Abstract

Depreciation of medical record documents is an activity to separate documents that are declared active and inactive. If the implementation of depreciation is delayed, there will be a buildup of medical record documents that fill the medical record document storage rack. The accumulation of medical record documents makes storage shelves untidy and prone to misplacement of medical record documents (missfile). In Permenkes No. 269/MENKES/PERIII/2008 concerning medical records, medical records are files containing notes and documents regarding patient identity, examination, treatment, actions and other services provided to patients. The purpose of this study was to determine the implementation of shrinking medical record files at Madani Hospital Medan. This type of research is descriptive qualitative. This method is used to describe the cause of the non-implementation of shrinkage of medical record files at RSU Madani Medan. Collecting data in this study conducted interviews with medical record officers. Based on the results of research at Madani General Hospital in Medan, there were 6 medical record officers and only 4 people with RMIK D-III educational background. The implementation of sorting medical record files at RSU Madani Medan is not in accordance with standard operating procedures because the sorting process is more than 5 years old. Hospitals should provide training for filing officers. For non-medical record officers, medical record education and health information to better understand the implementation of depreciation. Hospitals also need to immediately make a retention schedule so that there is a regular schedule for retention.

Abstract

Analysis of the medical record workload is very necessary in improving health services in a hospital. This aims to determine which workloads need to be streamlined. In addition, with the analysis or measurement of workloads, scientific evidence-based decision making can be made. Seeing the large role of medical records in health services in hospitals, it is necessary what things are needed to improve the best quality in medical record installations. The purpose of this study was to determine the need for medical record personnel based on the workload in the filling section of Mitra Sejati General Hospital Medan. This type of research is descriptive with a cross sectional approach with a population of 4 medical record officers. The place where the research was conducted was at Mitra Sejati General Hospital Medan because the hospital had problems regarding work requirements in the filling section. The available working time is based on the number of effective working hours in the medical record section, which is 2200 hours/year, in 1 day working hours are 8 hours. The above regulation is based on the number of working hours of medical record officers at Mitra Sejati Hospital Medan, the number of outpatient visits is 6098 patients/year and for inpatients is 14789 patients/year while the standard time allowance for officers is 0.01 minutes. Based on the results of the calculation of the need for filling officers using the WISN method, the total human resource requirements in the outpatient filling unit were 2.89 or 3 officers while the outpatient filling unit officers at Mitra Sejati General Hospital Medan were 2 people. Meanwhile, the total human resource requirements for the inpatient filling unit are 3.11 or 3 officers, while the inpatient filling staff at Mitra Sejati General Hospital Medan are 2 people, so there is still a need for additional medical records officers specifically in the filling section.

Abstract

Sistem Pencatatan dan Pelaporan Terpadu Puskesmas (SP2TP) merupakan kegiatan pencatatan dan pelaporan puskesmas secara menyeluruh (terpadu) dengan konsep wilayah kerja puskesmas. Tujuan SP2TP adalah agar semua data hasil kegiatan Puskesmas dapat dicatat serta dilaporkan ke jenjang di atasnya sesuai kebutuhan secara benar, berkala dan teratur, guna menunjang pengelolaan upaya kesehatan masyarakat. Berdasarkan analisis dan pengamatan kami selama PKL di Puskesmas Medan Denai bahwa Sistem Pencatatan dan Pelaporan Terpadu Puskesmas (SP2TP) di Puskesmas Medan Denai yaitu dengan melaksanakan kegiatan merekapitulasi data yang di catat dengan baik di dalam gedung maupun di luar gedung serta mengumpulkan laporan dari masing-masing pelaksana kegiatan, lalu bersama dengan para pelaksana kegiatan membuat laporan bulanan SP2TP dan mengirimkan laporan tersebut ke Dinas kesehatan Dati II paling lambat tanggal 10 ditiap bulannya, Dati I dan Pusat. Hasil rekapitulasi/entri data, diperiksa diolah dan dimanfaatkan sebagai bahan untuk umpan balik, bimbingan teknis ke Puskesmas dan tindak lanjut yang diperlukan dalam rangka meningkatkan kinerja program Puskesmas tersebut.
