Abstract

Introduction: Transposition is an extreme and unique form of ectopic eruption. Orthodontic correction of transposed teeth in permanent dentition comprises the following treatment options: teeth alignment in the transposed position, orthodontic tooth movement into correct teeth order, or extraction on one of the transposed teeth followed by orthodontic correction. Case report: A 20-years-old male presented severe crowding and dental midline shift on both arches, followed by complete transposition of maxillary left canine – first premolar. A supernumerary tooth was also revealed between maxillary lateral and first premolar on the left side. The patient was treated with a pre-adjusted edgewise fixed appliance system. Extraction of supernumerary tooth and unilateral first premolars on the left side were performed to relieve crowding and to achieve stable occlusion. Improvement of smile aesthetic, correction of midline shift, and also an ideal class I of canine and posterior interdigitation were achieved after 22 months of treatment. Conclusion: Definitive treatment of transposition depends on the occlusion, degree of crowding, aesthetics, root position of the transposed teeth, and specific needs of the patient. Care must be taken to prevent midline shift and development of arch asymmetry during orthodontic correction of transposition involving unilateral extraction.

Abstract

Frequent upper respiratory tract infections and allergic reactions may cause upper respiratory tract obstruction (OURT). Mouth breathing (MB) occurs in individuals with nasal breathing problems. A person with MB will raise his head higher; thus, MB is a risk factor for head posture (HP) deviation. Children with MB during growth and development may exhibit dentocraniofacial (DCF) deviation. Objective: To evaluate the prevalence of MB, HP, and DCF deviation in OURT patients to know risk factors of HP deviation, morphological aberrations of DCF and DCF deviation growth and development. Methods: This cross-sectional study included 285 OURT subjects aged 9–15 years. Data obtained from cephalometric analysis, physical examination, and questionnaires were analyzed. Results: Of 285 OURT subjects, 80.4% showed MB, 44.2% HP deviation, and 66.7% DCF deviation. As risk factors for DCF deviation, MB and HP showed odds ratios of 20.45 and 8.11 and population attributable risks of 87.5% and 59.7%, respectively. Conclusion: The prevalence of MB and DCF deviation in OURT patients is high, but that of HP deviation is generally comparable. MB and deviated HP are risk factors for DCF deviation growth and development.

Abstract

Proper orthodontic treatment in adult patients with severe skeletal Class II discrepancy can be challenging. Recently, miniscrew implants have been used as a strategy to treat skeletal Class II patients. Objective: This report illustrates a skeletal Class II malocclusion management combining straight wire technique and miniscrew implant anchorage. Case report: The patient was a 21-years- old Indonesian female with Class II skeletal discrepancy, a retrognathic mandible, a high mandibular plane angle, and a mouth breathing habit due to a history of allergic rhinitis. Anchorage control is important in the sagittal and vertical directions. Miniscrew implants were placed in the interradicular ar ea bet ween t he upper second pr emolar and fir st mol ar on bot h sides. Further more, en -masse retraction of the six anterior teeth was performed using miniscrew implants as the anchorage. After 16 months of treatment, esthetics and function were improved and the chief complaint of the patient was resolved. Class I canine and incisor relationship was achieved. These mechanics contributed to the correction of the gummy smile of this patient. Conclusion: Placement of miniscrew implants in the posterior regions of the maxilla effectively camouflaged a high-angle skeletal Class II discrepancy. This technique requires minimal patient compliance and is useful for the correction of high-angle cases in adult patients.

Abstract

ABSTRACTIntroduction: With the increasing number of adult orthodontic patients, there is also a growing demand for more aesthetic and comfortable treatments. Clear aligners are a quite new treatment technique in the field of orthodontics that is being developed using digital-based technology, and their usage is increasing due to their perceived aesthetic and comfortable nature. This research objective was to analyze differences in factors influencing orthodontists’ consideration of clear aligners, satisfaction, and stability levels in a capital city. Methods: This comparative analytical study with a cross-sectional design was conducted to examine the perception of orthodontists in a capital city in Indonesia, Jakarta, regarding the use of clear aligners. Fifty orthodontists taken through Slovin sampling technique, were given a set of questionnaires and divided into two groups based on users and non-users of aligner. The survey utilized a questionnaire consisting of 17 questions with both clinical and satisfactionary criteria, which were sent online via WhatsApp Messenger to the respondents. To compare the data were analyzed by the chi square test statistically. Results: Most of respondent practiced in elite areas of South Jakarta, 52.2%, among other 4 differences cities were observed in the perception of orthodontists who were 46% users and 54% non-users aligner regarding periodontal health factors, treatment efficiency, pain complaints, root resorption, satisfaction levels, and treatment outcome stability. Both clear aligners users and non-users have similar perceptions regarding treatment cost factors influencing the usage of clear aligners, improved oral hygiene, minimal white spot lesions, and better aesthetics. Conclusion: Some parameters were not different, especially for the parameters of treatment cost as a negative perception, and oral hygiene, white spot lesion, and aesthetics, all of which gave similar positive perceptions rated by respondents. Clear aligner merupakan teknik perawatan yang cukup baru di bidang ortodontik yang sedang dikembangkan menggunakan teknologi berbasis digital, dan penggunaannya semakin meningkat karena sifatnya yang estetis dan nyaman. Tujuan penelitian ini adalah untuk menganalisis Perbedaan faktor-faktor yang mempengaruhi pertimbangan penggunaan clear aligners, tingkat kepuasan, dan stabilitas oleh ortodontis di sebuah ibu kota negara. Metode: Metode penelitian analitik komparatif dengan desain potong lintang  dilakukan melalui teknik sampling Slovin, untuk mengkaji persepsi ortodontis di Jakarta, sebuah ibukota negara, Indonesia, mengenai penggunaan clear aligner. Sebanyak lima puluh ortodontis di Jakarta, diberikan kuesioner dan dibagi menjadi dua kelompok berdasarkan pengguna dan bukan pengguna aligner. Survei menggunakan kuesioner dengan 17 pertanyaan tentang reaksi klinis dan kepuasan, yang dikirimkan secara online melalui WhatsApp Messenger kepada responden. Data dianalisis secara statistik dengan uji Chi-Square. Hasil: Sebagian besar responden berpraktik di kawasan elit Jakarta Selatan, 52,2%, di antara 4 kota lainnya perbedaan diamati pada persepsi ortodontis yang merupakan 46% pengguna dan 54% bukan pengguna aligner mengenai faktor kesehatan periodontal, efisiensi perawatan, keluhan nyeri, resorpsi akar, tingkat kepuasan, dan stabilitas hasil perawatan. Baik pengguna clear aligner maupun bukan pengguna memiliki persepsi yang sama mengenai faktor biaya perawatan yang memengaruhi penggunaan clear aligner, peningkatan kebersihan mulut, minimalnya white spot lesion, dan estetika yang lebih baik. Simpulan: Beberapa parameter tidak berbeda, terutama untuk parameter biaya perawatan sebagai persepsi negatif, dan kebersihan mulut, lesi white spot, dan estetika, yang semuanya memberikan persepsi positif yang sama yang dinilai oleh responden. Hal ini cukup baik bagi produsen dan pengguna aligner. Kata kunci Pengguna dan non-pengguna clear aligner, Ibukota negara, persepsi ortodontis.

Abstract

Background: Malocclusion is the irregularity of teeth, considered as an oral health problem resulting from various etiological factors, causing esthetic dissatisfaction to functional impairment. Malocclusions must be fixed by orthodontic treatment. Achieving proper incisor inclination and angulation is essential for orthodontic treatment stability and facial harmony. However, limited evidence exists regarding post-treatment incisor positioning in Class I skeletal malocclusion patients within the Indonesian population. Objective: This study aimed to analyze the post-treatment inclination and angulation of maxillary and mandibular incisors in Class I skeletal malocclusion patients and to evaluate whether these parameters fall within established cephalometric norms. Methods: A descriptive cross-sectional analysis was conducted using secondary cephalometric data from 96 post-treatment medical records. Incisor inclination was measured using U1–SN, U1–PP, L1–MP, and IMPA angles, while angulation was assessed for upper and lower central incisors. Results: The mean upper incisor angles were 106° (U1-SN) and 115° (U1-PP), and the lower incisor angles were 94° (L1-MP) and 96° (IMPA). The mean angulation of individual incisors was approximately 89–91° for both upper and lower central incisors. Conclusion: Pre-adjusted bracket systems used in the study effectively guide incisors toward favorable positioning within normal ranges, with minor variations such as the IMPA angle, supporting both functional and esthetic outcomes.This study took the initiative to describe both post-treatment upper and lower incisor position in Indonesian orthodontic patients.
