Abstract

Diabetes Mellitus (DM) is a chronic metabolic disorder that will be suffered for a lifetime with the disease process continuing so that at one time it can cause complications. Comprehensive treatment must be carried out so that blood glucose levels in DM patients can be controlled and complications can be prevented as early as possible. Control of blood glucose in DM must be carried out through 5 pillars which include regulating diet, exercise, medication, monitoring blood glucose levels and education. The role of nurses in caring for DM patients includes the roles of care givers, counselors, educators, advocates who are oriented to the needs and health of patients. Nurses who act as educators and counselors in increasing the independence of patients and families to carry out integrated DM management. This role is carried out by carrying out nursing interventions regarding nursing management of DM patients. Efforts are needed to improve the competence of nurses in caring for DM patients. The solution to the problem is in the form of efforts to increase the knowledge and skills of nurses regarding the management of diabetes mellitus nursing care based on IDHS, SLKI and SIKI through training and mentoring and equipped with the distribution of DM care smart books for nurses. The target of this activity is nurses who work in Tugu Ibu hospitals in Cimanggis. The purpose of this activity is to improve the quality of care for DM patients. The output of this activity is an increase in the knowledge and skills of nurses in conducting assessments, determining diagnoses, planning, carrying out nursing actions and evaluating DM patients based on the SDKI, SLKI and SIKI which ultimately improves the quality of nursing care in DM patients. Abstract

Background: Diabetes Mellitus (DM) is a clinical syndrome of impaired metabolic secretion, insulin action, or both characterized by hyperglycemia. Various nursing problems can occur due to hyperglycemic conditions that are not handled properly. The instability of blood glucose levels is a major problem that occurs in uncontrolled DM patients. This condition can lead to various complications and complications so comprehensive treatment management is needed. Objectives: This study aims to explore and analyze the nursing management of unstable blood glucose levels in type 2 DM patients who were hospitalized. Methods: This study is a qualitative study using an exploratory case study method in diabetes mellitus patients type 2 who were hospitalized. The data obtained were processed and analyzed based on theories that explain the occurrence of the problem of instability in blood glucose levels and the nursing management carried out in patients with type 2 diabetes mellitus. Results: The results of this study indicate that the main problem in patients with type 2 diabetes mellitus is the instability of blood glucose levels which is characterized by hyperglycemia. Nursing management carried out is monitoring blood glucose level laboratory results, management of hyperglycemia, medication management, nutrition/diet management, treatment, and care education programs can reduce blood glucose levels. Conclusion: The problem of blood glucose instability is a priority problem in type 2 diabetes mellitus and comprehensive nursing management will determine the success of controlling blood glucose levels so that they are normal and stable.

Abstract

Nyeri pascaoperasi abdomen merupakan masalah prioritas yang harus dikelola dengan baik. Tantangan pengelolaan nyeri pascaoperasi abdomen membutuhkan penilaian individu yang komprehensif, pendekatan dan protokol yang fleksibel, koordinasi tim yang efisien untuk memberikan perawatan yang optimal. Pendekatan konvensional seringkali kurang efektif dalam mengatasi rasa nyeri secara optimal. Pendekatan multimodal dalam pengelolaan nyeri adalah penggunaan berbagai metode atau teknik secara simultan untuk mengatasi nyeri secara  lebih efektif. Pendekatan multimodalmengombinasikan intervensi farmakologis dan non-farmakologis, menawarkan potensi untuk mengurangi nyeri lebih efektif. Penelitian ini bertujuan untuk mengevaluasi efektivitas pendekatan multimodal yang melibatkan relaksasi otot progresif, dukungan psikologis dan penggunaan analgesik dalam menurunkan nyeri pasien pascaoperasi abdomen. Penelitian ini menggunakan desain quasi eksperimental prates dan pascates dengan group kontrol. Responden sebanyak 60 pasien pascaoperasi abdomen dibagi menjadi dua kelompok, kelompok intervensi (n=30) yang menerima pendekatan multimodal dan kelompok kontrol (n=30) yang hanya menerima analgetik standar. Tingkat nyeri diukur menggunakan skala Visual Analog Scale (VAS) pada hari pertama dan kedua pascaoperasi. Analisis data dilakukan menggunakan uji Paired t-Test dan uji Pooled t-Test. Hasil uji Paired t-Test menunjukkan rata-rata rasa nyeri sebelum intervensi antara kelompok intervensi dengan kelompok kontrol berbeda tapi tidak bermakna (p=0,40), namun setelah intervensi menunjukkan berbeda secara bermakna pada kelompok intervensi maupun kelompok kontrol (p=0,03). Hasil uji Pooled t-test menunjukkan adanya perbedaan yang bermakna penurunan rata-rata rasa nyeri antara kelompok intervensi dengan kelompok kontrol (p=0,000, α=0,05). Kesimpulan: Pendekatan multimodal yang menggabungkan relaksasi otot progresif, dukungan psikologis dan analgesik terbukti lebih efektif dalam menurunkan nyeri pascaoperasi abdomen dibandingkan dengan penggunaan analgesik saja. Rekomendasi: Perlu pengembangan protokol perawatan standar yang mencakup intervensi multimodal sebagai bagian dari manajemen nyeri pascaoperasi, khususnya operasi besar seperti operasi abdomen.
