Abstract

Post-ventilator patients can experience Post Intensive Care Syndrome (PICS) problems. This includes physical, psychological and cognitive changes. This research aims to analyze the incidence of PICS in post-ventilator patients in Surabaya. The design of this study used a descriptive research method with research variables including physical, psychological and cognitive health. The population of post-ventilator patients amounted to 56 patients with a sample of 49 patients using the simple random sampling technique. The instrument used in this study was a questionnaire sheet. Which consists of 16 questions, divided into physical, psychological and cognitive aspects. Post intensive care syndrome in post-ventilator patients, the majority of the physical aspects are in the sufficient category, such as changes in fatigue and pain. In the psychological (mental) aspect, most are in the sufficient category marked by anxiety and sadness. The cognitive aspect is also mostly in the sufficient category which includes changes in patient memory. Post intensive care syndrome in the physical, psychological and cognitive aspects is in the sufficient category. This is marked by mild changes. The recommendation in this study is that patients need to get health education only if they experience PICS.

Abstract

Anesthesia services are part of perioperative services that have a major influence in determining the success of adequate surgical procedures and avoiding the risk of anesthetic injury to patients. Anesthesia providers in carrying out their professional practices are authorized to provide anesthesia care services in pre-anesthetic assessments. To determine the implementation of pre-anesthetic assessments in patients with general anesthesia in the Central Surgery Installation Room of Buleleng District Hospital. This study used a qualitative research design, and a Multiple Case Study research method with a Descriptive approach. The number of participants in this study was 2 participants who met the inclusion criteria. Data collection tools used interview checklists, Observation sheets, and Pre-Anesthesia Assessment Implementation Documentation sheets. The results of this study on the examination of the participant's anamnesis carried out an assessment using the Foundational Four and Sacred Seven, on the physical examination the participant did not carry out BMI and peripheral nerve disorders. And in the pre-anesthesia documentation, the participant wrote the assessment results on the documentation sheet during the operation and post-operation. The anesthesia technician in carrying out the assessment of the patient's anamnesis, reviewing the physical examination, supporting examination and the patient's psychology almost in accordance with the applicable SOP, except for the examination of BMI, peripheral nerve disorders, providing therapeutic greetings at the end of the procedure and in documenting the procedure did not comply with the applicable SOP, this was due to the short time during pre-anesthesia.

Abstract

Preoperative anxiety is a common and significant issue that can negatively impact patient outcomes. Effective communication and empathy from anesthesia providers have been suggested as crucial factors in mitigating this anxiety. This study aimed to investigate the relationship between anesthesia provider communication and empathy and preoperative anxiety levels in adult patients undergoing elective surgery. A prospective, observational study was conducted at a tertiary care hospital. Adult patients scheduled for elective surgery under general anesthesia were enrolled. Preoperative anxiety was assessed using the State-Trait Anxiety Inventory (STAI) before the anesthesia provider's preoperative visit and again immediately before surgery. Anesthesia provider communication and empathy were evaluated using the CARE (Communication Assessment Tool for Anesthesia Providers) questionnaire and the Jefferson Scale of Empathy (JSE), respectively. A total of 200 patients were included in the study. The mean preoperative anxiety score decreased significantly from before the anesthesia provider's visit to immediately before surgery (p < 0.001). Higher scores on the CARE questionnaire and the JSE were associated with a greater reduction in preoperative anxiety (p < 0.05). Multivariable analysis revealed that both effective communication and empathy independently contributed to reducing preoperative anxiety. Anesthesia provider communication and empathy play a crucial role in reducing preoperative anxiety. Implementing communication and empathy training programs for anesthesia providers may improve patient experiences and surgical outcomes.
