Abstract

ABSTRACT Anemia is a clinical symptom caused by the disruption of erythrocyte formation, bleeding, and premature blood hemolysis. Severe anemia can occur in patients taking dapsone for the treatment of leprosy. Long-term use of steroids is also a predisposing factor for the occurrence of hematological changes and raises the suspicion of Cushing's syndrome. Reportedly a 20-year-old woman with the main complaint of sudden shortness of breath, accompanied by a cough with phlegm. The patient has had a history of multibacillary type leprosy since a month ago. There was a history of steroid consumption for about two years and only stopped a couple of months ago. Physical examination showed the oxygen saturation was 93%, conjunctival anemia, hyperpigmented plaques, tenderness in the abdominal, meteorism, and pitting edema in the upper and lower extremities. Laboratory examination revealed hemoglobin (Hb) level 2.4 mg/dl, erythrocytes 0.8x106/ul, and albumin 3.2g/dl. Peripheral blood smear results suggest refractory anemia and myelodysplasia syndrome. After being treated for seven days, the patient was re-evaluated and controlled at the Internal Medicine outpatient clinic, Syekh Yusuf Hospital. Complex symptoms and limited investigations make it difficult to establish the exact cause. Immediate blood transfusion and management of the underlying disease causing anemia, such as discontinuation of dapsone, are the primary management of this case. Keywords                : severe anemia, leprosy, steroid, Cushing syndrome.Correspondence        : gedepadmawijaya@yahoo.com

Abstract

Hospitals as health facilities play a vital role in providing efficient and quality services. The quality of health services that are in line with the code of ethics and professional service standards is expected to create satisfaction for patients. This research is an analytical observational study using a cross sectional study approach. The research population was general patients undergoing treatment in internal and pediatric wards for 1 month. The sampling method used accidental sampling, with a total sample of 37 people. Data analysis was carried out using the chi-square statistical test. The quality of interaction has a significant relationship with patient satisfaction (p-value 0.001 < 0.05). The patient satisfaction variable at Syekh Yusuf Hospital, Gowa Regency is significantly related to the quality of interaction. These findings provide important insights for efforts to increase patient satisfaction at these hospitals. This research can be the basis for developing health care interaction strategies to increase patient satisfaction. The results of the research can be a basis for hospitals in identifying and improving certain aspects of health services in order to increase the level of patient satisfaction in providing optimal health services at Syekh Yusuf Hospital, Gowa Regency.

Abstract

Introduction: Tetanus is an acute infectious disease caused by Clostridium tetani, characterized by muscle rigidity and spasms. Case Presentation: We report the case of a 9-year-old boy who presented with generalized body stiffness, difficulty opening his mouth, and seizures triggered by sound and movement. Two days before the onset of symptoms, the patient sustained a puncture wound from a rusty nail without receiving adequate medical treatment. Physical examination revealed trismus, risus sardonicus, generalized muscle rigidity, and reflex spasms. The diagnosis was established based on the patient’s clinical presentation and classified as moderate tetanus using the Ablett, Phillips, and Dakar scoring systems. The patient was treated with intravenous metronidazole, tetanus immunoglobulin, diazepam, supportive therapy, and intensive care unit management. Conclusion: This case highlights the importance of early diagnosis and prompt comprehensive management in reducing tetanus-related morbidity and mortality.
