Abstract

Germ cell tumours of the central nervous system infiltrating the ipsilateral extracranial tissues are not a common presentation. A case of intracranial germ cell tumour with extracranial extension in a four month old infant is reported. The patient had undergone excision of the extracranial lesion followed by combination chemotherapy. Abstract

Many of breast-cancer patients experience distress and most of them experience depression which may lead to amplification of physical symptoms, increased functional impairment, and poor treatment adherence. We did a review on available literature from PubMed about prevalence, distress magnitudes, coping styles, and treatment methods of major depression in women with breast cancer from 1978 to 2010. Diagnosis and treatment of depressive episodes in women with breast cancer is challenging because of overlapping symptoms and co-morbid conditions. Major depression is often under-recognized and undertreated among breast cancer patients. This review highlighted the issues on identifying and managing depression in breast cancer patients in clinical settings. Abstract

A 27 year old male patient with chronic myeloid leukemia under irregular hydroxyurea treatment presented with a left thigh swelling associated with a dull aching pain. Blood examination revealed a total leukocyte count [TLC] of 125,000/mm3 with 13% metamyelocytes, 8% myelocytes, 1% promyelocytes and 3% band cells. A fine needle aspiration biopsy of the swelling revealed mostly myeloblasts with occasional promyelocytes. He was maintained on tablet hydroxyurea 2g given in divided daily doses along with allopurinol, systemic alkaliser and plenty of oral fluids. Gradual symptomatic relief was achieved over next 2 months with no palpable lump after 3.5 months. TLC was 38,400/mm3 with 1% promyelocytes and myeloblasts and 5% band form. There was reduction in the liver size during this period but no reduction in spleen size. He is currently being maintained on hydroxyurea 2g with other supportive care and has not had any recurrence of symptoms for the last 9 months.

Abstract

A case of Langerhans Cell Histiocytosis (LCH) in a 58 year old woman who presented with bilateral cervical adenopathy without any other complaints is reported. Clinical examination did not reveal any other positive finding except the adenopathy. Histopathological examination confirmed the node to be Langerhans cell histiocytosis. In conclusion, LCH is a rare disease with varied clinical presentation, including cervical adenopathy. Diagnosis should be confirmed by histopathological examination of biopsy specimen. Observation may be an effective management strategy in cases of adult LCH with cervical adenopathy as the only symptom, due to its possible spontaneous regression. Abstract

A 43-year old male presented with persistent discomfort and pain upper abdomen (epigastrium) more on left side associated with fever on and off, along with fatigue and loss of appetite for the last four months. Physical examination revealed mass on left hypochondrium extending to epigastrium with mild distension of the abdomen. Imaging studies of the patient showed dextrocardia on chest x-ray  postero-anterior (PA) view, thoracic and abdominal CT scan showed situs inversus totalis with multiple SOL (space occupying lesion) in right lobe of liver with largest measuring 8x6 cm2 in the 4th segment. USG-guided FNAC of the mass showed features of hepatocellular carcinoma. Thereupon, hepatocellular carcinoma in situs inversus totalis was diagosed to this patient and was clinically staged as T3aN0M0. He was given sorafenib 400 mg orally twice daily with an advice to come for regular assessment every 4 week.Keywords: hepatocellular carcinoma, situs inversus totalis, case report
