Abstract

Diabetes Mellitus (DM) is a chronic disease that is often found with diabetic foot wounds. Diabetic foot ulcers can occur partially (Partial Thickness) or completely (Full Thickness) in the skin area that extends to the subcutaneous tissue, tendons, muscles, bones or joints. DM patients with diabetic foot wounds require long-term care to recover. The purpose of this study was to explore several wound care techniques both modern and conventional. This study uses electronic databases from PubMed, Google Scholar, and Garuda. The inclusion criteria of the articles in this study are: publications in 2015 – 2020, full text, articles using English or Indonesian, and research methods using experimental studies with full text PDF format. Found 4 studies with details of 1 case study methods, 1 quasi-experimental method, and 2 observation method. All articles were analyzed using the PICO (Population, Intervention, Comparator, Outcome) method. After treating diabetic foot wounds through TIME management, it has been found that modern therapeutic methods show a significantly higher rate of diabetic foot wound healing compared to conventional therapeutic methods for various types of diabetic foot wounds. Thus, DM patients with diabetic foot wounds can recover through wound care using dry and moist principles where modern wound care techniques are more effective than conventional treatment techniques (using sterile gauze).

Abstract

The incidence of fractures in Indonesia continues to increase with the highest cases being lower extremity fractures with a prevalence rate of 67.9%. Complications can occur due to one of the factors, namely the lack of patient mobilization. This study aims to determine the relationship between motivation and the implementation of early mobilization in postoperative lower extremity fracture patients at Meuraxa Hospital. The design of this study used a descriptive cross-sectional study approach. The sample in this study used purvosive sampling with a total sample of 36 lower extremity fracture patients. The univariate and bivariate data analysis used the Chi Square statistical test. The results showed that the results of the chi square test with a p value of 0.003 (p <0.05). This shows that there is a relationship between patient motivation and the implementation of early mobilization in lower extremity post fracture patients at Meuraxa Hospital Banda Aceh 2021.

Abstract

ABSTRAK Ketidaktahuan tentang diabetes melitus tipe 2 (DMT-2) yang terjadi dan keterlambatan merespon terhadap perubahan tubuh akibat DMT-2 dapat menimbulkan dampak yang fatal. Upaya pencegahan sukunder bagi pasien DMT-2 melalui kader kesehatan diperlukan untuk  mengendalikan  dan  mengurangi resiko terjadinya komplikasi akibat DMT-2. Oleh karena itu, pelatihan dan pendampingan kader kesehatan tentang latihan syukur dalam pengendalian gula darah diabetes perlu diberikan. Kegiatan pemecahan  masalah dilaksanakan dengan pembuatan modul, pemberian pelatihan dan pendampingan. Hasil  kegiatan  didapatkan  10  orang dengan kategori prediabetes dengan rentang usia 45 sampai 65 tahun. Selain itu, pengetahuan  kader  kesehatan  meningkat  sebesar  58,5  %  dilihat  dari  rata-rata skor pretest 60,2 menjadi rata-rata skor posttest 95,43. Kemampuan latihan syukur pada kader kesehatan dan pasien DMT-2 juga mengalami kemajuan yang semula mereka belum mampu melakukan latihan syukur dan pada akhir kegiatan mereka memiliki insight positif yang didukung dengan hasil akhir pemeriksaan kadar gula darah sewaktu dengan rata-rata sebesar 154 ml/dl. Pelatihan dan pendampingan kader kesehatan efektif dalam meningkatan pengetahuan dan kemampuan pasien DMT-2 tentang latihan syukur berbasis e-modul dalam pengendalian gula darah diabetes sebagai upaya pencegahan sekunder dan mengurangi risiko komplikasi akibat DMT-2. Kata Kunci: DMT-2, Kader Kesehatan, Pelatihan, Pendampingan  ABSTRACT Ignorance about the occurrence of type 2 diabetes mellitus (DMT-2) and delays in responding to changes in the body due to DMT-2 can have fatal consequences. Secondary prevention efforts for DMT-2 patients through health cadres are needed to control and reduce the risk of complications due to DMT-2. Therefore, training and assistance to health cadres regarding gratitude exercises in controlling diabetes blood sugar needs to be provided. Problem solving activities are carried out by creating modules, providing training and mentoring. The results of the activity showed that there were 10 people in the prediabetes category with an age range of 45 to 65 years. Apart from that, health cadres' knowledge increased by 58.5%, from an average pretest score of 60.2 to an average posttest score of 95.43. The ability to practice gratitude among health cadres and DMT-2 patients also experienced progress from previously not being able to do gratitude exercises and at the end of the activity they had positive insight which was supported by the final results of checking blood sugar levels with an average of 154 ml/dl. Training and mentoring of health cadres is effective in increasing the knowledge and abilities of DMT-2 patients regarding e-module-based gratitude exercises in controlling diabetes blood sugar as a secondary prevention effort and reducing the risk of complications due to DMT-2.  Keywords: DMT-2, Health Cadres, Training, Mentoring
