Abstract

The lack of the leadership function in monitoring and evaluation, the limited training on patient safety is suspected to have resulted in a low safety culture and clinical risk management. This study aims to determine the influence of transformational leadership, patient safety knowledge and culture on clinical risk management at PMI Hospital. Design of a cross-sectional study, 138 nurses in August 2024. Simple random sampling technique, the independent variable is knowledge about patient safety goals and transformational leadership; the dependent variable of clinical risk management, and the intervening variable of patient safety culture. Descriptive analysis shows that PMI Hospital has nurses who have high knowledge of patient safety targets, with a charismatic leadership style, and a communicative patient safety culture. The results show that knowledge of patient safety goals, transformational leadership, and patient safety culture simultaneously and partially have a significanteffect on clinical risk management so that high knowledge, charismatic leadership, and communicative patient safety culture are proven to encourage improvements in clinical risk management at PMI Hospital.

Abstract

The background of this research is relevant to evaluate and improve services in Mayapada Hospital Kuningan , especially in the obstetrics polyclinic. By understanding the relationship between family support and medical personnel, medical personnel competence, patient experience to the obstetrics polyclinic as an intervening variable, and the decision of the birth plan. In addition, there is also a need to evaluate the factors that influence the decision of the birth plan. The purpose of the study is to analyze influence family support and medical personnel, the competence of medical personnel simultaneously with patient experience at the obstetrics polyclinic regarding the decision to plan childbirth at Mayapada Hospital Kuningan. Method Study, this is study associative causality with cross sectional design. This study aims to determine the influence of family support and medical personnel (X1), the competence of medical personnel (X2) on the decision of birth plan (Y) with the patient's experience to the obstetric polyclinic as an intervening variable (Z). The findings of this study are from the statistical test analysis we can find that the most influential variables on the decision of birth plan are family support and medical personnel. The results of the study say that the patient's experience to the polyclinic as an intervening variable does not affect the decision of birth plan. The implications of this study can help management in improving the system for family support and medical personnel, the competence of medical personnel and the patient's experience to the obstetric polyclinic so that it is expected to improve the decision of birth plan to the hospital.

Abstract

Non-formulary drug utilization in Harapan Kita Children’s and Mother’s Hospital presents a substantial challenge in healthcare cost management, especially for patients enrolled in the BPJS health insurance program. Given its status as a national referral hospital, Harapan Kita Children’s and Mother’s Hospital encounters a high prevalence of complex maternal and child health cases requiring the use of medications beyond the scope of the national formulary. This research endeavor seeks to examine the patterns and consequences of non-formulary drug use within this context. This study adopted a quasi-experimental design, incorporating both quantitative and qualitative methods, to evaluate the impact of an sosialisation intervention and audit clinic on physician compliance with formulary drug use. A pre-test and post-test design was employed to measure changes in non-formulary drug utilization. Findings indicated a statistically significant decrease in non-formulary drug use from 18% to 13% post-intervention. The integration of a clinical pathway and tailored physician training was instrumental in achieving improved formulary adherence. Given these results, it is recommended to broaden training and surveillance programs, strengthen technological support through the implementation of formulary information systems, and undertake longitudinal research to assess the long-term sustainability of the intervention's effects..

Abstract

This research is aimed at analyzing the influence of service quality and physical facilities on WoM (Word of Mouth) with Patient Experience as an intervening variable at Mekar Sari Hospital. The research method used in this research is a correlational quantitative research type with a cross-sectional research design approach by looking at the influence of service quality and physical facilities on WoM with satisfaction as an intervening variable. The sampling technique used purposive sampling, namely a sampling technique using the criteria of internal medicine inpatients at Mekar Sari Hospital. The research sample involved 110 respondents. Path analysis is used as a WoM model analysis tool with patient experience as an intervening variable. The results of this study found a joint positive influence of service quality and physical facilities on WoM with patient experience as an intervening variable. The quality of service and physical facilities have a positive influence on both patient experience and WoM.

Abstract

Background: Patient safety culture forms the cornerstone of safe and high-quality healthcare delivery. However, its implementation often encounters barriers, particularly the persistence of a blaming culture that discourages staff from reporting patient safety incidents. Clinical leadership and the intensity of incident reporting are believed to play a pivotal role in shaping and sustaining a positive patient safety culture. Objective: This study aims to examine the influence of clinical leadership and patient safety incident reporting intensity on patient safety culture, with blaming culture serving as an intervening variable at Sentra Medika Cikarang Hospital. Methods: A quantitative research approach with an explanatory design was applied. The study involved 147 nurses selected through stratified random sampling. Data were collected using structured questionnaires and analyzed using Structural Equation Modeling (SEM) with the Partial Least Squares (PLS) method to test the direct and indirect relationships among variables. Results: The findings revealed that both clinical leadership and incident reporting intensity significantly influence patient safety culture, both directly and indirectly, through the mediation of blaming culture. Strong clinical leadership and a high level of incident reporting were associated with a more positive patient safety culture, while a high blaming culture weakened this relationship. Conclusion: The study underscores the importance of fostering supportive clinical leadership and cultivating a non-punitive reporting environment to strengthen patient safety culture. Hospital management should focus on leadership development and the creation of open, blame-free communication systems to enhance safety outcomes.
