Abstract

The purpose of this activity is to optimize the performance assessment of nurses in the MPKP Kuantan Room, Tampan Mental Hospital, Riau Province, in order to improve the quality of comprehensive nursing services and patient safety. This optimization is expected to produce an objective, standardized, and sustainable performance evaluation system as a basis for improving nurse professionalism and the effectiveness of the implementation of the Professional Nursing Practice Model (MPKP). Community Service activities are carried out using the method of initial discussion, simulation and implementation of assessment as well as monitoring and evaluation. From the initial discussion through a mini workshop, it was agreed that issues related to performance assessment with activities carried out simulation and implementation of assessment as well as monitoring and evaluation. Simulation and implementation activities were carried out from July 22 to 30, 2025. Of the 14 nurses assessed, most had good performance. It is hoped that optimizing this assessment can increase the accountability of nurse performance, improve the quality of nursing care, and create a safe, effective, and sustainable service environment for patients with mental disorders.

Abstract

The increasing prevalence of elderly people has led to various changes that impact health problems, such as decreased heart function, one of which is hypertension. Hypertension is a circulatory system disorder that can cause blood pressure to rise above normal values, namely systolic pressure above 140 mmHg and diastolic pressure above 90 mmHg. Non-pharmacological therapy that can be provided to hypertension sufferers includes effluarge foot massage therapy combined with garlic oil. Based on observations of real-world conditions, it was found that hypertensive patients experienced acute pain. Treatment for acute pain includes effluarge foot massage therapy using garlic oil. The results of this study revealed that Mrs. S's pain assessment revealed P (high blood pressure), Q (stabbing and pressing), R (back of the head, neck, and nape), a pain scale of 4, T (recurring and intermittent), and BP of 170/100 mmHg. The nursing diagnosis for Mrs. S was acute pain related to increased blood pressure. The nursing intervention provided to Mrs. S was effluarge foot massage therapy using garlic oil to reduce pain in hypertensive patients. The nursing implementation was in accordance with the established interventions until the acute pain problem was resolved, and there were no significant differences compared to previous research. Nurses are expected to apply effluarge foot massage therapy using garlic oil to hypertensive patients to address acute pain.

Abstract

Cataract is a condition of cloudiness in the lens of the eye that causes decreased visual acuity. One common cause of cataracts is a long-term complication of Type 2 Diabetes Mellitus (DM). Type 2 DM is a chronic metabolic disease characterized by increased blood glucose levels due to impaired insulin secretion or insulin resistance. This case study research aims to provide nursing care for Mrs. S with Type 2 DM complicated by cataracts in the Pejuang Room of Bangkinang Hospital in 2025. The design of this study is a case study that includes assessment, nursing diagnosis, intervention, implementation, and evaluation. Data collection was carried out through interviews, observations, and physical examinations. The results of the assessment showed that Mrs. S had unstable blood glucose, impaired sensory perception (vision), and risk of injury. Interventions were carried out to help manage blood sugar levels, educate self-management, and prevent injury due to visual impairment. After three days of nursing care, the evaluation results showed that blood sugar levels were more stable, the client began to understand how to manage diabetes, and no injuries occurred during treatment. The results of this study are expected to provide information and understanding to DM patients about the importance of comprehensive disease management to prevent complications such as cataracts

Abstract

Pneumonia is a lower respiratory tract infection characterized by inflammation of the lung tissue due to bacterial, viral, fungal, or aspiration of foreign substances. This disease is one of the main causes of morbidity and mortality, especially in the elderly. This case study aims to provide a comprehensive overview of the implementation of nursing care for pneumonia patients treated in the isolation room of Bangkinang Regional Hospital in 2025. The approach used is the nursing process method which includes assessment, determination of nursing diagnosis, planning, implementation, and evaluation. The subject of the study was a 61-year-old male patient with the main complaint of coughing up phlegm accompanied by blood, shortness of breath, fever, and weakness. Based on the results of the assessment and supporting examinations, several nursing diagnoses were determined, including: ineffective airway clearance, hyperthermia, activity intolerance, ineffective breathing patterns, and risk of nutritional deficit. Nursing interventions were carried out in accordance with the Indonesian Nursing Intervention Standards (SIKI). Evaluation showed an improvement in the patient's condition after nursing actions were carried out for three days. This study emphasizes the importance of systematic and continuous nursing care to improve the health status of pneumonia patients.

Abstract

Hypertension is one of the major non-communicable diseases and a leading cause of death worldwide. The management of hypertension requires medication and regular blood pressure monitoring; however, patient adherence remains low. This study aimed to determine the relationship between treatment adherence and blood pressure control among hypertensive patients at UPT Puskesmas Tambang. The research employed a quantitative approach with a cross-sectional design. A total of 33 respondents were selected using accidental sampling. Data were collected through medical records and patients’ blood pressure measurements. The results showed that most patients were non-adherent to treatment (66.6%) and had uncontrolled blood pressure (72.7%). Fisher’s Exact Test yielded a p-value of 0.681 with a POR of 0.48, indicating no significant relationship between treatment adherence and blood pressure control. Nevertheless, adherent patients had a lower likelihood of experiencing uncontrolled blood pressure compared to non-adherent patients. It is recommended that healthcare providers enhance education and motivation for patients to regularly attend follow-up visits and comply with prescribed therapy. Hypertensive patients are also advised to adopt a healthy lifestyle to support treatment effectiveness.

Abstract

Appendicitis is an inflammation caused by infection and requires appendectomy surgery in its treatment procedure. The purpose of this study is to carry out nursing care in patients with post-op appendicitis in the Khadijah 2 room of Bangkinang Regional Hospital in 2025. This study uses a descriptive method in the form of a case study with data collection methods used, namely interviews, observations, physical examinations, data collection and documentation. Data obtained through interviews, physical examinations and supporting examinations. During the assessment, the patient said pain in the lower right quadrant surgical scar, pain appears during activity, pain appears suddenly, pain feels like being stabbed. So that researchers can formulate the main problem with the nursing diagnosis of acute pain related to physiological injury agents (surgical procedures) (D. 0007). The nursing implementation provided non-pharmacological techniques to reduce pain through deep breathing relaxation techniques. This case report is intended to provide or serve as a reference in nursing care for children with post-op appendicitis.

Abstract

Bencana hidrometeorologi seperti banjir bandang berdampak signifikan terhadap kondisi kesehatan masyarakat, terutama pada fase pascabencana. Keterbatasan akses layanan kesehatan dasar menjadi salah satu permasalahan utama yang dihadapi masyarakat terdampak. Kegiatan pengabdian kepada masyarakat ini bertujuan untuk meningkatkan akses pelayanan kesehatan melalui kegiatan pengobatan gratis bagi korban bencana di Desa Meunasah Lhok, Kecamatan Meureudu, Kabupaten Pidie Jaya. Kegiatan dilaksanakan pada Februari 2023 melalui kolaborasi antara dua perguruan tinggi swasta bidang kesehatan, Klinik Restu Ibu, dan Dinas Kesehatan Kabupaten Pidie Jaya. Metode pelaksanaan meliputi tahap persiapan, pelaksanaan, dan evaluasi kegiatan. Hasil kegiatan menunjukkan respons positif dari masyarakat serta meningkatnya akses terhadap pelayanan kesehatan dasar pascabencana. Sinergi lintas institusi berperan penting dalam mendukung kelancaran dan efektivitas kegiatan. Kegiatan ini menunjukkan bahwa pengabdian masyarakat berbasis kolaborasi multipihak dapat menjadi model strategis dalam mendukung pemulihan kesehatan masyarakat pascabencana.
