<![CDATA[ABSTRAK Pendokumentasian yang tidak dilakukan dengan lengkap dapat menurunkan mutu pelayanan keperawatan karena tidak dapat mengidentifikasi sejauh mana tingkat keberhasilan asuhan keperawatan yang telah diberikan, dalam aspek legal perawat tidak mempunyai bukti tertulis jika klien menuntut ketidak puasan akan pelayanan keperawatan (Nursalam,2008). Tujuan penelitian ini adalah mengetahui capaian asuhan keperawatan jiwa dalam hal ini dokumentasi asuhan keperawatan jiwa pada praktik klinik mahasiwa Prodi D-3 Keperawatan Universitas Bondowoso.Metode penelitian ini adalah diskriptif analitik melalui studi dokumentasi asuhan keperawatan jiwa pada praktik klinik mahasiswa Prodi D-3 Keperawatan. Tempat penelitian di Program Studi D-3 Keperawatan Universitas Bondowoso yang dilakukan pada September 2017. Seluruh mahasiswa semester IV Prodi DIII Keperawatan Universitas Bondowoso tahun 2017 berjumlah 55 mahasiswa, yang terdiri dari kelas A  dan kelas B. Teknik sampling penelitian menggunakan cluster randem sampling didapatkan jumlah sample kelas B sebanyak 27 mahasiswaDokumentasi asuhan keperawatan jiwa mengacu proses keperawatan yang meliputi pengkajian, diagnosa keperawatan, perencanaan, pelaksanaan dan evaluasi. Secara keseluruhan dokumentasi asuhan keperawatan rata-rata dokumentasi asuhan keperawatan jiwa pada praktik klinik mahasiswa Prodi DIII Keperawatan Universitas bondowoso dengan nilai baik dimana telah mendokumen asuhan keperawatan jiwa 71,52%Diperlukan upaya peningkatan bimbingan asuhan keperawatan jiwa yang ditekankan pada pengkajian dan perencanaan yang berhubungan dengan evaluasi tentang deteksi masalah baru dan modifikasi rencana keperawatan. Kata Kunci : Dokumentasi asuhan keperawatan jiwa, praktek klinik mahasiswa ABSTRACT Documentation that is not done in full can reduce nursing service quality because it cannot identify the extent to which the success of nursing care has been given, in the legal aspect the nurse does not have written evidence if the client demands dissatisfaction with nursing services (Nursalam, 2008).
