Abstract

Premature rupture of membranes is the leading cause of preterm labor with its consequences. The incidence of KPD approaches 10% of all deliveries. In the case of labor <34 weeks, the incidence was about 4%. The dangers of KPD are the possibility of uterine infection and preterm delivery which may increase maternal and infant morbidity and mortality. The purpose of this study is to analyze the magnitude of risk factors associated with premature rupture in maternity mothers at the Gajah Mada Community Health Center from 2014 to 2016. The type of this research is Cross Sectional Descriptive Analytic. Sample was taken by using Total Sampling. The number of samples in this study was 134 people. The data was expanded through medical record. The data was analyzed by using Chi Square test with p<0,05. The results showed that the variables associated with the incidence of premature rupture of membranes were the p-value of age on 0.000 (p<0.05) and OR (6,711) women with a high risk of life having 6.7 times getting the incidence of KPD compared with women with low risk age. The p-value of work on 0.000 (P <0.05) and OR 8.606, working mothers would have 8.6 times experiencing KPD rather than those who do not. The p-value of gemelli pregnancy on 0.001 (P <0.05) and 4.667 mothers with gemelli pregnancy would get 4.6 times receiving KPD rather than those who do not. The variables which are unrelated to the incidence of premature rupture of membranes are parity, anemia, and breech delivery.

Abstract

Immunization is to actively induce or increase a person’s immunity to a disease. Current immunization is to prevent about 2 to 3 million deaths each year. According to the data of Directorate for Disease Prevention and Control, Ministry of Health of the Republic of Indonesia, shows that there were 1.7 million children had not been immunized or incomplete immunized since 2014 to 2017. The purpose of the research is to analyze the correlation among the mothers’ knowledge, the family supports, the health workers supports, and the completeness of basic immunization on babies and toddlers in RT 1-5, Sungai Buluh, Pulau Palas. This research is an analytical descriptive with cross sectional study. The population and sample of the research were all mothers who have babies and toddlers. The sampling technique was accidental sampling (30 respondents). Data collection used questionnaire sheets. Data analysis was chi-square test with p < 0.05. The result verifies that the variables related to the completeness of the immunization was support provided by health workers (p value is 0.003 (p<0.05)). Accordingly, it is highly expected that the health workers cultivate collaboration with health cadres and local stakeholders to enrich mothers’ knowledge about the significance of immunization for babies and toddlers over counseling and discussion during posyandu. They are to perform good communication with family members (husband, mother, in-laws), so the family support can increase the coverage of complete basic immunization in babies and toddlers.

Abstract

Immunization is an attempt to cause or actively increase a person's immunity to a disease. The achievement of Universal Child Immunization (UCI) in Auxiliary health center of Teluk Tuasan Village 44.67%, this shows it has not reached the national target of 86%. The purpose of this study is to find out the factors that affect the provision of basic immunization in infants (0-12 months) in the Auxiliary health center of Teluk Tuasan Village, Illiran River Health Center in 2019. This research is qualitative descriptive with a phenomenological approach. The study was conducted from July 9 to July 11, 2019. The number of samples of the main informants in this study was 6 people and 6 supporting informants (key informants). The results of the study obtained knowledge, family support, and the number of children affect the provision of basic immunization while the work and affordability of the place of service does not affect the provision of basic immunization. It is expected that the agency where the research can provide counseling to mothers and families about the importance of immunization, as well as the activeness of mothers in Integrated Healthcare Center activities against complete basic immunization in infants, so that the success of immunization programs can be achieved optimally.

Abstract

The nutritional status of toddler is an important factor because their development period is vulnerable to nutrition. More than a third of child deaths in the world are related to the problem of malnutrition every years. Gajah Mada Tembilahan Health Center recorded in 2017 the number of malnutrition toddlers is 45 and increased in 2018 as many as 49 toddlers. The study aim to determine the overview of toddlers nutritional status at MCH Poly, PHC Gajah Mada Tembilahan in 2018. The research is descriptive with a cross sectional design and a retrospective approach. The study was conducted at the MCH Poly, PHC Gajah Mada Tembilahan on August 8th to 17th 2019. The population is used total sampling was all toddlers aged 12-59 months who had visited the MCH Poly of PHC Gajah Mada Tembilahan in 2018 with number 634 toddlers. The results obtained, from 634 toddlers, 386 (60.9%) toddlers with good nutritional status. It is hoped that the research institution can continue to provide counseling and education for mothers toddlers to continuous monitoring the growth and development of their toddlers to reduce the number of malnutrition in future.

Abstract

The number of maternal deaths In Indragiri Hilir Regency in 2018 was 9 people and neonates 21 people. The high death rate of mothers and neonates is expected to be able to carry out their duties professionally and qualifiedly. The purpose of this care is able to provide comprehensive midwifery care services to maternity mothers and newborns at Mrs"S" G3P2A0H2 gestational age 39 weeks in accordance with obstetrics management in the VK room of Nilam Sari Tembilahan Main Clinic on July 29, 2019. This upbringing uses varney's documented 7-step management in SOAP form. The result of obstetric care there is a gap between theory and practice that is not in accordance with the 60-step guidelines of APN, namely not assessing DJJ every uterine contraction completed because the head decline is rapid, does not do an overall assessment because there is still care to be done, the early initiation of breastfeeding is not up to 1 hour because the mother has refused and lack of knowledge about the benefits of the early initiation of breastfeeding, Not immunizing hepatitis B 1 hour after vitamin K because it provides the injection distance to the baby and the maximum dilation phase to the deceleration phase is only 1 hour because it is strong. Health workers are expected to improve services in providing midwifery care in accordance with the 60-step APN guidelines in order to reduce existing gaps and explain the benefits of care provided.

Abstract

Approximately 60% of maternal deaths occur after childbirth and nearly 50% of it happened in the first 24 hours after childbirth. The total of post-partum mothers at the Bunda Puja Maternity Hospital Tembilahan were 271 people which from January to December 2019 was 195, and January to July 2020 was 76. The aim of this care is to provide midwifery care for Mrs. "E" P2A0H2 at Bunda Puja Maternity Hospital Tembilahan that was documented in SOAP form and analyzed their gaps in theory and practice. This care was carried out at the Bunda Puja Maternity Hospital Tembilahan on July 7th to August 4th, 2019. The data collection method used midwifery management by using varney mindset and documented in SOAP form. The results show there were gaps in theory and practice in the management of the third postpartum visit, where the mother did not do postpartum exercise due to the absence of the trainer / instructor, postpartum gymnastics facilities and infrastructure while on the third visit one of the care provided is postpartum exercise. It is hoped that it can maintain and improve midwifery care in the normal postpartum period, can facilitate postpartum gymnastics and for midwifery care centers can make overall visits to postpartum mothers.
