Abstract

This research aims to evaluate the anti-hyperuricemic activity of Syzygium polyanthum ethanolic extract in hyperuricemic male rats (Rattus norvegicus) induced by liver juice.  A total of 25 animals were divided into five groups: a negative control group, a positive control group, and three treatment groups receiving S. polyanthum extract at doses of 150, 200, and 250 mg/kg body weight, respectively. The result showed that the dose of 250 mg/kg body weight resulted in the highest decrease of uric acid plasma, measuring 3.44 ± 2.03 mg/dL. This reduction is comparable to the effect of allopurinol, which showed a decrease of 3.70 ± 1.54 mg/dL. A minimum dose-dependent activity was observed. To conclude, the administration of ethanolic extract of S. polyanthum for 14 days significantly reduced uric acid levels. Further exploration of higher doses or a long-term treatment period to enhance its effectiveness is needed.

Abstract

Hingga kini penggunaan antibiotik di rumah sakit cukup tinggi yang berisiko meningkatkan bakteri resisten akibat penggunaan antibiotik yang tidak rasional. Selain itu terdapat faktor lain yang menyebabkan berkembangnya bakteri resisten yaitu penggunaan antibiotik dengan indikasi yang tidak jelas, cara pemakaian yang kurang tepat, status obat yang tidak jelas, serta pemakaian antibiotik secara berlebihan. Metode Gyssens merupakan alat penilaian kualitatif yang sudah dipergunakan secara manual oleh PPRA di Indonesia. Evaluasi kualitatif penggunaan antibiotik dengan metode Gyssens menilai ketepatan penggunaan antibiotik yang meliputi ketepatan indikasi, ketepatan pemilihan berdasarkan efektivitas, toksisitas, harga dan spektrum, lama pemberian, dosis, interval, rute dan waktu pemberian. Tujuan penelitian ini adalah untuk mengembangkan suatu sistem informasi dan teknologi atau program perangkat lunak dalam mengevaluasi ketepatan penggunaan antibiotik dengan menggunakan metode Gyssens. Disain penelitian ini adalah non eksperimental dengan rancangan penelitian secara deskriptif. Pengambilan data dilakukan secara retrospektif di RSUD dr. Zainoel Abidin. Data diambil dari catatan pengggunaan obat dan rekam medis pasien dengan diagnosis penyakit infeksi yang mendapatkan terapi antibiotik. Data catatan penggunaan obat yang telah di review secara manual diolah untuk mengetahui rasionalitas antibiotik kemudian dibuatkan desain sebuah program perangkat lunak untuk memudahkan dalam evaluasi kualitatif. Hasil penelitian ini didapatkan pada penggunaan antibiotik empiris dari total 156 diperoleh sebanyak 85 (54,5%) yang rasional (kategori 0). Sementara pada penggunaan antibiotik definitif dari total 64 diperoleh sebanyak 33 (51,5%) yang rasional (kategori 0). Jenis antibiotik empiris yang banyak digunakan adalah meropenem sedangkan antibiotik definitif terbanyak digunakan adalah vancomicin. The use of antibiotics in hospitals is currently quite high which can increase the risk of bacterial resistance due to irrational use of antibiotics. The other factors that cause the development of bacterial resistance, including the use of antibiotics with inappropriate indications, inappropriate methods of use, unclear drug routes, and excessive use of antibiotics. The Gyssens method is a qualitative assessment that has been used manually by PPRA in Indonesia. Qualitative evaluation of antibiotic use using the Gyssens method to assess the appropriateness of antibiotic use which includes accuracy of indication, accuracy of selection based on effectiveness, toxicity, price and spectrum, duration of administration, dose, interval, route and time of administration. The aim of this research is to develop an information and hospital information system program to eliminate the use of antibiotics using the Gyssens method. This research design is non-experimental with a descriptive method. Data collection was carried out retrospectively at RSUD dr. Zainoel Abidin on inpatients in May to July 2023. Data was taken from drug use records and medical records of patients diagnosed with infectious diseases who received antibiotic therapy. Data on drug use records that have been manually reviewed to determine the rationality of antibiotics, then designed a hospital information system to facilitate qualitative evaluation. The results of this study were obtained on the use of empirical antibiotics, out of a total 156, 85 (54.5%) were rational (category 0). Meanwhile, out of a total 64 definitive antibiotics, 33 (51.5%) were rational (category 0). The most widely used type of empirical antibiotic is meropenem, while the most widely used definitive antibiotic is vancomycin. Keywords: antibiotic, Gyssens, qualitative, hopital information system
