DMT2. Metode: Database yang digunakan dalam tinjauan sistematis ini adalah Scopus dan PubMed/Medline, dengan bahasa Inggris, Makalah yang dianalisis diterbitkan antara 2016 – 2020. Sebanyak 20 uji coba terkontrol secara acak dimasukkan yang berfokus pada intervensi program pendidikan dan aktivitas fisik pada pasien DMT2. Hasil: Artikel yang dipilih telah menunjukkan bahwa pelaksanaan program aktivitas fisik (aerobic, resistensi, gabungan aerobic dan resistensi, serta Passive Static Stretching (PSS)) dan program pendidikan yang diperoleh berdasarkan analisa artikel yaitu profesional Problem Based Learning (PBL), Conversation maps (CMs), edukasi manajemen diri yang dilakukan oleh perawat, pendidikan dengan media video, berbasis keluarga, berbasis edukasi kelompok, metode intervensi multimodal berbasis masyarakat, program Diabetes Self-Management Education (DSME), dukungan otonomi (keluarga dan teman sebaya). meningkatkan kontrol glikemik serta profil lipid pada pasien DMT2. Simpulan: program pendidikan mengenai pentingnya mengubah gaya hidup dan program aktivitas fisik secara ilmiah diungkapkan sebagai strategi terapi komplementer dalam pengobatan dan kontrol metabolic pada DMT2. Saran: Program pendidikan dan aktivitas fisik secara independen maupun dimodivikasi dapat dijadikan sebagai alternative terapi non-farmakologis / komplementer dengan memperhatikan tingkat pendidikan dan riwayat kesehatan pasien DMT2. Abstract

Background: Type 2 diabetes mellitus was one of non-communicable diseases that increased of prevalence in word wide, included in Indonesia. Utilization of technology as an effort of increase of diabetes treatment is important for achieving of optimum glycemic control and to prevent of complication of diabetes mellitus. However, intervention for self management in patients with diabetes mellitus at this time still not using technology based mobile health intervention. Purpose: This study aims to verify of effectiveness of mobile health intervention on self management and glycemic control in patient with type 2 diabetes mellitus. Method: Design of this studies was used systematic review of randomized controlled trial with PRISMA approach. Article search was carried out through databases: Scopus, Science Direct, and ProQuest with randomized controlled trial design within last 10 years. Results: The finding showed 407 articles have been obtained. Articles selection process were through few steps: topic selection, full text selection, design of studies and obtained 10 articles have been as appropriate of inclusion criteria. Based on results of finding of 10 articles were showed that mobile health intervention was effective in improving of glycemic control by decreasing hbA1c, fasting blood glucose, 2 hours post pandrial. Furthermore, mobile health intervention was effective in increasing of self management in patient with type 2 diabetes mellitus and increased adherence of diabetes medication. Moreover, mobile health intervention can also improve of insulin level and lipid profile in patient with type 2 diabetes mellitus. Conclusion: Mobile health intervention was effective in improving of glycemic control and self management, and giving of facilitate communication between patient and health providers Keywords: mobile health application; self management; glycemic control; diabetes mellitus ABSTRAK Latar belakang: diabetes melitus (DM) tipe 2 merupakan salah satu penyakit tidak menular yang mengalami peningkatan angka kejadian di dunia, termasuk di Indonesia. Penggunaan teknologi sebagai upaya meningkatkan manajemen diabetes melitus sangatlah penting untuk dilakukan guna mencapai kontrol glikemik optimal dan mencegah komplikasi dari Diabetes Melitus. Namun, manajemen diri pada sebagian besar pasien Diabetes Melitus saat ini masih belum menggunakan bantuan teknologi berbasis mobile health. Tujuan: Penelitian ini bertujuan untuk memverifikasi efektivitas dari penggunaan mobile health intervention terhadap manajemen diri dan kontrol glikemik pasien dengan diabetes melitus tipe 2. Metode: desain dalam penelitian ini adalah systematic review dengan menggunakan pendekatan PRISMA. Pencarian artikel dilakukan pada beberapa database yang meliputi: Scopus, Science Direct dan ProQuest dengan desain Randomized controlled trial dalam 10 tahun terakhir. Hasil: hasil temuan didapatkan sejumlah 407 artikel penelitian. Proses seleksi artikel dilakukan beberapa tahap meliputi: seleksi topik, fullteks, desain artikel penelitian dan didapatkan 10 artikel penelitian yang sesuai dengan kriteria inklusi. Berdasarkan hasil temuan dari 10 artikel penelitian yang digunakan, menunjukkan bahwa mobile health intervention efektif dalam memperbaiki kontrol glikemik pasien diabetes melalui penurunan kadar hbA1c, gula darah puasa, 2 jam post pandrial. Selanjutnya, mobile health intervention efektif dalam meningkatkan manajemen diri pasien diabetes dan meningkatkan kepatuhan pengobatan. Selain itu, mobile health intervention juga dapat memperbaiki level insulin dan profil lipid pasien diabetes melitus tipe 2. Kesimpulan: Mobile health intervention efektif dalam memperbaiki kontrol glikemik dan meningkatkan manajemen diri pasien diabetes melitus serta memudahkan komunikasi antara pasien dengan tenaga kesehatan Kata kunci: mobile health application; manajemen diri; kontrol glikemik; diabetes melitus

Abstract

Introduction: Various efforts have been made to improve the self-care management of patients with type 2 diabetes mellitus (T2DM). One of them is by using a Diabetes Self-Management Education and Support (DSME/S) program. DSME/S produces positive effects in relation to patient behavior and health status. This is a systematic review of randomized controlled trials published where the aim was to evaluate the impact of the DSME/S program in term of biological. psychological and social aspects.Methods: The articles were searched for using the PRISMA approach from within Scopus, Sage Journal, ProQuest, Google Scholar and PubMed to identify the relevant English publications on DSME over the last 5 years (2013-2018). In total, 15 studies met the inclusion criteria.Results: The articles included at least one result that covered the biological, psychological, and social aspects that are more general and relevant for T2DM patients who received the DSME program. DSME had a positive impact on T2DM, namely the reduction of HbA1c, blood glucose, LDL, cholesterol, blood pressure, weight, waist circumference, decreased distress, anxiety and increased self-efficacy and self-empowerment.Conclusion: It can increase the social and family support, improve self-management motivation, increase knowledge and improve the behavior of T2DM patients. The findings of our review showed that DSME has a positive impact on the biological, psychological and social aspects of type 2 diabetes mellitus.

Abstract

Introduction: The present study aimed to analyze the effects of aerobic exercise, resistance and a combination of both on blood glucose levels, HbA1c and quality of life for patients with T2DM.Methods: This systematic review used PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analysis). Scopus, Science Direct and PubMed online databases were extensively searched, focusing on the last five years. The search utilized the phrases "diabetes," "physical exercise,” "glucose" and "quality of life." The inclusion criteria in the study regarding the literature was that it had to be an original article, that the source was a journal, that the article was in English and that it was available in full text.Results: We identified 1017 articles where 17 were considered to be relevant for this systematic review. The combination of resistance and aerobic physical exercise seems to have a greater impact on glucose, HbA1c and quality of life. Aerobic exercise, resistance exercise and a combination of both have benefits when it comes to reducing blood glucose levels and HbA1c, as well as improving the quality of life of patients with T2DM.Conclussion: The type and intensity of the exercise chosen for the management of T2DM must be adjusted to the clinical condition and the patient’s individual physical fitness

Abstract

Introduction: Diabetes is a chronic disease characterized by abnormal blood sugar levels due to pancreatic disorders.  Quality of life and ankle brachial index values are an area of concern for nurses related to non-pharmacological treatments to prevent complications. Progressive Muscle Relaxation (PMR) can improve relaxation and cellular immunity, as well as Neuromuscular Taping (NMT) is a non-pharmacological intervention to prevent lower extremity peripheral blood circulation decline, so both have a potential positive impact on the patient's quality of life. The aim of this study is to analyze the effectiveness of PMR and NMT on ankle brachial index (ABI) and quality of life in Type 2 Diabetes Mellitus (T2DM). Methods: The study was a quasy-experiment with a pre post test control group design. Consecutive sampling was implemented and resulted a sample of 32 T2DM patients with certain criteria, such as patients who were treated in the internal medicine ward, age 40-70 years, suffering from T2DM for more than5 years, not suffering from diabetic ulcers, and still can do some exercise. Data were analyzed using the Wilcoxon Signed Ranks Test with correlation test with alpha 0.05. Results: There was a significant effect of the PMR-NMT combination intervention on ABI (p= 0.034) and quality of life (p= 0.007) in the intervention group. Conversely, the control group showed no difference in ABI (p= 0.142) and quality of life (p= 0.739) before and after the study. Conclusion: PMR and NMT can improve ABI values and Quality of Life in patients with T2DM. This PMR-NMT combination can be an alternative non-pharmacological therapy in patients T2DM to help improve ankle brachial index and quality of life.
