Abstract

Upper gastrointestinal bleeding is one of the most frequently encountered emergency cases. For most general practitioners, diagnosing and providing initial management is a challenge in itself. Upper gastrointestinal bleeding is divided into variceal and non-variceal bleeding. Clinical symptoms that can be found include hematemesis or melena. Patients with melena tend to have lower hemoglobin (Hb) levels than hematemesis because clinical manifestations appear more slowly. Drug Induced NSAIDs are also one of the etiologies that can cause upper gastrointestinal bleeding. Endoscopy is the gold standard for diagnosis, but not all health facilities are able to perform it. This case reported a 66-years-old male patient came to the emergency room with complaints of blackish stools with a soft, sticky consistency for 6 days, accompanied by weakness, dizziness, nausea in the stomach and disruption of daily activities. The patient has a history of taking over-the-counter NSAIDs long term to relieve his knee pain. On physical examination, hepatomegaly and splenomegaly were not found and the patient’s hemodynamics is stable. On laboratory examination, Hb was found to be 3.4 g/dl. It was suspected that there was upper gastrointestinal bleeding, then received therapy in the form of PPI injections, antiemetic injections and planned PRC transfusion.

Abstract

BACKGROUND: Secondary Polycythemia Vera due to Eisenmenger Syndrome remains a big challenge, and the reported cases regarding this condition are still scanty. CASE: This was a case of a 22-year-old girl who, since childhood, had complaints of fatigue most of the time and thus showed more intolerance to heavy physical work. She had a history of a congenital heart defect, with lip and finger cyanosis that increased upon fatigue, and she had undergone Bi-directional Cavo-pulmonary Shunt. Physical examination showed cyanosis with clubbing of fingers with low oxygen saturation. Further diagnosis testing revealed this was a case of Eisenmenger Syndrome due to congenital heart abnormalities, with Polycythemia Vera: Secondary to the diagnosis. The patient also had a hemoglobin level of 21.1g/dL and a hematocrit level of 64.2%. Based on the diagnosis, treatment was given in the form of hydration therapy along with medications. Additionally, phlebotomy was performed. Because the patient improved clinically from the applied treatment, the patient was discharged after 5 days. CONCLUSION: Early diagnosis, a multidisciplinary approach, and proper interventions are important in the management of Eisenmenger Syndrome and associated Polycythemia Vera to avoid complications and hence improve outcomes.

Abstract

Staycation has been growing and continuously gaining popularity in Indonesia during and after the COVID-19 pandemic. This study aims to identify segments of staycation tourists/staycationers in Jakarta, one of Indonesia's most popular staycation destinations. Using quantitative methods, this study implements a proportional sampling design based on accommodation types with a total of 130 valid and reliable online questionnaire surveys. This study identifies factors and clusters using lifestyle variables from Activity, Interest, and Opinion (AIO) Model and examines their correlation with demographic, geographic, and behaviour segmentation. Data were analysed by descriptive statistics, including factor analysis, cluster analysis, discriminant analysis, and cross-tabulation analysis. The findings revealed four types of staycationers in Jakarta: (1) I-want-everything staycationers, (2) independent and attention seekers, (3) indulgent shopaholics, and (4) culture and local adventurers. Profiles and characters of each staycationer segment were analysed, giving significant knowledge about staycation market, particularly in Jakarta. Thus, this study offers valuable information for tourism and hospitality businesses to focus on the most potential market segment.
