Abstract

Background: Neonatal sepsis is a clinical syndrome on infants aged 28 days or more. According to WHO data on 19th April 2018, estimated 3 million newborns and 1.2 million children suffer from sepsis globally every year. Hematological profile assessment can be used as a clinical guide to diagnose sepsis and early management of patients with early onset, especially in hospitals with limited facilities and the examination data can be obtained faster. Objective: This study was aimed to determine the hematological profile in sepsis patients in neonatal unit at RSUD Dr. Pirngadi Medan. Method: This research was a descriptive cross-sectional design. The research sample was selected by total sampling. Results: The results of the study were found from 74 neonates, 44 (59.5%) were male. Neonates with normal labor were 57 (78.1%) and survived 45 (60.8%). It was found that leukocytes (76.4%), neutrophils (60%), monocytes (87.9%), and hemoglobin (40.3%) were increased. Conclusion: The hematological profile can be used as a clinical assessment of sepsis at the first 24 hours on neonates. Hematological examination found an increased leukocytes, neutrophils, monocytes, and hemoglobin in neonatal sepsis patients at the first 24 hours.

Abstract

Background: Human Immunodeficiency Virus (HIV) is a viral infectious disease that attacks the immune system or white blood cells (lymphocytes). HIV, which belongs to the Retroviridae family, is lymphatic, which can damage the specific immune system, namely T-helper lymphocytes or Cluster of Differentiation 4 (CD4). The CD4 count is normal when it is 500–1400 cells/mm3, which is usually the determinant of whether a patient is infected with HIV or not. Objective: This study aims to assess the comparison of CD4 counts in HIV-TB patients before and after ARV and OAT treatment at Dr. Pirngadi Medan City. Methods: Observational analysis research with a cross-sectional design was conducted at RSUD Dr. Pirngadi Medan City in September 2022. The sample for this study was 20 patient records from the medical records of RSUD Dr. Pirngadi, Medan City. The data were analyzed using an unpaired t-test. Results: The results of the study showed that the most HIV-TB pulmonary patients at RSUD Dr. Pirngadi Medan City were aged 36-45 years (50.0%). The majority of male patients (90.0%) and the domicile of most patients in Medan City are in the Medan Tembung sub-district (15.0%). The results of the unpaired t-test obtained a p-value of 0.022, meaning that there was a significant difference between the CD4 count before and after ARV and OAT treatment in HIV-TB patients at RSUD Dr. Pirngadi Medan City. Conclusion: There were significant differences in CD4 counts before and after ARV and OAT treatment in HIV patients with pulmonary TB co-infection at Dr. Pirngadi, Medan City. Abstract

Acute pancreatitis (AP) is clinically defined as a sudden onset of abdominal pain associated with increased digestive enzymes in the blood and urine. Acute pancreatitis is not necessarily a rare disease, even in children and adolescents and may be life-threatening if it is severe. Therefore, acute pancreatitis should always be considered during the differential diagnosis of prolonged and reccurent abdominal pain in children, and appropriate treatment should be started promptly when necessary. In general, laboratory tests for the diagnosis of AP are not specific, yet amylase and lipase enzyme examination remain helpful to rule out the diagnose. In this report, we present a recurrent abdominal pain 3 years old girl with acute pancreatitis.

Abstract

During the child and adolescent years, internet and social media use has a significant impact on social, emotional and mental health development. UN research shows that 47 percent of the world's population uses the internet, with one-third of them under the age of 18. Intense social media use, such as checking more than 10 times per day, can have negative impacts, including threats to personal safety, sleep disturbances and addiction. However, the internet also provides extensive access to information and strengthens social relationships. The role of pediatricians is crucial in educating parents and providing wise guidance on children's use of the internet and social media, including supervising, setting limits, and providing information on cyberbullying and sexual harassment. With proper supervision, children and adolescents can utilize the internet and social media positively without harming their mental health and development.
