Abstract

Pendaftaran pasien merupakan proses awal terjadinya rekam medis di setiap fasilitas pelayanan kesehatan, di tempat pendaftaran pasien akan di data identitas pribadi pasien serta keperluan kunjungannya ke rumah sakit. Selain mencatat data identitas pasien, akan diperlukan juga pendataan lain mengenai penanggung jawab pasien, asuransi, pekerjaan, dan lain sebagainya. Dalam sistem pendaftaran, pasien akan diterima sebagai pasien rawat jalan, dan rawat inap. Tujuan dilaksanakannya kegiatan pengabdian masyarakat ini untuk meningkatkan pengetahuan anak asrama tentang alur pendaftaran pasien di Rumah Sakit atau fasilitas pelayanan kesehatan. Kegiatan pengabdian dilakukan dalam beberapa tahap yaitu tahap persiapan, tahap pelaksanaan, serta tahap evaluasi. Kegiatan pengabdian masyarakat dilaksanakan pada bulan Agustus 2022. Hasil pengabdian diperoleh nilai pretest 61,90 dan nilai posttest. 77, 14. Terjadi peningkatan pengetahuan anak asrama setelah dilakukan penyuluhan tentang alur pendaftaran pasien di Rumah Sakit 15,24 poin, yang berarti bahwa penyuluhan tentang alur pendaftaran pasien di rumah sakit yang dilaksanakan berdampak terhadap peningkatan pengetahuan peserta penyuluhan.

Abstract

Burns are the destruction or death of tissue that occurs when tissue comes into contact with heat. Burned skin causes damage to the epidermis, dermis, and tissue lining under the skin on the causative factors and length of contact with heat sources. These burns are caused by human activities every day, whether in family work, companies, traffic accidents, or natural disasters. The purpose of this study is to find out an overview of the level of knowledge of mothers about first aid. This type of research uses a descriptive design, and sampling in this study uses an accidental sample with a total of 58 respondents. It can be concluded that this study obtained results from Definitions in the good category of 52 respondents (89.7%), as well as Etiology in the good category of 29 respondents (50.0%), and Implementation as many as 40 respondents (69.0%). It is hoped that with the increase in maternal knowledge about burns, nurses need to provide education and counseling to the community so that the level of maternal knowledge about burns can increase at the Romana Tanjung Anom Clinic 2024.

Abstract

The accuracy of the bronchopneumonia case code is that all diagnoses are filled out correctly based on the completeness of filling out the medical record with code J18.0 as the code in the ICD 10 book. Completeness of filling out the medical record is a medical record that has been completely filled out by a doctor within 1x24 hours, after completing outpatient services. or after an inpatient is decided to go home which includes a patient identity sheet, medical resume, and supporting diagnostic examination sheet. Researchers take specific cases of bronchopneumonia because during the practice service at St. Hospital. Elisabeth in 2021 there are 10 files of bronchopneumonia patients, there are 3 medical record files with incorrect codes. Based on this background, researchers are interested in conducting research with the title of code accuracy and completeness of medical records of bronchopneumonia patients at St. Hospital. Elisabeth Medan 2022. The type of research design used was a descriptive design with a sampling method of 82 respondents in total at St.Elisabeth Hospital Medan 2022. The instrument used in data collection was a checklist study documentation. The results of the study obtain that the correct category code was 70 medical records (85.3%) and the incorrect category are 12 medical records (14.7%) medical record files. While the completeness of the patient identity sheet is 85.3%, the medical resume sheet is 85.3%, and the laboratory support results sheet is 81.7% complete.

Abstract

Medical record is a document that contains patient identity data, examination, treatment given to patients. Factors for damage to medical record documents can be caused by intrinsic damage, meaning that damage that can occur comes from the object itself and extrinsic factors are the cause of document damage caused from outside the archive object. The research objective was to describe the factors that influence damage to medical record documents at Elisabeth Hospital Medan in 2022. The research design uses descriptive analysis which explains and also describes the characteristics of each variable. The population in the study was 59,956 medical record documents. The sampling technique used systematic random sampling, in order to obtain a sample for this study, namely 100 medical record documents taken from January to December 2022. The results of the research paper quality were mostly (78%) good and a small portion (22%) were not good. Most of the ink quality (64%) is good and a few (36%) is not good. The adhesive quality is almost entirely (99%) good and a small portion (1%) is not good. Extrinsic damage includes: judging from the physical factors, most of them (59%) are good and there are a few (41%) that are not good. All biological and chemical factors are good (100%) and there is no biological or chemical damage. Suggestions for research conducted by researchers at the Elisabeth Hospital in Medan are expected to provide new knowledge and information about document damage found in the medical record storage room.
