Abstract

HIV merupakan penyakit infeksi yang masih menjadi masalah besar kesehatan di Indonesia maupun dunia. Berbagai macam bentuk infeksi yang disebabkan oleh HIV, salah satunya ditandai dengan turunnya jumlah sel limfosit T CD4+ dan juga penurunan kadar IFN-GAMMA yang dapat menyebabkan percepatan replikasi virus hingga terjadi kegagalan sistem imun. NAC yang merupakan antioksidan diharapkan dapat memperbaiki kondisi tersebut. Penelitian ini bertujuan untuk mengetahui efek pemberian NAC terhadap perubahan kadar IFN-GAMMA  pada penderita HIV/AIDS di RSUP dr. Mohammad Hoesin Palembang. Penelitian ini merupakan uji klinik acak tersamar ganda. Sampel yang digunakan dalam penelitian ini sebanyak 32 pasien HIV/AIDS yang menjalani pengobatan ARV di RSUP dr. Mohammad Hoesin Palembang dibagi secara acak ke dalam 2 kelompok, yaitu kelompok Plasebo dan NAC. Pada kelompok Plasebo, pasien diberi kapsul yang berisi laktosa dengan dosis 3x1 kapsul/hari, sedangkan kelompok NAC, diberi NAC dengan dosis 3x 200 mg/hari. Setiap kelompok diberi perlakuan sama selama 12 minggu. Darah pasien diambil sebelum dan setelah perlakuan untuk diperiksa kadar IFN-GAMMA . Hasil penelitian menunjukkan bahwa kelompok uji yang diberikan NAC maupun placebo tidak menunjukkan perubahan kadar IFN-GAMMA yang bermakna setelah 12 minggu perlakuan. Kesimpulan penelitian, pemberian NAC tidak mempengaruhi kadar IFN-GAMMA pada penderita HIV/AIDS yang menjalani pengobatan ARV.HIV is an infectious disease that becomes a major health problem in Indonesia and the world. There are various forms of infection caused by HIV, one of which is marked by a decrease in the number of CD4 + T lymphocytes and a decrease in IFN-GAMMA level which can accelerate viral replication to the point of immune system failure. NAC, an antioxidant, is expected to improve this condition. This research aimed to determine the effect of NAC on IFN-GAMMA levels in HIV / AIDS patients in Dr. Mohammad HoesinCentral Hospital, Palembang. This research was a randomized double blind clinical trial. The samples were 32 HIV / AIDS patients who were undergoing ARV treatment at Dr. Mohammad Hoesin Central Hospital, Palembang, which were randomly divided into 2 groups; the placebo and the NAC groups. In the placebo group, patients were given capsules containing lactose at a dose of 3x1 capsules / day, while patients in the NAC group were given NAC at a dose of 3 x 200 mg / day. Each group was given the same treatment for 12 weeks. The patients’ blood were taken before and after treatment to check their IFN-GAMMA levels. The results showed that both groups did not show any significant changes in IFN-GAMMA levels after 12 weeks of treatment. The conclusion of this researchis that NAC administration did not affect IFN-GAMMA levels in HIV / AIDS patients who were undergoing ARV treatment.

Abstract

Introduction. Old age is related to geriatric syndrome where there are several health problems that often occur related to a decrease in body function and an increase in the inflammatory process. Decrease in muscle mass, muscle strength and physical performance will cause a condition of sarcopenia and frailty syndrome in the elderly. The importance of prevention so that sarcopenia does not occur in the elderly by evaluating the factors that can cause it, one of which is controlling interleukin 6 (IL-6) as proinflammatory. The purpose of this study was to determine the correlation between serum IL-6 levels with muscle mass, muscle strength and physical performance in the elderly gymnastics-community in Mohammad Hoesin General Hospital, Palembang. Methods. This research is an observational analytic study with a cross-sectional approach which was conducted at the Integrated Geriatric Clinic Internal Medicine RSMH Palembang from November 2019 to November 2020. A sample of 31 people aged > 60 years was examined for muscle mass, muscle strength, physical performance and IL-levels. 6 serum. All data processing and analysis in this study used the SPSS version 25 for Windows program. Results. of the 31 subjects, 27 were female (87.1%) and 4 male (12.9%). The mean value of muscle mass is37.9 kg/m2, muscle strength 23 kg, and physical performance 6.1 m/sec.There were 28 (90.3%) samples with elevated serum IL-6 levels, while 3 (9.7%) samples had normal levels. There was a significant negative correlation (p = 0.000) with a strong correlation strength (r = -0.722) between serum IL-6 levels and muscle mass and a significant negative correlation (p = 0.050) with a weak correlation strength (r = -0.354) between IL levels -6 serums with physical performance. Conclusions. There was a significant negative correlation between serum IL-6 levels and muscle mass and physical performance. So, it is very important to prevent an increase in serum IL-6 levels so that the elderly do not fall into a state of sarcopenia.

Abstract

Background. Systemic lupus erythematosus (SLE) is a prototypical autoimmune disease with various significant diseases, where SLE can affect the entire population in the world. This study aims to compare and analyze differences in the composition of the gut microbiota of SLE patients compared to healthy controls based on SCFA examination in Indonesia. Methods. The type of research conducted in this research is an analytical observational study with a case-control design. The research was conducted at Dr. RSUP. Mohammad Hoesin Palembang from October 2020 to October 2021. The sample in this study was divided into two groups, namely, the case group, and the control group. The case group was all SLE patients who met the inclusion criteria, while the control group was a healthy population who did a medical check-up at Dr. RSUP. Mohammad Hoesin Palembang. SCFA examination was carried out using Gas Chromatography-Mass Spectrometry (GCMS) from fecal samples. Results. The results showed that there were two groups of SCFA values, namely the normal group and the microbiota dysbiosis group where the SCFA value was low or less than 4. in the SLE and normal populations where there were 6 SLE subjects who had normal SCFA values or 37.5% and there were 10 SLE subjects who experienced microbiota dysbiosis. or 62.5%. In the healthy population group, all subjects had SCFA values that were included in the normal category, namely 16 subjects or 100%. Based on the severity of SLE using the SLEDAI MEX score, all SLE patients in the study were in the active or category flare where the SLEDAI MEX score was > 5. Conclusion. Patients with microbiota dysbiosis tended to have an LES of 7,222 or 7 times greater than patients who did not have microbiota dysbiosis or had normal SCFA values.

Abstract

Background. COVID-19 infection, both moderate and severe symptoms can cause pneumonia that can be detected on a chest X-ray. Along with the increasing severity of the clinical picture of COVID 19, it can also trigger a cytokine storm, one of which is an increase in Interleukin-6 levels. This study was conducted to see the correlation between IL-6 levels with clinical features and chest radiographs in patients with COVID-19 Methods. This research is a cross-sectional study using an observational analytic. The samples in this study were confirmed COVID-19 patients who were being treated at Dr. Mohammad Hoesin General Hospital, Palembang. A history and physical examination were performed to determine the clinical picture as well as a chest X-ray and IL-6 levels. Results. There were 31 samples of COVID 19 patients. High levels of IL-6 were found in 26 (83.9%) samples. The clinical picture of the patient was dominated by dyspnea as much as 19 (61.3%), fever 16 (5.6%), low O2 saturation 12 (38.4%) and a chest X-ray of pneumonia 26 (83.9%). There was a significant relationship (P<0.05) between IL-6 levels with clinical features and chest X-rays. The R-value on the thorax with pneumonia (0.692), lesion area (0.711) and clinical features with temperature (0.906), respiratory rate (0.706) indicated a strong correlation, while O2 (-0.732) indicated a strong negative correlation. Conclusion. There is a significantly strong correlation between Interleukin 6 levels with clinical features and chest X-rays in patients with COVID-19.

Abstract

Background. SLE is an autoimmune disease characterized by the presence of autoantibodies in the nucleus, deposition of immune complexes, and can attack various body tissues. Gut biota in this case short-chain fatty acids (SCFA) play a role in the inflammatory process in the SLE, while the levels of IL- 6 can also be associated with a response to inflammation which will be seen in the degree of SLE activity. This study aims to determine the correlation of SCFA Gut biota and IL - 6 with SLE degree activity. Methods. This study is a cross-sectional study with a correlation test design, conducted from November 2021-January 2022 at RSMH Palembang with the research subjects being all SLE patients seeking treatment at the Allergy-Immunology Division of RSMH Palembang with categories of mild and moderate-severe SLE activity degree. Examination of SCFA Gut biota using stool samples and serum IL-6 levels were associated with SLE activity degree. Statistical analysis of the correlation test with Spearman for numerical data not normally distributed, and continued with linear regression test to assess the multivariate analysis in this study. Results. The sample consisted of 32 patients, every 16 patients with mild and moderate-severe SLE activity degrees. The correlation between SCFA Gut biota with SLE activity degree was found to have a correlation coefficient of r=-0.777 with p=0.000. Correlation between IL-6 with SLE activity degree obtained a correlation coefficient of r=0.910 with p=0.000, while the correlation test between IL-6 and SCFA Gut biota obtained r=-0.633 with a value of p=0.000. Multivariate analysis found that 70.5% of SCFA Gut biota and IL-6 affected SLE activity degree. Conclusion. SCFA Gut biota and IL-6 had a significant correlation in statistical tests with the SLE activity degree.

Abstract

Background: The elderly and geriatric population is the population most at risk for complications of COVID-19. Preliminary data show that individuals aged >59 years are estimated to experience five times more severe COVID-19 symptom onset than those aged 30 to 59 years. Nutritional status is believed to play a role in the body's ability to produce antibodies after COVID-19 vaccination. Methods: A cross-sectional analytic observational study to determine the role of nutritional status on levels of IgG SARS-CoV-2 after COVID-19 Vaccination in Palembang. IgG levels were assessed by the ECLIA method. Data analysis was carried out univariate and bivariate. Results: The mean quantitative IgG of SARS-CoV-2 in underweight nutritional status was higher with a value of 5757.42±2594.08 U/mL and with the lowest mean quantitative IgG SARS-CoV-2 in overweight nutritional status with a value of 2998.13±4386.95 U/mL. Conclusion: Nutritional status did not play a role in SARS-CoV-2 IgG levels after the COVID-19 vaccination in Palembang.
