Abstract

Objective: To know the incidence of urinary tract infection (UTI) with different antibiotic prophylaxis for transrectal ultrasonography (TRUS) prostate biopsy. Material & Method: The study included 34 patients at Soetomo Hospital Surabaya, who were divided into 2 groups, each group consisting of 17 patients. In the first group patients received 1000 mg of ciprofloxacin orally, in the second group cefotaxime 1000 mg iv was given prior to biopsy. The two groups were compared in terms of UTI incidence as observed from the blood levels of leukocytes, C-reactive protein (CRP) and urine culture 3 days after the procedure. Results: Based on blood leukocyte levels, there was no statistically significant difference between the two groups (p = 0,74 and p = 0,42). So was the comparison of CRP levels. There was no other significant difference found (p = 0,53 and p = 0,27). From the results of urine culture, the ciprofloxacin group had positive urine culture results lower than the cefotaxime group (29,4% : 35,3%), although it was not statistically significant (p = 1,0). Conclusion: Based on the parameters of blood leukocytes levels, CRP and urine culture, there were no differences in the incidence of UTI after biopsy in the two groups.Keywords: TRUS prostate biopsy, UTI, ciprofloxacin, cefotaxime.

Abstract

Objective: This research aims to identify factors influencing the degree of erectile dysfunction in bicycle racing athletes. Material & Methods: This study used quantitative analysis and cross-sectional research design. Samples included in this research were 54 respondents in accordance to the inclusion and exclusion criteria of this study, and were willing to participate in this research. The data were collected using the IIEF-5 (International Index of Erectile Function-5) questionnaire. The data were then edited, coded and analyzed using Chi-square (bivariate analysis). Results: The results showed correlations between the degree of erectile dysfunction in bicycle race athletes and age (PR= 1.182; 95%CI= 0.403-3.465; p= 0.976), cycling history (PR= 0.462; 95% CI= 0.490-4363; p= 0.687), cycling frequency (PR=1.400; 95% CI= 0.299-6.560; p= 0.72), cycling duration (PR= 0.333; 95% CI= 0.074-1.507; p= 0.165), furthest cycling distance (PR= 0.883; 95% CI= 0.267-2.919; p= 1.00), and type of saddle (PR= 0.271; 95% CI= 0.481-1.547; p= 0.221) respectively. Conclusion: There were no significant correlation (p= >0.05) in all variables analyzed in this study.

Abstract

Objective: Excessive use of MSG in everyday life can cause infertility to men. Exposure to MSG can cause cells to experience oxidative stress which subsequently triggers the generation of free radicals. Free radicals can be resisted with antioxidants. Material & Methods: This research was a laboratory experimental test with a post-test only control group design using male mice as the experimental object. The statistical test used the One Way ANOVA test. Results: In this research, 5 male mice were assigned into 4 groups and 1 control group. Using the One Way ANOVA test, it was found that there were significant differences in ≥ 2 treatment groups in the motility group of spermatozoa with active movement (p < 0.001) and the motility group of spermatozoa with weak movement (p = 0.036). The two groups showed significant differences. The Post hoc LSD test in the motility group with active movement showed significant differences between the control group with MSG treatment (p < 0.001) and the treatment group given Moringa leaves extract of 600 mg/kgW (p < 0.001) and 1200 mg/kgW (p < 0.001). Conclusion: Moringa leaves extract can effect the improvement of the motility of spermatozoa exposed to monosodium glutamate at a given dose of 300 mg/kgW and 600 mg/kgW.

Abstract

Background: Testicular torsion is an emergency situation and requires immediate surgery. Testicular torsion accounts for 13-54% of cases of acute pediatric scrotal disease and occurs in 1 in 4,000 men aged <25 years and 1 in 160 men over 25 years. Even though the intervention was carried out for less than 6 hours, post-detortion testicular tissue mortality was still high, due to reperfusion ischemic injury due to excessive production of ROS (Reactive Oxygen Species). The contralateral testis is often affected after torsion-detortion due to the effects of ROS. The provision of additional therapy after detortion is an urgent need to be found because it can reduce the death of testicular tissue due to reperfusion ischemic injury. Methods: It is an experimental study with a randomized controlled study design with post test only control group design. The research sample was 18 Wistar rats divided into 3 groups. Torsion of the ipsilateral testis is performed, then detortion and intravenous therapy are administered. The apoptotic index (infertility) was observed on the Johnsen score variable, the number of seminiferous tubular cell layers, neutrophil adhesion and the number of necrotic cells in the Eosin Hematoxyline in the contralateral testis. Furthermore, statistical analysis is carried out, the results are presented in tables and figures. Results: There was a significant difference between the Methylprednisolone group and the Torsion Detortion group with p = 0.008 in the Johnsen score variable and the number of seminiferous tubular cell layers. Discussion: Methylprednisolone as an anti-inflammatory has been proven in its function to reduce ROS so that it can reduce reperfusion ischemic injuries.

Abstract

Latar Belakang : Spondilitis ankilosis merupakan penyakit peradangan sendi yang bisa mempengaruhi tulang-tulang axial yang dapat menyebabkan nyeri sendi, dan dapat berlanjut kepada penyatuan dari tulang belakang1. Prevalensi penyakit ini diperkirakan mencapai 16,7 dari 10.000 orang di daerah Asia.2 Meskipun penyakit ini utamanya menyerang sendi, tetapi dilaporkan pula bahwa terdapat kasus manifestasi ekstraartikuler seperti halnya nefrolitiasis. Nefrolitiasis menyumbang sekitar 13,6% dari penderita spondilitis ankylosis.3 Kasus Nefrolithiasis Staghorn sinistra dengan spondilitis ankilosis pernah dilaporkan, tetapi belum pernah dengan kasus nefrolithiasis staghorn bilateral4 Tujuan studi kasus ini adalah untuk mengetahui diagnosis dan tatalaksana spondilitis ankilosis dan nefrolitiasis staghorn bilateral. Presentasi Kasus : Seorang pria berumur 38 tahun datang dengan keluhan nyeri hilang timbul pada pinggang sebelah kiri. Pasien sebelumnya telah didiagnosis dengan spondilitis ankilosis dan nefrolitiasis bilateral dekstra dan sinistra serta telah menjalani operasi Percutaneous Nephrolithotomy dekstra pada bulan Juni 2018. Pada pemeriksaan didapatkan keterbatasan gerak sendi tulang belakang dan nyeri ketok kostovertebra di sebelah kanan dan kiri. Dari pemeriksaan foto polos didapatkan gambaran batu staghorn bilateral Kesimpulan : Pasien didiagnosis dengan spondilitis ankilosis dan nefrolitiasis staghorn bilateral. Prosedur penegakan diagnosis melalui anamnesis, pemeriksaan fisik dan pemeriksaan penunjang perlu dilakukan dengan tepat untuk menghindarai kesalahan diagnosa. Penatalaksanaan nefrolitiasis perlu memperhatikan banyak aspek seperti ukuran batu, ketersediaan sarana dan prasarana, serta mobilitas pasien, supaya dicapai tingkat kesembuhan yang tinggi.

Abstract

BACKGROUND: Obstructive uropathy and hydronephrosis are common complications of cervical cancer. In order to provide an alternative route due to ureteral obstruction, urinary diversion was performed. Urinary diversion procedures are a therapeutic option for obstructive uropathy but their effectiveness is remain unclear. There is limited data that support urinary diversion as a treatment of ureteral obstruction to improve overall survival and progression-free survival. AIM:  To determine the effectiveness of urinary diversion in the treatment of obstructive uropathy in cervical cancer patients. METHOD: This study is a cross sectional involved cervical cancer patients with symptoms of obstructive uropathy. Data were obtained from medical records and limited to patients admitted from May, 2020 to May, 2021. Primary outcome was urea level, creatinine level, stage of hydronephrosis and overall survival rate of patients. RESULT: A total of 100 patients were included in the study. In both groups, the mean age of study subjects was 52-53 years with stage IIIB being the most widely reported. There was an improvement in post creatinine in the urinary diversion group. There was a significant difference in post creatinine levels between the non-urine diversion and urinary diversion groups (p=0.039).The 12-month survival rate, in the non-urinary diversion group had a value of 86% while the urinary diversion group had a value of 94%. CONCLUSION: This study shows that urinary diversion procedure is effective in increasing the renal function and increasing the 12-month survival of cervical cancer patients with obstructive uropathy.
