Abstract

Introduction: Ocular toxoplasmosis is a major cause of infectious posterior uveitis worldwide. There was no exact number of ocular toxoplasmosis prevalence in Indonesia, but Indonesia was considered to have high seroprevalence in Southeast Asian. This study is conducted to determine clinical characteristics and management of ocular toxoplasmosis at outpatient clinic of Dr. Saiful Anwar General Hospital, a tertiary referral hospital in Malang, East Java. Methods : This was retrospective study. We reviewed the medical records of patients with ocular toxoplasmosis and collected the data associated with age, sex, laterality, visual outcome, type of lesions, serum serological titers, therapeutic regimens, and complications. Result : There were 48 eyes from 38 patients included in this study, mostly were female (66%) with mean age was 33,5 years. Unilateral infection (71%) was more frequent than bilateral cases (39%). Active lesions were found more than cicatrical lesions (56,25%). Most patients with active diseases had unilateral lesion (87,5%). The most common presenting complain was blurred vision (73%). Most of lesions (22 eyes; 81,4%) were located on macular region. All of patients have positive IgG antitoxoplasma serum. There were 22 patients received oral Trimethoprim-Sulfamethoxazole and steroid. Visual acuity improved in 6 patients at the end of follow-up period. Complications of retinal detachment and choroidal neovascularization were found in 3 patients. Conclusion : Active ocular toxoplasmosis is more likely to be unilateral infection with main presenting complain is blurred vision. Most of our patients show good responses to oral trimethoprim-sulfamethoxazole and steroid.

Abstract

Introduction: Lupus retinopathy and posterior uveitis are complications due to systemic lupus erythematosus which can threaten the vision. The presence of posterior segment manifestation is suggestive of high disease activity. The aim of this study is to identify posterior segment manifestation (Lupus Retinopathy and Posterior Uveitis) in SLE patient and their correlation with SLE disease activity using The Mexican-SLEDAI (MEX-SLEDAI) score. Methods: This was an analytical observational study with cross-sectional design, conducted from August to October 2020 and involved 114 SLE patients in Dr. Saiful Anwar General Hospital. We calculated MEX-SLEDAI score to assess SLE disease activity. All participant that met inclusion criteria underwent ophthalmology examinations using a portable slit-lamp, head indirect ophthalmoscope, and fundus finding were documented using portable fundus imaging. Result: Lupus retinopathy (LR) presents in 25/114 (21.9%) and posterior uveitis (PU) occurs in 2/114 (1.8%) SLE patients. The mean age of patient with LR, PU, and without retinopathy were 32.92; 37.00; and 31.08 years respectively. The posterior segment findings were hemorrhages, cotton wool spots, hard exudates, and vasculitis reflecting vascular damage. The most common manifestation found in retina was cotton wool spot. The mean of MEX-SLEDAI score of SLE patient with LR (7.200 ± 3.905) and SLE patient with PU (3.500 ± 2.121) was higher than the mean of SLE patient without LR and PU (2.871 ± 2.534). There was a significant association between LR and MEX-SLEDAI score (p=0.000). An insignificant association between PU and MEX-SLEDAI score was found (p=0.353) Conclusion There is a significance correlation between lupus retinopathy and SLE disease activity based on MEX-SLEDAI scores. The mean of MEX-SLEDAI score in SLE patients with lupus retinopathy was higher than SLE with posterior uvetis and SLE without posterior segment manifestations.

Abstract

Tujuan : Untuk melaporkan aspek klinis, terapi, hasil terapi dan insiden dari endoftalmitis akut pasca operatif katarak di RSUD Dr. Saiful Anwar, Malang.Metode : Penelitian deskriptif analitik retrospektif yang menggunakan rekam medik dengan diagnosis endoftalmitis akut pasca operatif dari Januari 2015 - Desember 2018 di RSUD Dr. Saiful Anwar. Kami mencatat kasus endoftalmitis pasca operasi katarak yang terjadi dalam 6 minggu setelah operasi. Data yang dikumpulkan meliputi demografi, temuan klinis, hasil mikrobiologi, terapi dan komplikasi. Hasil : Didapatkan 31 kasus endoftalmitis akut pasca operatif, dimana 29 kasus merupakan rujukan. Dua puluh dua kasus pasca fakoemulsifikasi, 5 kasus pasca extra capsular cataract extraction dan 4 kasus pasca small incision cataract surgery. Lebih dari 50 % pasien adalah laki-laki (58,06 % ), berusia 61-70 tahun (38,7%) dan terjadi pada minggu pertama pasca operasi (45,2%). Penyakit penyerta meliputi diabetes mellitus (41,9%) dan hipertensi (32,3%).Terapi diberikan sesuai protokol Endophthalmitis Vitrectomy Study, diantaranya injeksi antibiotik intravitreal, vitrektomi, antibiotik sistemik dan topikal, kortikosteroid sistemik dan topikal. Didapatkan perbedaan signifikan antara visus sebelum dan setelah terapi (p=0.038), namun tidak didapatkan perbedaan antar kelompok terapi. Pada 9 kasus terjadi komplikasi, 2 diantaranya dilakukan eviserasi oleh karena perforasi kornea. Kesimpulan : Sebagian besar kasus endoftalmitis akut pasca operatif terjadi setelah fakoemulsifikasi, pada laki-laki, usia 61-70 tahun dan terjadi pada minggu pertama pasca operasi. Organisme terbanyak yang teridentifikasi adalah bakteri gram positif. Didapatkan perbaikan visus yang signifikan dengan pemberian terapi.
