Abstract

The existence of health facilities in the form of hospitals can improve the health standard of the people of Denpasar, as evidenced by the high life expectancy in 2022 equal to 75.3 years. By increasing the level of public health, other sectors will receive a positive influence, especially the economic sector. The high need for health facilities in the city of Denpasar has caused many investors to start pursuing the health business, especially hospitals. Businesses in the Health sector provide considerable profits with a relatively high Initial Rate of Return (IRR) or investment return rate of 50.44% (Arwati, 2016). In addition to providing benefits to the community and businesses, the health business, in the form of hospitals, also has a significant negative impact on the environment. The characteristics of the waste generated by hospital operations are in the form of domestic waste and B3 waste (hazardous toxic materials) such as leftover syringes, leftover laboratory washing water and leftover reagents. The management of the resulting liquid B3 waste can be carried out with the operation of an WWTP (wastewater treatment installation). It is necessary to carry out calculations related to the performance efficiency of the WWTP so as to ensure that the processed water is suitable for reuse and to calculate the clean water savings that can be achieved in order to apply the concept of a sustainable economy. Based on the research results, the level of treatment efficiency which is quite high, which is more than 90%, will produce processed water that can be reused to support operational activities. The use of ground water for operational activities can be reduced by 60% or around 48 m3/day.

Abstract

The mangrove ecosystem within the Ngurah Rai Grand Forest Park (Tahura) in Benoa Bay experienced vegetation dieback and degradation across an area of 3,800 m², triggered in part by hydrocarbon contamination resulting from a fuel oil (biosolar) seepage from a pipeline near the Bali Mandara Toll Road exit. This study aims to determine the quantity and volume of recovery requirements based on the condition of the affected area, develop a cost estimate for each stage of mangrove ecosystem recovery by applying the remediation cost concept—defined as the cost required to restore contaminated environmental conditions to an established standard—and analyze the cost component that contributes most significantly to the total recovery cost. A descriptive quantitative approach was used, decomposing recovery activities into five cost component groups: preparation and supporting facilities, soil and land handling, bioremediation, revegetation–maintenance–monitoring, and temporary storage site (TPS) operations. The recovery area was set at 3,800 m² based on measurements by the National Land Agency, with an assumed excavation depth of 0.8 m, yielding an affected material volume of 3,040 m³. The results indicate a total estimated recovery cost of IDR 23,158,025,000, with preparation and supporting facilities—particularly the acquisition of an 800 m² TPS site—identified as the largest cost contributor. These findings suggest that decisions regarding TPS location and land ownership status significantly influence total recovery costs, and should therefore be a primary consideration in budget planning for the recovery of hydrocarbon-contaminated coastal ecosystems.

Abstract

The implementation of hospital service transformation through the Public-Private Partnership (Kerja Sama Pemerintah dengan Badan Usaha/KPBU) scheme requires adequate human resource (HR) readiness as a key supporting factor in improving service competency. This study was conducted to describe the planning and management of HR required to support the transformation process of a type B Regional General Hospital (RSUD) located in Bali Province. The transformation is directed toward improving service competency from its current condition to an intermediate (madya) competency level, particularly in five priority service areas: Oncology, Cardiology, Neurology, Uronephrology, and Maternal and Child Health (referred to by the Indonesian acronym KJSUIA), as part of the implementation of a KPBU project. To maintain the confidentiality of the as-yet-unpublished feasibility study document, the hospital's identity and project-specific figures are not disclosed explicitly, but are instead presented in the form of general patterns rather than raw data. This study employs a descriptive qualitative approach using document study methods. The findings indicate that HR needs calculations for this type of transformation project are generally derived from three complementary approaches: fulfillment of targeted intermediate competency standards, workload analysis (analisis beban kerja/ABK) based on the Decree of the Minister of Health No. 21 of 2004, and optimal utilization of existing non-medical support staff. Additional findings show that the hospital's institutional status as a Regional Public Service Agency (Badan Layanan Umum Daerah/BLUD) tends to provide structural flexibility in HR governance, supported by a clearly defined organizational structure led by a Director and several Deputy Directors. Furthermore, a number of specialist and subspecialist physician competencies are generally not yet available or are still undergoing further education (fellowship) at the pre-feasibility study stage, indicating the need for structured recruitment, continued fellowship training support, and periodic evaluation to ensure the successful implementation of such transformation efforts.
