Abstract

Background: Rational use of drugs plays a role in reducing morbidity and mortality in the community associated with the treatment of disease. Acute respiratory infections (ARI), diarrhea, and myalgia are diseases that are still commonly found and treated in primary health centers, so far the three diagnoses are considered potential for irrational treatment. Data from existing reports can actually be used to identify the rationality of rational drug use, but until now the quality of report data is still poor. Therefore, how interventions can improve the quality of reporting the performance indicators of rational drug use in the Karawang District. Objective: To improve the quality of reporting on performance indicators of rational drug use (POR) in Karawang District. Methods: This study used a Quasi-Experimental research design with one- group pretest-posttest design, without a comparison group (control), in Karawang District in January - March 2020. Twenty officers reporting rational drug use in primary health centers were selected by purposive sampling. Results: Training interventions can improve the quality of reporting the performance indicators of rational drug use from the results of increased pretest after the training intervention (posttest) namely; report preparation process 25% to 80%, report completeness from 45% to 95%, timeliness from 15% to 75%, report compliance with technical instructions from 0% to 75%. Conclusion: Providing interventions in the form of explanations of technical guidelines for rational drug use and training has been proven to improve the quality of reporting on performance indicators of rational drug use.

Abstract

Background: Continuity of drug availability in hospitals with no excessive supplies is still a problem. The range of inaccuracies in the plan for drug needs can be wasteful in procurement costs and affect the continuity of health services. Hospitals need to improve historical processing data on drug absorption to optimize drug demand planning. Objective: The research aimed to simulates optimizing the planning and control of drug inventory using the periodic review system. Methods: Case study research to plan drug needs, using daily drug uptake data longitudinally (2014-2017). The obtained formulas are used to perform simulations by comparing drug supplies based on existing methods with optimization methods on accurate uptake data in 2018. Interviews In-depth research is carried out on informants selected based on the relationship with system. Data analysis was performed using the t-test statistical test and qualitative data were analyzed using open coding. Results: An assessment of 10 types of drugs, each of 5 drugs has carried out in the fast moving and slow-moving groups. Eight out of the ten drugs reviewed, there were significant differences in drug supply between the existing method and the periodic review system method. The price of drug supplies during 2018 is Rp.13,315,337,556, while the existing method is Rp. 16,481,465,208. There were 25 cases of drug shortages worth Rp. 655,282,174 in the periodic review system method that is lower than the existing method, which is 31 instances of drug shortages worth Rp.756,743,295. Conclusion: The periodic review system method has advantages compared to existing methods in terms of fewer inventories and cost needs, and the occurrence of fewer drugs shortages and the budget to finance the drug shortages rates. Thus periodic review system method has the potential to improve the planning of drug needs in hospitals.

Abstract

Background: An increase in the number of type 2 Diabetes Melli tus (DM) patients at Mitra Sehat Clinic in 2018 as much as 200% compared to 2014. This has resulted in an increase in laboratory and drug claims to BPJS as the insurance agency. Analysis of drug use in patients with type 2 DM provides useful information regarding the management of antidiabetic drug use to determine the direction of management policy in efforts to control quality and costs. Objective: To analyse drug use and direct cost description of antidiabetic drugs at Mitra Sehat clinic using Prescribing Quality Indicator (PQI), for evaluation of the management of the patient referral program (PRB – PROLANIS) Type 2 Diabetes Mellitus. Methods: This study used a cross-sectional method with purposive sampling as a sampling technique. Subject inclusion criteria were PRB-PROLANIS patients with a single diagnosis of type 2 DM at Mitra Sehat Clinic aged 18 – 78 years who had an HbA1C examination, with a sample of 54 respondents. Results: The results of the statistical correlation test showed that there was a relationship between the use of antidiabetic drugs using the four PQI indicators with HbA1C (p value<0.05). There is the effectiveness of using antidiabetic drugs on the first, second, and fourth PQI indicators. Meanwhile, the use of antidiabetic drugs with the third indicator showed ineffectiveness. The use of antidiabetic drugs that has the highest direct cost is the use of the biguanide group, which is 43 respondents as much as Rp. 72,870,020, while the average total direct cost is the most expensive class of insulin. Conclusion: The use of antidiabetic drugs with PQI indicators can reduce HbA1C. Patients who received either insulin alone or insulin in combination did not achieve the target of therapy. The average total direct cost of the most expensive patients is those who use insulin drugs or a combination of insulin with other oral drugs.

Abstract

Background: Drug management realizes the availability of drugs needed to be effective and efficient, especially during the COVID-19 pandemic. The high use of COVID-19 drugs makes several types of drugs needed to experience vacancies, and due to changes in government regulations make some COVID-19 drugs stagnant.Objectives: To evaluate the supply of COVID-19 drugs during the pandemic and the factors that influence it in government and private hospitals in Makassar CityMethods: This study is a mixed-method study with a sequential explanatory design. Quantitative data were obtained retrospectively from January to December 2021 by searching drug management documents. Qualitative data was obtained through in-depth interviews with 12 informants.Results: The availability of COVID-19 drugs in government hospitals is 19.3 months, while in private hospitals, it is 13.6 months. Factors that affect drug supply are the method of planning drug needs, Lead Time, and several changes in guidelines.Conclusions: The supply of COVID-19 drugs is still excessive at that Public Hospital, and the factor that most influenced the supply of COVID-19 drugs was changes in guidelines.
