Abstract

Background : Hypertension is one of the most common worldwide diseases that can be an important public health challenge. The causes of hypertension are high consumption of salt and fat, obesity, stress, age, sex, family history, smoking, and consumption of alcoholic drinks. The prevalence of hypertension in the world, Asia, and Indonesia are 15-20%, 8-18%, and 1,8-28,6%, respectively. The solution to this problem is not only depend on drugs but also counseling through leaflet as the process of assisting clients in facing the problem of hypertension. Objectives : To know the effect of nutrition counseling on nutrition intake and blood pressure of hypertension outpatients at local hospital of the Province of Southeast Sulawesi. Methods : The study was quasi experimental with pre- and post-control design. Samples were divided into 2 groups; intervention group was given nutrition counseling and leaflet and the second (control) was only given leaflet. Samples were hypertension outpatients who fulfilled inclusions and exclusions criteria, and there were 25 samples per group. Data of nutrient intake were processed using Nutrisurvey. Chi-square test was used to identify the difference between the intervention group and the control group; whereas t-test was used to identify independent as well as dependent variables. Results : There were no significant differences in intake of fat, natrium, potassium, and magnesium between the intervention group and control group (p > 0.05); but there were differences between them at the end of study (p < 0.05). Blood pressures of both of them were also not different (p > 0.05); however, at the end of study there was difference between them (p < 0.05).Conclusion : Nutrition counseling could improve nutrition intake and blood pressure of hypertension outpatients at local hospital of the Province of Southeast Sulawesi.

Abstract

Background: Coronary heart disease is a health problem both in developed and developing countries including Indonesia. The result of household health survey in 1992 and 1995 indicated that coronary heart disease was the first cause of death with the prevalence as much as 16.5% and 19% subsequently. Coronary heart disease is caused by many factors such as hypertension, hyperlipidemia, obesity, smoking and diabetes mellitus. High intake of unsaturated fat for a long time can increase blood lipid level. Efforts to lowering blood cholesterol can be made by managing macro nutrient intake as well as micro nutrient intake such as vitamin B3, C, E, and fibers. Objective: To find out the relationship between intake of vitamin B3 (niacin), C, E and fibers and dyslipidemia in coronary heart disease at dr. Mohammad Hoesin Hospital of Palembang.Method: The study was analytic observational which used cross sectional design. Subject of the study were dyslipidemia patients of coronary heart disease who were newly diagnosed. There were as many as 50 patients who fulfilled inclusion and exclusion criteria and were purposively chosen. The study was carried out at cardiology room from October 2007 to February 2008. Data of intake were obtained from questionnaires of quantitative food frequency and processed using computer. Chi square test was used to identify the relationship between intake and lipid profile; Spearman rho correlation was used to find out the level of relationship; and odds ratio was used to identify the risk.Result: There was significant relationship between intake of vitamin B3, C, E and fibers and total cholesterol (p < α). There was significant relationship between intake of vitamin E and triglyceride (p = 0.015). There was significant relationship between intake of vitamin B3, E and fibers and high density lipoprotein cholesterol (p < α). There was significant relationship between intake of vitamin B3, C, E and fibers and low density lipoprotein cholesterol (p < α).Conclusion: There was relationship between low intake of vitamin B3, C, E and fibers of dyslipidemia patients in coronary heart disease.

Abstract

Background: Aging that happens naturally in human life does not only cause physical dysfunction but also have an impact to mental and social aspects. In the elderly there is a problem of degenerative disease. Hypertension has become a serious health problem and a major challenge of public health worldwide because of either high prevalence or major risk factor for cardiovascular disease. Pharmacological as well as non-pharmacological efforts have been made to prevent and cure the disease; however until today the growing number of hypertensive patients has not been successfully controlled. Consequently behavioral intervention has to be made to cure hypertensive patients. One of the recommended interventions is fitness exercise for the elderly. Objective: The study aimed to identify the effect of fitness exercise to increasing stamina of heart and lung, nutrition status and reduced blood pressure of hypertensive elderly at integrated service post of Sub district of Pahandut, Palangkaraya Municipality.Method: The study was an experiment (before and after) without control group using one group pretest posttest study design. Subject of the study was a group with pre experimental, evaluation, effect of variables and post experimental evaluation. Measurement was made in week two, three and four to blood pressure, heart and lung stamina, nutrition status of hypertensive elderly, discipline in exercise and food recall 24 hours. The interventions made were lecture and fitness exercise package D for the elderly. Results: There was significant association between elderly fitness exercise and systolic blood pressure, heart and lung stamina and nutrition status of hypertensive elderly (p<0.05). Result of Wilcoxon signed ranks test and paired sample test between gymnastic elderly on increase of endure capacity heart lung are significantly associated (p=0,001) and so on nutritional status (p=0,002) and decrease systolic tension (p=0,001) and diastolic tension (p=0,002).Conclusion: There was significant association between elderly fitness exercise and the increase of heart and lung stamina, nutrition status and the decrease of blood pressure in hypertensive elderly.
