Abstract

Abstrak Penanganan COVID-19 saat ini secara simtomatik dan suportif berdasarkan pemantauan klinis karena pencegahan ataupun pengobatan COVID-19 yang efektif masih dalam proses pengembangan. Karena itu perlu dilakukan pemantauan terhadap faktor-faktor yang dapat mempengaruhi keparahan infeksi COVID-19 dalam upaya tatalaksana penderita dengan tepat untuk mengurangi kemungkinan perburukan klinis yang lebih parah. Beberapa kondisi klinis, kelainan pemeriksaan laboratorium maupun radiologi yang dapat meningkatkan risiko keparahan COVID-19 telah diobservasi. Pemeriksaan virologi SARS CoV-2 sebagai patogen kausal terus ditelusuri, terutama dalam korelasinya dengan keparahan infeksi COVID-19. Studi virologi SARS CoV-2 sebelumnya dari tinjauan literatur menunjukkan bahwa mutasi G614 pada protein Spike dan viral load tergantung dari lokasi pengambilan sampel, didukung dengan uji kultur, berkorelasi signifikan untuk pemantauan kondisi keparahan COVID-19. Hasil pemeriksaan virologi mengenai durasi dan clearance time SARS CoV-2 yang lebih lama secara bermakna pada kasus berat, dapat memberi masukan awal bahwa pada tahap infeksi COVID-19 yang parah diperlukan penguatan manajemen yang mendukung pencegahan penyebaran virus, sehubungan dengan pemantauan dan pengendalian infeksi penyakit yang dapat berlangsung lebih lama. Abstract The treatment of COVID-19 presently evolves around symptomatic and supportive management based on clinical monitoring because effective prevention or therapy of COVID-19 is still under development. Therefore it is necessary to monitor the factors that can affect the severity of the COVID-19 infection in attempting to manage it appropriately and keep the patients from going through more severe clinical deterioration. Several clinical conditions, laboratory and radiological abnormalities that can increase the risk of COVID-19 severity have been observed. The virological examination of SARS CoV-2 as the causal pathogen continues to be investigated, especially in its relation with the severity of COVID-19 infection. Previous SARS CoV-2 virological studies from the literature review showed that G614 mutation in spike protein and viral load depend on sampling location, supported by culture tests have a significant correlation for monitoring of COVID-19 severity. The results of virological examinations regarding the duration and clearance time of SARS CoV-2 which is significantly longer in severe cases can provide initial suggestion that it is necessary to strengthen management that supports the prevention of the spread of the virus at the stage of severe COVID-19 infection, corresponding with monitoring and control of disease infections that may last longer.
