Abstract

Anisometropia if not treated accordingly may cause other issues especially in developing pediatric eyes. This case is a 13 year old female presented with chief complaint of headache and double vision upon wearing prescribed spectacles. Patient had history of high myopia on both eye, amblyopia and spontaneous rhegmatogenous retinal detachment on the left eye. Retinal reattachment and lensectomy surgery were conducted to correct the problem. The surgery was performed without intra ocular lens (IOL) implantation, which left her left eye become high hyperope due to aphakia. Patient was treated with RGP CLs. Final best visual acuity with EDTRS chart was 6/48 on both eyes. Patient reported subjective visual improvement, no headache or double vision.

Abstract

AbstractBackground,Minibreak intervention may solve asthenopia which has a main problem among workers using VDT. Asthenopia is measured with near point convergence (NPC). The purpose of this study was to determine convergence power difference that mini break produces between workers using standard VDT and widescreen VDT. Methods, Sixty six workers were divided into four groups: minibreak and no minibreak intervention in standard VDT users and widescreen VDT users. The difference of NPC before and after 4 hours of VDT exposure and 4 hours of exposure with a 15 minute minibreakwere measured in all groups using RAF ruler.  Results,  Minibreak intervention resulted in a significant difference of convergence power between workers using standard VDT (p=0.000) and widescreen VDT (p=0.000).   But the differences of convergence between standard VDT and widescreen VDT were not significant in those given minibreak (p=0.9694) and those not given (p=0.6251). Conclusion, Mini break intervention produced significant differences in convergence power between both groups but the size of the VDT had no effect. Keywords: Asthenopia, VDT, near point convergence, mini break AbstrakLatar Belakang, Mini break dapat menjadi solusi untuk kelelahan mata yang menjadi masalah utama pada pekerja dengan pekerjaan menggambar menggunakan VDT.  Kelelahan mata dapat diukur dengan mengukurnear point convergence (NPC). Tujuan penelitian ini untuk mengetahui perbedaan NPC yang dihasilkan oleh mini break pada pekerja yang menggunakan VDT standar dan wide screen. Metode, Enam puluh enam pekerja dibagi menjadi empat kelompok: mini break dan tanpa mini break pada pekerja VDT  standar dan wide screen. Perbedaan NPC sebelum dan sesudah pajanan VDT selama 4 jam terus-menerus tanpa mini break dan pajanan selama 4 jam dengan mini break 15 menit di tengah-tengah diukur pada semua grup menggunakan RAF ruler. Hasil, Intervensi mini break menghasilkan perbedaan bermakna pada kekuatan konvergensi antara pekerja yang menggunakan   VDT   standar   (p=0.000)   dan   VDT  widescreen   (p=0.000).   Namun   perbedaan konvergensi antara VDT standar dan widescreen tidak berbeda bermakna pada grup yang diberikan mini break (p=0.9694) dan tanpa mini break (p=0.6251). Kesimpulan, Intervensi mini break menghasilkan kekuatan konvergensi yang berbeda antara dua kelompok pekerja, namun hal tersebut tidak dipengaruhi oleh ukuran VDT. Kata kunci: Mata lelah, VDT, pekerja gambar, near point convergence, mini break

Abstract

Background: Fitting Contact Lenses for post penetrating keratoplasty patiens can be a great challenges to contact lens practitioner. Despite advances in surgical techniques for penetrating keratoplasty, post- operative ametropia and high irregular astigmatism still remain common. Patients who have such those problems may need contact lenses to achieve optimal vision.Case Illustration: Case 1. A 12 years old boy have a high myop and astigmatism after undergone penetrating keratoplasty combined with traumatic cataract extraction and IOL implantation. His right eye vision was 6/24 with corrections of S – 8.00 C – 6.50 x 150. His right eye was fitted with RGP contact lens with Base curve of 7.20 mm, Power of – 10.25 D, and diameter of 8.80 mm. His RE vision with RGP contact lens was 6/12; Case 2. Female, 18 years old, was diagnosed with compound high myopia astigmatism of right eye due to disfigured cornea post keratoplasty and high astigmatism of the left eye due to corneal scar. Piggyback contact lens system is then used to treat her right eye.The patient’s uncorrected visual acuity on the right eye was half meter counting finger, became 6/40 with spectacle and 2 meters counting finger became 3/60 with spectacle on the left eye. With piggyback contact lens consisting of soft contact lens 8.60/-10.00/14.20 and RGP lens 7.20/-4.00/8.8 on the right eye, and RGP lens 7.90/-5.00/9.2 on left eye, her visual acuity became 6/12 on her right eye became 6/9 on the left eye.Conclusion: Rigid gas-permeable and combination of rigid gas permeable and soft contact lenses can be fitted with piggy-back system successfully in patients unable to achieve optimal vision with spectacle correction after penetrating keratoplasty

Abstract

Background: The aim of this study is to evaluate the efficacy, predictability and safety of the first experience in laser in situ keratomileusis (LASIK) surgery using Wavelight Refractive Suite in Kirana Cipto Mangunkusumo HospitalMethods: Design of this study was descriptive retrospective. Data of patients who underwent LASIK surgery from April 2012 to January 2013 were collected. The data consist of pre and post operative refractions, uncorrected visual acuity (UCVA), best corrected visual acuity (BCVA).Results: Fifty eight eyes of 31 patients, 13 males and 18 females, mean age of 28 year-old were analyzed. The mean spherical error corrected was -6,42 ± 3,37 D with range of -1.25 D to -16,87D. Median UCVA post surgery 1 day, 1 week, and last follow up time were 6/7,5, 6/6, and 6/6 respectively. Postoperatively, 91,67% eyes achieved UCVA of 6/12 or better with 77,78% achieving 6/7,5 or better. One patient lost 1 Snellen line of BCVA became 6/6,6. The mean postoperative manifest refraction spherical equivalent was -0,24 (±0,42) D. The general refractive predictability was 88,2% within ±0.5D from the target refraction, while 94,1% within ±1D ofthetarget.Conclusion: LASIK surgery performed in Kirana Cipto Mangunkusumo Hospital is safe and effective with good refractive predictability.

Abstract

Background: To compare visual acuity correction of astigmatism and comfort between toric softcontact lens and rigid gas permeable. Thus, ophthalmologists will less reluctantly propose contactlenses for the patients with astigmatism, and maximize the potential advantages of contact lenses forthe wearer.Methods: Literature review of all prospective and retrospective studies which reported managementof astigmatism with rigid gas permeable of toric soft contact lenses. The articles were divided intocharacteristics and outcomes. Outcomes reviewd were visual acuity, lens fit characteristics, cornealstaining, and subjective problems.Results: Lens corrected visual acuity by toric soft lens showed variation, which 3 studies reported afew amount of visual loss from spectacles corrected. Meanwhile other studies reported better toric softlens corrected visual acuity than spectacles. In RGP group, four studies reported better or the samevisual acuity with spectacles, and only one study showed a few amount of visual loss. Visual clarityand poor comfort were the most frequent subjective problems in toric soft lens group. Meanwhile,dryness and poor comfort were the most frequent subjective problems in RGP group.Conclusion: RGP is superior than toric soft lens in correcting astigmatism. Meanwhile, toric soft lensprovides quick adaptation and comfort. Keywords: astigmatism, rigid gas permeable, toric soft lens

Abstract

Background: Rigid gas-permeable contact lenses (RGP CL) are commonly used to manage moderate tosevere keratoconus. The range of RGP CL type currently available for the successful optical correctionof keratoconus has greatly expanded. We compared the efficacy and safety of available RGP CL basedon visual acuity (VA), keratometry, comfort score, and adverse event.Methods: Literature search on RGP CL and keratoconus retrieved 37 citations from 2010-2015. Ninearticles were selected as relevant to our purpose.Results: All studies showed improvement of VA following RGP CL wearing. Greatest improvementwas seen in Rose K CL (.5 - .36) in naïve CL patient and Twinbel bevel toric in refitting CL patient(.22). All studies reported lower mean ΔK post fitting RGP CL than pre-fitting. Greatest difference ofmean ΔK pre and post-fitting was observed in YK lens (.72). The most comfortable lens was Rose K(91.5%) followed by Soper CL (87.9%) and Twinbel bevel toric (64.4%). Punctuate corneal stainingfound in a considerable amount with YK lens patient.Conclusion: Among 5 types of RGP CL available now for keratoconus, Rose K provided greatestBCVA improvement in naïve CL patient whereas Twinbel bevel toric in refitting patient. YK lensgave greater changes in keratometry post-fitting RGP CL. The most comfortable lens for patient isRose K lens. The most common adverse event of CL wear is punctuate corneal staining in YK lenspatient.Keywords: rigid gas permeable contact lens, keratoconus

Abstract

Background: To compare corneal thickness and endothelial cell morphology in myopic patients wearing 1 month hydrogel conventional and silicon hydrogel contact lenses in daily wear.Methods: This is a prospective, single blind, randomized study. Seventeen (34 eyes) myopic patients were randomly assigned to receive either nelfilcon A or lotrafilcon B. Central corneal thickness (CCT), coefficient of variation (CV), and percentage of six-sided cell (6A) were examined using specular noncon robo Konan microscope. Changes in CCT, CV, and 6A were evaluated before contact lenses fitting as well as 1 week and 1 month after the treatment. Patients were also evaluated for any subjective side effects and complications during the treatment period. Results: There were 64.7% subjects with history of contact lens wearers and 52.9% of them was hydrogel wearers. After 1 month daily wear of nelfilcon A and lotrafilcon B, no statistically differences changes in CCT (p=0.285; Mann Whitney test), CV (p=0.587; unpaired t-test), and 6A (p=0.353; unpaired t-test). General linear model analysis in follow up evaluation with test of within subject effect revealed decreased in CCT (p=0.001) and CV (p=0.001), also increased in 6A (p=0.022) in all subjects.Conclusion: There were no statistically difference in CCT, CV, and 6A between daily wear of nelfilcon A and lotrafilcon B. Subjects with history of contact lens wearers gained advantages in oxygen availability, which decrease corneal thickness and improve endothelial cell morphology.  Keywords: nelfilcon A, lotrafilcon B, simple myopic, corneal thickness, endothelial cell morphology

Abstract

Background: Steven-Johnson syndrome is one of life-threatening skin abnormality. SJS complication involve multiorgan disfunction such as mucosa, ocular, respiration, and digestive tract. Around 30- 50% of SJS patient experience chronic ocular sequel, which concludes progressive symblepharon, margo palpebral keratinization, trichiasis, entropion, corneal pannus, dry eye syndrome, and persistent epithelial cornea defect. Scleral contact lenses has beneficial in managing SJS patient, on both acute and chronic phases. Methods: A 36-year-old female patient with SJS, presented for eye examination due to unpleasant sensation on both of their eyes and unsatisfactory quality of her vision. She presented high myopia, margo palpebral keratinization with trichiasis and distichiasis, symblepharon and hyperemic conjunctiva, wide corneal scar, and ocular neovascularization. A diagnostic trial set was used in the fitting process, and she was assessed according to standardized fitting methodology. Visual acuity, corneal topography, biometry, and ocular aberrations were evaluated. The follow-up period was 12 months. Results: The best corrected visual acuity was 20/60 with correction lens S – 1.00 D for the left eye (LE) and 20/25 cannot be corrected with correction lens for the right eye (RE). After corneal contact lens fitting, visual acuity was improved to 20/20 and 20/25 for the RE and LE, respectively. The patient wore these contact lenses an average 8 hours a day. The patient’s dry eye complaint was significantly decrease with more comfortable ocular sensation Conclusion: The present case report describes how the patient had scleral contact lens fitted successfully for management of SJS ocular complication. They provided optimal visual quality and restore the patient’s comfort.
