<![CDATA[ABSTRAK Dokumentasi keperawatan adalah suatu catatan yang dapat dibuktikan atau dapat dijadikan bukti dari segala macam tuntutan, yang berisi datalengkap, nyata dan tercatat bukan hanya tentang tingkat kesakitandari pasien, tetapi juga jenis/type, kualitas dan kuantitas pelayanan kesehatan dalam memenuhi kebutuhan pasien. Berdasarkan data Rekam Medik Rumah Sakit Muhammadiyah Gresik pada bulan Januari 2014 sampai dengan April 2014. Total 490 Rekam Medik, dengan model dokumentasi asuhan keperawatan sesuai kebijakan rumah sakit, dokumen asuhan keperawatan pada bagian diagnosa keperawatan dan intervensi semua masih kosong (belum terisi), seharusnya ini diisi semua (≥ 85 % dari yang seharusnya terisi).Penelitian ini menggunakan jenis Pra-eksperimental One Group Pre and Post TestOnly Design.Sampel dalam penelitian ini terdiri Seluruh Tenaga Perawat di Ruang Inap Anak, RDU, dan Mecca-Medina Rumah Sakit Muhammadiyah Gresik : 34 orang. Data yang diperoleh diuji menggunakan Uji wilcoxon Test,dengan taraf signifikasi p ≤ 0,05 berarti ada pengaruh yang bermakna antara dua variabel yang diukur, yaitu pengaruh penerapan model dokumentasi asuhan keperawatan POR.Pada penelitian ini diperoleh perhitungan nilai rerata sebelum penerapan adalah X1 = 1.16 dan nilai standar deviasinya 0.370 Sedangkan nilai rerata setelah penerapan adalah X2 =1.07 dan nilai standar deviasinya 0.250, sedangkan nilai z adalah -3,606 dan Asymptotic Significance (2-tailed) p= 0,000, berarti p<0,05 maka H0 ditolak dan H1 diterima artinya ada pengaruh dalam penerapan model dokumentasi asuhan keperawatan POR.Terdapat pengaruh dalam penerapan model dokumentasi asuhan keperawatan Problem Oriented Record (POR) terhadap kinerja perawat di Rumah Sakit Muhammadiyah Gresik. Kata kunci: Dokumentasi keperawatan, Model dokumentasi POR, Kinerja Perawat    ABSTRACT Nursing documentation is a record that can be proven or can be used as evidence of all sorts of demands, which contains complete data, real and recorded not only on the level of pain of patients, but also kind/ type, quality and quantity of health services to meet the needs of patients. Based on data fromthe Hospital Medical Records Muhammadiyah Gresik in January 2014 to April 2014. Of the total 490 medical record, the model of nursing care documentation at the discretion of the hospital, document nursing care on the part of nursing diagnosis and intervention are all still empty (unfilled), this should have filled all (≥ 85% of the supposed filled).This research uses experimental One Group Pre-Pre and PostTest Only Design. The sample in this study consisted Entire Energy Confinement Room Nurse at Children's, RDU, and Mecca-Medina Room of Muhammadiyah Hospital of Gresik are : 34people. The data obtained were tested using the Wilcoxon test, with a significance level of p≤0.05 means there is a significant effect between the two variables were measured, namely the effect of applying the model of POR nursing care documentation. In this research, the average value calculation before aplication is X1= 1.16 and standard deviation value of 0370, while the average value after aplication is X2= 1.07 and standard deviation value of 0.250, while thes value is-3.606 and Asymptotic Significance (2-tailed) p=0.000, mean p<0.05 then H0 is rejected and H1 accepted meaning that there is the effect of applying the model of POR nursing care documentation.
