Abstract

Prevalence of type 2 diabetes mellitus and chronic kidney disease is increasing nowdays. Cystatin C is a more sensitive marker in detecting kidney disturbance at an early stage. Microalbuminuria indicates that there is an increasing risk in kidney disorders in diabetes mellitus patients. HbA1c is used to assess average blood glucose in three (3) months. An evaluation about the relation among Cystatin C, HbA1c, and ACR is needed to decide which test will be appropriate for the patient. Cystatin C was determined using Particleenhanced nephelometric immunoassay (PENIA) method from BN Prospec. High Performance Liquid Chromatography (HPLC) method from BioRad was used for HbA1c test, while random urine sample was used for Albumin Creatinine Ratio (ACR) test. Urine and serum creatinine were tested by immunoturbidimetric assay from Cobas C501. Correlation among Cystatin C, HbA1c, and ACR was estimated using Spearman’s correlation test. There was a significant correlation among Cystatin, HbA1c, and ACR. Spearman correlation value showed a positive weak correlation. There was a strong relation between Cystatin C and HbA1c, but weak relations between Cystatin C and ACR, and between HbA1c and ACR. It can be concluded that due to the weak correlation between ACR and kidney function as well as between ACR and HbA1c, ACR is not a good assessment to predict diabetic nephropathy progressiveness.

Abstract

The prevalence of chronic kidney disease (CKD) is increasing. Hypertension is an established risk factor of CKD, yet found prevalent in Indonesia. To date, there is no test that can be used as predictor of , especially hypertensive patients. Recently, the relationship between aldosterone level (plasma aldosterone concentration/PAC) with hypertension and CKD has been extensively studied. Advanced studies have found that aldosterone/renin ratio (ARR) can be a promising parameter to predict the occurrence of CKD. This result brings hope for hypertensive patients, as we are able to predict not only the occurrence of CKD, but also the probability of cardio- and/or cerebrovascular events. Particularly for East Asia ethnic groups that are known to consume a relatively high amount of salt, ARR could also be a marker of salt intake sensitivity that may lead to a proper salt restriction in daily diet.

Abstract

Diagnosis tuberkulosis laten (latent tuberculosis infection/LTBI) merupakan kunci penting dalam pengendalian kejadian tuberkulosis (TB). Uji interferon gamma release assay (IGRA) umumnya digunakan untuk mendeteksi interferon gamma (IFN-γ) sebagai penanda LTBI, dengan metode seperti T-SPOT.TB yang telah diimplementasikan secara luas. Akhir-akhir ini, metode chemiluminescent immunoassay (CLIA) dengan analyzer yang sepenuhnya otomatis, juga telah digunakan untuk deteksi dan kuantifikasi IFN-γ. Penelitian ini bertujuan untuk membandingkan antara kedua metode tersebut. Dua puluh sampel darah heparin dari 20 pasien diambil di Laboratorium Rampal Diagnostika. Hasil penelitian ini menunjukkan bahwa metode otomatis CLIA sebanding dengan metode T-SPOT.TB dalam mendeteksi IFN-γ dengan signifikansi 0,625 dan korelasi 0,857. Terdapat kasus yang tidak sesuai antara kedua metode sebanyak 4/20 (20%). Dapat disimpulkan bahwa CLIA-IGRA merupakan pemeriksaan yang sebanding dengan metode T-SPOT.TB untuk mendeteksi LTBI.
