Abstract

Nursing care for clients with hypertension is a series of nursing processes in overcoming problems with clients. Hypertension is systolic blood pressure > 140 mmHg and diastolic blood pressure > 90 mmHg. Nursing care is carried out to prevent and reduce mortality in the future. The purpose of this study was to obtain an overview of the implementation of nursing care for clients who experience acute pain with hypertension at RSAU dr. Esnawan Antariksa, East Jakarta. This research design uses descriptive qualitative research with a case study and intervention approach. Participants in this study were 2 (two) clients and their families with acute pain nursing problems and medical diagnoses of hypertension. The characteristics of the participants were the same sex, age 45–64 years, blood pressure >140/90mmHg, pain complaints on a moderate scale (4-6) and a severe scale (7-9). After nursing action on client 1, 3 nursing diagnoses were obtained with the criteria for the problem resolved, namely: acute pain associated with increased cerebral vascular pressure, activity intolerance related to general weakness, and knowledge deficit related to hypertension related to lack of information obtained. While on client 2, there were 3 nursing diagnoses with the criteria for resolved problems, namely: acute pain associated with increased cerebral vascular pressure, weight was more related to adequate nutritional intake, and knowledge deficits about hypertension related to lack of information obtained. The results of giving breath relaxation actions can reduce pain levels in clients with hypertension.

Abstract

Diabetes Melitus merupakan sekumpulan gangguan metabolik yang ditandai dengan peningkatan kadar glukosa darah (hiperglikemia) akibat kerusakan sekresi insulin, kerja insulin atau keduanya. Berdasarkan penelitian data yang di dapatkan dari rekam medik di RSAU dr. Esnawan Antariksa Jakarta Timur pada bulan November dan Desember 2017 diabetes melitus termasuk dalam jumlah terbanyak yang memperoleh presentase 14,65%. Sedangkan terdapat 7 orang atau sekitar 15,05% klien diabetes melitus dari 30 klien yang ada di ruang Merpati selama penelitian. Studi kasus ini bertujuan untuk mendapatkan gambaran pelaksanaan asuhan keperawatan pada klien yang mengalami defisit nutrisi dengan diabetes melitus di RSAU dr. Esnawan Antariksa Jakarta Timur. Desain penelitian yang digunakan adalah kualitatif deskriptif dengan intervensi melalui pendekatan studi kasus. Partisipan dalam studi kasus ini meliputi klien, keluarga dan perawat ruangan, dengan diagnosa medis dan masalah keperawatan yang sama yaitu klien yang mengalami defisit nutrisi dengan diabetes melitus. Intervensi utama yang dilakukan pada kedua klien adalah observasi intake nutrisi dan menimbang berat badan. Hasil penelitian menunjukkan pada kedua klien, masalah defisit nutrisi dapat teratasi dengan adanya peningkatan berat badan, nafsu makan klien baik, dan tidak ada tanda-tanda mal nutrisi pada kedua klien.

Abstract

Dengue hemorrhagic fever (DHF) is a very common disease in Indonesia and other tropical countries. This disease is more common in urban and sub-urban areas. In 2013 there were reported as many as 2.35 million cases in America, of which 33,687 cases were severe dengue. (WHO, 2016). The case fatality rate (CFR) in the early years of dengue cases spreading in Indonesia was very high. Then from year to year it began to decline from 41.4% in 1968 and continued to decline to 0.89% in 2009. The purpose of this study was to obtain an overview of the implementation of nursing care for clients experiencing Hyperthermia with Dengue Haemorragic Fever at Bhakti Asih Hospital. The design of this research is descriptive qualitative without intervention. This research was carried out at the Bhakti Asih Hospital, when the research was carried out on July 10-13 2019. A literature review was carried out both in print and online. Data analysis using tri angulation. The assessment on Mr. R who had DHF had symptoms of an increase in body temperature of 38.80C, with symptoms of fluid deficiency. There are some theoretical signs and symptoms that do not appear, namely seizures and decreased consciousness. At this stage of diagnosis, 4 nursing diagnoses were found. At the planning stage, the priority diagnosis equations were found in theory. No special implementation was carried out in the case of Mr. R. At the evaluation stage, based on the results of previous studies, it was stated that giving warm compresses was very effective in reducing hyperthermia.
