UPAYA KEGIATAN PANDU DAN EDUKASI PENCEGAHAN ANEMIA PADA IBU HAMIL DI WILAYAH KERJA PUSKESMAS KARANGGEDE BOYOLALI Oktasari, Nur Aida

Abstract

ABSTRAK Anemia defisiensi besi merupakan anemia paling banyak ditemukan pada ibu hamil yang terkait dengan angka kematian ibu dan janin. Berdasarkan data yang didapat dari Pemerintah Kabupaten Boyolali didapatkan bahwa data cakupan capaian pemberian Fe30 yaitu 89% dan Fe90 mencapai 71% di wilayah kerja Karanggede Boyolali, akan tetapi masih banyak dijumpai kasus anemia pada Ibu hamil, oleh karena itu diperlukan upaya khusus untuk menurunkan angka kematian ibu hamil akibat anemia. Upaya kegiatan pandu edukasi bertujuan untuk mencegah anemia pada ibu hamil.  Purposive sampling dilakukan pada 22 ibu hamil bulan November 2020 di Desa Klari. Terdapat 7 ibu hamil yang mengalami anemia.  Edukasi menggunakan media leaflet, video, materi presentasi, pemberian buku saku diet gizi seimbang, pengisian buku food recall dan buku kepatuhan tablet Fe. Ibu hamil dengan anemia dilakukan pemeriksaan hemoglobin sebelum dilakukan intervensi. Evaluasi pengisian buku food recall dan buku kepatuhan konsumsi tablet Fe secara rutin dilakukan 1 minggu sekali melalui kunjungan rumah. Sedangkan pemeriksaan kadar hemoglobin dilakukan setelah 1 bulan intervensi dengan hasil uji paired T- test menunjukkan hasil peningkatan yang signifikan dengan nilai p: 0.007. Hal ini menunjukkan bahwa kegiatan pandu edukasi pencegahan anemia pada ibu hamil berhasil meningkatkan kadar hemoglobin pada ibu hamil dan menurunkan angka anemia pada ibu hamil.   ABSTRACT Iron deficiency anemia is the most common type of anemia found in pregnant women, associated with maternal and fetal mortality rates. Based on data obtained from the Boyolali District Government, it was found that the coverage of Fe30 provision reached 89% and Fe90 reached 71% in the Karanggede Boyolali working area. However, many cases of anemia in pregnant women are still encountered, hence specific efforts are needed to reduce maternal mortality due to anemia. Educational outreach efforts aim to prevent anemia in pregnant women. Purposive sampling was conducted on 22 pregnant women in November 2020 in Klari Village. Seven pregnant women were found to have anemia. Anemia prevention was carried out through education using leaflets, videos, presentation materials, provision of balanced nutrition pocket books, filling out food recall books, and Fe tablet adherence books. Pregnant women with anemia underwent hemoglobin testing before intervention. Evaluation of food recall book and Fe tablet adherence book filling was routinely done weekly through home visits. Meanwhile, hemoglobin level examination was conducted after 1 month of intervention, with paired T-test results showing a significant increase with a p-value of 0.007. This indicates that the guided education activities for anemia prevention in pregnant women successfully increased their knowledge regarding the consumption of iron-rich foods and adherence to regular Fe tablet consumption.

Abstract

Introduction: Changes in blood viscosity can cause thrombosis in the brain, causing a stroke. Polycythemia vera is one of the causes of blood hyperviscosity. This study aimed to present cases of cerebral infarction related to polycythemia vera. Case presentation: A 55-year-old woman came to the emergency room with complaints of sudden weakness in the left limbs. One week before admission, the patient complained of headaches accompanied by spinning dizziness. Her left limbs often tingled and became weaker. There was an asymmetric facial deviation to the right, and the patient had difficulty communicating with unclear articulation. Specific examination of neurology shows dysarthria, left-sided motor type facial paralysis, examination of motor strength of the upper and lower extremities on the left 2 and 5 on the right, and the left upper limb often feels tingling. On the left limb, normal physiological biceps reflex was found, positive Chaddock pathological reflex. Morphological examination of peripheral blood showed normocytic normochromic with erythrosis and thrombocytosis and found giant platelets. This patient was diagnosed with an ischemic stroke caused by cerebral infarction, with polycythemia vera etiology, and accompanied by comorbid diseases of diabetes mellitus and uncontrolled hypertension. Conclusion: In patients with high-risk criteria for thrombosis, the management of phlebotomy, administration of low-dose aspirin, and cytoreduction in the form of hydroxyurea aims to achieve a hematocrit <45% so as to prevent worsening and recurrence of stroke.
