Abstract

Objective: To evaluate the association between testosterone deprivation and atheroplaque formation, testosterone receptors, and collagenization in Wistar penile tissues. Material & method: Two months after orchiectomized bilaterally, penile tissue of eight Wistar were harvested for semi-quantitative analysis of atheroplaque formation using hematoxyllin-eosin staining. Similar analysis were also conducted to look for collagenization by collagenase staining as well as the expression of testosterone receptors by immunohistochemistry staining. Control group were eight Wistar underwent Sham surgery. Statistical analysis for atheroplaque formation in Wistar compare to control group used was Chi-square, Fishers’ exact test was used for the expression of testosterone receptors, and independent t-tests for evaluating collagenization. Results: A significant decline in the testosterone receptors and increase collagenization were obtained in orchiectomized Wistar, while atheroplaque formation was not significantly different. Conclusion: Testosterone deprivation revealed a significant decline in the expression of testosterone receptors, and a significant increase in collagenization while atheroplaque formation was not significantly changed. Abstract

Objective: To present a rare case of infiltrating urothelial carcinoma along the urinary tract on a young adult. Case(s) Presentation: A 39 year old Asian male patient with > 20 pack-year smoking history, came to the hospital with a chief complaint of hematuria and flank pain since two weeks ago. Physical examination revealed Eastern Cooperative Oncology Group (ECOG) Performance Status 1, the right kidney was palpated, no tenderness on fist percussion. Abdominal ultrasound (USG) revealed 1.5 cm and 0.5 cm masses located in the urinary bladder. The CT scan show mass in the bladder was exophytic, > 3 cm in size, and occupying lesion was located on the right posteroinferior and left lateral bladder wall accompanied with dilatation of pelvic calyces and right ureter. Histopathology of the specimen revealed low-grade infiltrating urothelial carcinoma of bladder and right ureter. The pathological stage was pT2NxMx, The patient refused neoadjuvant chemotherapy and surgery. Four months after TURBT, he came with weakness and right flank pain. The patient consent to surgery and underwent the right nephroureterectomy. Histopathology of the specimen showed infiltrating urothelial carcinoma of right kidney, ureter, Gerota fascia with lymphovascular invasion (pT3NxMx). Discussion: Urothelial carcinoma (UC) is commonly arising in the urinary bladder, but it can develop along the urinary tract. Cigarette smoke contains a lot of carcinogenic agents and stimulates DNA damage. Conclusion: Upper tract urothelial carcinoma (UTUCs) is subset of UC with a poor prognosis. Cigarette smoking is the main risk factor that induces DNA damage.

Abstract

Objective: This study examines quality of life outcomes in post-ischemic stroke patients with urinary disorders at Hasan Sadikin Hospital Bandung in 2023. Material & Methods: This retrospective study analyzed medical registry data from a single center. Quality of life was evaluated using EORTC Core Quality of Life Questionnaire-30 (EORTC QLQ C-30). Descriptive statistics and Chi-Square/Fisher's Exact tests (95% CI, P < 0.005) were used for clinical and demographic data. Multilinear regression tested the relationship between urinary symptom severity and quality of life. Results: The study included 79 ischemic stroke patients in Hasan Sadikin General Hospital. Patients with urinary disorders had an average age of 52 years (SD ± 13.42), mostly within the 50-60 age group. Hypertension was strongly associated with urinary disorders (P < 0.001). Quality of life decline was highest in role and physical function within the functional domain, and fatigue and insomnia within the symptom domain. Age showed no significant relationship with patient quality of life (P = 0.847 and P = 0.804), but a statistically significant link existed between IPSS total score and quality of life functional outcomes in stroke-ischemic patients. Conclusion: A notable link between urinary disorder severity and the functional domain of quality of life was observed. Further investigation into specific quality of life risk factors related to stroke lesion types is recommended. Keywords: Stroke, ischemia, quality of life, urinary disorders.
