Abstract

Polycystic ovary syndrome with insulin resistance (SOPK-RI) is caused among endocrine disorders with an incidence rate of 5-10% that occurs in women of reproductive age. Characteristics of polycystic ovary syndrome are found anovulation, hyperandrogens, polycystic ovarii and impaired insulin sensitivity involving proinflammatory increases in several inflammatory cytokines including interleukin-6 (IL-6). Increased inflammatory cytokines in women with SOPK-RI found health problems there is fat accumulation in adipocyte tissue such as in an obese person, metabolic syndrome and type 2 diabetes. The purpose of this study was to determine the effect of a low-carbohydrate high-protein diet on changes in IL-6 levels in SOPK-RI model mice. Laboratory experimental research, namely post test only control group design. Consisting of three groups, namely, the negative control group (K-) as a normal group did not get treatment; The positive control group (K+) of the SOPK-insulin resistance model mice were given standard feed, and the treatment group (P) of the SOPK-insulin resistance model mice were given a low-carbohydrate high-protein diet (CTR) with a composition of 40% carbohydrate and 30% protein. The study lasted for 48 days, using serum ELISA examination on IL-6 levels. The average results of IL-6 levels, namely the K- = 0.358 group, K+ = 0.387 and P = 0.442 and obtained different IL-6 levels of significance value p = 0.002 with a low-carbohydrate high-protein diet in SOPK - RI rats showed a significant difference (p < 0.05) between the control group and the treatment group. Significantly reduced IL-6 levels in SOPK-RI model mice. A low-carb, high-protein diet may be considered as a dietary modality in women with SOPK-insulin resistance. Further research is needed to better determine the other modulators involved in this mechanism in more detail.

Abstract

Dokter Muter (Near by Doctors and other health workers through integrated services) is an innovation of the Lumajang District Health, Population Control and Family Planning Office (Dinkes P2KB). This innovation aims to bring access to health services closer to remote or geographically challenging areas. The purpose of this study was to determine the effect of health service quality and the Doctor Muter service innovation on patient satisfaction in Lumajang District. The population of this study were patients who had received or were receiving Dokter Muter services in 25 health centers. The data collection method was a questionnaire using Likert scale. This study used a cross-sectional design with proportional stratified random sampling technique. The number of samples in this study was 384 respondents. Data analysis was done using path analysis. Based on the results of the study, it is known that the path coefficient value of service quality on patient satisfaction β = 0.234 is positive with a p-value of 0.000, so that service quality has an influence on patient satisfaction. The path coefficient value of Dokter Muter’s service innovation on patient satisfaction β = 0.639 is positive with a p-value of 0.000, so Dokter Muter's service innovation has an influence on patient satisfaction. The R-squared value for the patient satisfaction variable is 0.707, which means that 70.7 percent of the patient satisfaction variable is influenced by Dokter Muter's service quality and service innovation, and the remaining 29.3 percent is influenced by constructs outside the model.

Abstract

This study examines therapeutic interventions for expressive language disorders in early childhood through a qualitative case study approach. Using an indigenous psychology perspective, data were collected through observations, semi-structured interviews, and document analysis over a two-month period at a psychology and therapy institution. The findings indicate that behavior-based therapeutic interventions, when flexibly adapted to children’s developmental characteristics and contextual conditions, effectively support expressive language development. Differences in age and communicative readiness influenced therapeutic strategies and child responsiveness. The study also highlights the importance of parent–therapist collaboration in creating supportive communicative environments, particularly in contexts with limited parent–child verbal interaction. Although limited in scope, this study provides context-sensitive insights into therapeutic practices for expressive language disorders and contributes to a deeper understanding of early language intervention in early childhood education and therapy settings.
