Abstract

Intoduction : The assessment of level severity in patient with community acquired pneumonia (CAP) is very important to determine the next management of disease. Procalcitonin (PCT) is known as one of biomarker sepsis and infection. The application of PCT is known to be used in diagnosis, to help clinician to decide antibiotic treatment and to make prognosis. To determine the correlation between PCT and CURB-65 score in CAP patients at the early admission in hospital with 30-day mortality.Method : This was an cohort study. We had examined CAP subject with CURB-65(Confusion, Urea, Respiratory rate, Blood pressure, Age >65 years), PCT, other laboratory assessment and sputum and blood culture at the early admission at emergency room (ER). We had correlate the PCT levels with CURB-65 to determined prognostic utility of PCT and 30-day mortality. Results: In total, 57 subjects with CAP were enrolled in the study and subjects were follow-up for 30 days for survival. In 20 subjects (47.5%) who died during the follow-up,17 subjects (41.9%) died with PCT level • 0.25 ng/ml with severe CURB-65 score and 2 subjects (4.9%) with low-moderate CURB-65 score, but in PCT level >0.25 ng/ml with low-moderate CURB-65 score, 1 subject (6.3%) was died and no subject with severe CURB-65 score and total 30-day mortality were 20 subjects. We had found correlation between CURB-65 score and PCT level with 30-day mortality using pearson correlation (P = 0.0001). Conclusion : Procalcitonin is a biomarker of bacteria infection that has correlation with clinical scoring system CURB-65 and PCT level to determine 30-day mortality. PCT can be use to determine the prognosis in CAP at early admission.Keywords : community acquired pneumonia; CURB-65 score; procalcitonin; prognosis
