Abstract

Pembengkakan payudara jika tidak ditangani dengan segera dapat menimbulkan masalah baru yaitu mastitis dan bahkan abses payudara. Tujuan dilakukan riset ini yaitu untuk mengetahui pengaruh perbedaan penurunan pembengkakan payudara ibu nifas menggunakan kombinasi perawatan payudara dan kompres aloe vera. Riset yang dilakukan menggunakan jenis quasi experimental dengan Intact-Group Comparison., dengan desain post-test with control group. Ibu bersalin sejumlah 45 orang adalah populasi untuk riset ini, dengan sampel 32 yang diambil dengan cara purposive sampling. Pada riset ini menggunakan uji man whitney karena data berdistribusi tidak normal. Alat ukur pada penelitian ini menggunakan SPES. Riset ini mengahasilkan adanya ada perbedaan pembengkakan payudara ibu nifas pada kelompok kombinasi perawatan payudara dan kompres aloe vera dengan pembengkakan payudara ibu nifas pada kelompok perawatan payudara, dengan nilai p value 0,002 < 0,05. Diharapkan kepada petugas kesehatan untuk meningkatkan pelayanan kesehatan ibu nifas terutama dalam masalah penatalaksanaan payudara bengkak pada ibu nifas dan dapat meningkatkan penyuluhan pada ibu nifas tentang kondisi fisik dan psikologi ibu nifas.

Abstract

Pendahuluan: World Health Organization (WHO) dan United Nations Children’s Fund (UNICEF) merekomendasikan pemberian nutrisi yang optimal bagi bayi baru lahir melalui strategi global pemberian ASI eksklusif selama enam bulan. Tidak semua ibu post partum langsung mengeluarkan ASI karena pengeluaran ASI merupakan suatu interaksi yang sangat komplek antara rangsangan mekanik, saraf dan bermacam-macam hormon yang berpengaruh terhadap pengeluaran oksitosin. Penurunan produksi ASI pada hari – hari pertama setelah melahirkan dapat disebabkan oleh kurangnya rangsangan hormon oksitosin dan prolaktin yang sangat berperan dalam kelancaran ASI, sehingga menyebabkan ASI tidak segera keluar setelah melahirkan. Metode: Penelitian ini bertujuan untuk mengetahui adakah pengaruh pijat endorphin terhadap kelancaran ASI pada ibu menyusui di desa sinomwidodo. Jenis penelitian ini menggunakan metode pre-experimental, dengan desain yang digunakan adalah one-group pretest-posttest design. Penelitian ini terdiri dari 1 kelompok perlakuan yaitu kelompok pijat endorphin. Populasi dalam penelitian ini adalah 20 ibu nifas di Desa Sinomwidodo. Adapun untuk teknik sampling menggunakan purposive sampling. Sampel dalam penelitian ini sebanyak 20 responden. Hasil: Dapat disimpulkan berdasarkan hasil uji hipotesis menunjukkan nilai sig = 0,001 <0,05 sehingga terdapat pengaruh yang signifikan endorphin massage terhadap kelancaran ASI pada ibu menyusui. Saran: Hasil penelitian ini diharapkan menjadi masukan bagi profesi bidan dalam upaya meningkatkan pelayanan tambahan pada masa nifas serta sebagai treatment untuk mengatasi masalah kelancaran ASI pada masa nifas. Abstract

Introduction: A decrease in breast milk production in the first days after giving birth can be caused by a lack of stimulation of the hormones oxytocin and prolactin which play a very important role in the smooth flow of breast milk, causing breast milk not to come out immediately after delivery and the baby having difficulty sucking on the mother's nipple. Currently, in midwifery services, complementary-based services have emerged, which often include breast massage to overcome irregularities in breast milk production. Breast massage is the act of gently massaging the breasts which can be done every day. Method: This study aims to determine the effectiveness of breast massage using the "Z" technique on breast milk production in postpartum mothers. This type of research uses a Quasi-experimental method, the design used is Nonequivalent control group design. This study consisted of 2 (two) groups, namely a control group and a treatment group who were given breast massage using the "Z" technique. The research subjects were 30 postpartum mothers who breastfed their babies. The sampling technique uses purposive sampling. Results: The results of the research were that of the 15 postpartum mothers in the treatment group after breast massage using the "Z" technique, the majority experienced smooth breast milk production, 11 people experienced smooth breast milk production, and 4 postpartum mothers experienced fairly smooth breast milk production. Meanwhile, the 15 postpartum mothers in the control group mostly experienced fairly smooth breast milk production for 9 people, 2 people had breast milk flow smoothly and 4 people had less than smooth breast milk production. It can be concluded based on the results of the hypothesis test showing that the p value < α is 0.000< 0.05 so that breast massage using the "Z" technique is effective on breast milk production in postpartum mothers with an average increase of 144.50 ml. Recommendation: It is hoped that the results of this research will provide input for the health profession in an effort to increase the attainment of exclusive breastfeeding and to overcome the problem of breast milk production during the postpartum period. Breast massage is breast massage, which is gentle massaging of the breasts which can be done every day. This study aims to determine the difference between breast massage using the "Z" and "knuckle" techniques on breast milk production in postpartum mothers. Method: This type of research uses a Quasi experimental method, the design used is Nonequivalent control group design. This research consisted of 2 treatment groups, namely breast massage with the "Z" technique and the "Knuckles" technique. The research subjects were 20 postpartum mothers who breastfed their babies. The sampling technique uses purposive sampling. Results: The results of the research showed that out of 10 postpartum mothers after breast massage using the "Z" technique, the majority experienced smooth breast milk production, 8 of them experienced smooth breast milk production. Meanwhile, of the 10 postpartum mothers after breast massage using the "Knuckles" technique, the majority of them experienced smooth milk production, 7 of them experienced smooth milk production. It can be concluded that based on the results of the hypothesis test, the sig value (-2 tailed) = 0.429>0.05 so that there is no significant difference between breast massage using the "Z" technique and the "Knuckles" technique on the smooth flow of breast milk in postpartum mothers. Recommendation: This research recommendation can be used as a solution for postpartum mothers in overcoming the problem of smooth breastfeeding during the breastfeeding period for babies aged 0-6 months.
