Abstract

Background: Erythema Multiforme (EM) is a mucocutaneous inflammatory reaction with clinical manifestations ranging from mild to severe which are self-limiting but can also be life-threatening. EM is considered an immunological disease in the form of a hypersensitivity reaction caused by an immune response to certain antigens such as herpes simplex virus known as Herpes Associated Erythema Multiforme (HAEM) or certain types of drugs known as Drug Induced Erythema Multiforme (DIEM). Hematinic factors greatly affect the length of the healing process in cases and can also worsen anemia because there is a tendency for bleeding on the lips. Case and Management: This case report discusses the case of EM in a 41-year-old woman who came to the practice with complaints of canker sores and scabs on her lips, disappearing and bleeding easily. The patient's condition is accompanied by chronic anemia with results of Hb, MCV, MCH, MCHC (below the reference value). Anti HSV IgG examination was 44.2 (reactive) so that HAEM was diagnosed with chronic anemia. Pharmacological management was given paracetamol, acyclovir, folic acid, Fe, and vitamin B12. Lip lesions are given a blended ointment. Provided education and instructions to patients to maintain nutritional intake, adequate rest and regular use of drugs. Conclusion: The healing condition can be faster by paying attention to the chronic anemia condition experienced by the patient.

Abstract

Introduction: Chronic ulcers are considered as the most common ulcerations and can be differentiated with oral squamous cell carcinoma (OSCC) by the presentation of its features. OSCC define as a malignant epithelial neoplasm and the most common neoplasm of the oral cavity. OSCC appears as a mixed white or reddish proliferative growth-like lesion from chronic trauma. Case and management: A 50 years old female came with a painful non-healing ulcer on the lateral right side of the tongue which had been exist since 6 months prior. There had been a gradual increase in the size of the ulcer over the past 6 months. The patient did not have any sistemic disease or bad habits such as chewing or smoking and did not consume alcohol either. Based on the examination, the right submandibular lymphnodes were palpable, tender, firmed and mobile.  And there was also a single oval-shaped ulcer with irregular border on the lateral aspect of the tongue in parallel with fractured and decayed lower first molar. The ulcer size was 2x1 cm in diameter, with the base covered by yellowish pseudomembran slough with indurated border. The grinding of the sharp cusp of the tooth was done, followed by the prescription of multivitamins, antibacterial mouthwash and topical corticosteroid. She was also referred to have routine blood test done. She came for the second visit after 14 days, showing improvement of the ulcer. The blood test showed no abnormal values. Discussion: Based on clinical features, the presented lesion was mimicking OSCC. According to the patient’s history taking, clinical examination and appropriate investigation, the patient was diagnosed with traumatic ulcer. Ulcer resolves and heals on the removal of causative factors. Conclusion: Chronic ulcer is one of the most common solitary ulcer presenting in the oral cavity that mimicking OSCC by the presentation of its features. In this reported case, the ulcer arises due to its constant contact with lateral right of the tongue Improvement during the treatment by eliminating suspected risk factors can ruled out the possibility of OSCC thus prevent the unnecessary treatment.

Abstract

Latar belakang : Leukoplakia adalah istilah klinik untuk plak atau bercak putih pada mukosa mulut yang tidak dapat dihapus dan tidak dapat diklasifikasikan sebagai penyakit lain apapun yang dapat di diagnosis secara klinis. Insiden terjadinya leukoplakia pada suatu populasi sekitar 0,1%. Salah satu faktor predisposisinya adalah merokok. Kebiasaan merokok cukup sulit dihilangkan dalam edukasi terhadap pasien. Hal ini menimbulkan dilema dalam edukasi tehadap kasus leukoplakia. Tujuan : Melaporkan penatalaksanaan sebuah kasus suspek leukoplakia pada laki-laki 44 tahun yang dipicu oleh faktor merokok. Kasus : Seorang pasien laki-laki berusia 44 tahun datang dengan keluhan ingin memeriksakan bercak putih pada gusi dan langit-langit rongga mulutnya sudah 6 bulan dan tidak terasa nyeri. Pemeriksaan ekstraoral tidak ada kelainan. Pemeriksaan intraoral terdapat plak putih tidak dapat dikerok pada daerah gingiva dan palatum. Penatalaksanaan kasus : Melakukan KIE dengan pasien diinstruksikan agar mengurangi kebiasaan merokoknya dan merujuk ke dokter spesialis penyakit mulut. Pembahasan : Pemeriksaan histopatologi dan sitologi dapat membantu dalam penegakan diagnosis leukoplakia. Akan tampak adanya perubahan keratinisasi sel epitelium, terutama pada bagian superfisial. Secara mikroskopis, perubahan ini dapat dibedakan menjadi 5 bagian, yaitu hiperkeratosis, hiperparakeratosis, akantosis, diskeratosis atau displasia, karsinoma in situ. Simpulan : Untuk menegakkan diagnosis dan managemen kasus leukoplakia diperlukan kerja sama yang baik antara pasien dan klinisi.
