Abstract

PT IPMS (INTI PINDAD MITRA SEJATI) merupakan perusahaan industri manufaktur dengan salah satu hasil produksi adalah isolator. PT IPMS telah melakukan perbaikan dalam proses produksi, namun perbaikan tersebut masih belum mengurangi jumlah cacat secara signifikan. Metode Failure Mode and Effect Analysis (FMEA)  merupakan suatu prosedur untuk mengidentifikasi kegagalan produk berdasarkan potential cause. FMEA berfungsi untuk menentukan nilai Risk Priority Number (RPN) yang selanjutnya akan diidentifikasi lebih lanjut menjadi fokus utama dalam menentukan akar dari potential cause menggunakan Fault Tree Analysis (FTA). Dari akar permasalah tersebut dapat ditentukan usulan perbaikan dan pengendalian agar dapat membantu perusahaan untuk mengurangi resiko kegagalan produksi.   Kata kunci: Failure Mode And Effect Analysis, Fault Tree Analysis, Risk Priority Number, Potential Cause   ABSTRACT PT IPMS (INTI PINDAD MITRA SEJATI) is an industrial manufacturing company with one of the products is an insulator. PT IPMS has made improvements in the production process, but the improvement is still not significantly reduce the number of defects. Method of Failure Mode and Effect Analysis (FMEA) is a procedure to identify potential cause product failures based. FMEA is used to determine the value of the Risk Priority Number (RPN), which subsequently will be further identified a major focus in determining the root cause of potential use Fault Tree Analysis (FTA). From the roots of these problems can be determined proposed improvement and control in order to help the company to reduce the risk of production failures.
