Abstract

Patient safety is one of the aspects of providing excellent service for patients in hospitals, especially in the ICU. One method of managing patient safety is to prevent pressure ulcers in the ICU. The aim of this study is to explain the management of pressure ulcer prevention in the ICU for bedridden stroke patients using an intervention approach with form chart media. The research method used in this study is a case study design that includes assessment processes, data analysis, diagnostic rehabilitation, intervention, implementation, and evaluation. We collected data through observation, interviews, and physical examinations. We carried out the intervention in two shifts. We measured the risk of pressure ulcers using the Braden scale, provided health education to the family, and ensured the nurse correctly filled out the form. The obstacle to increasing the number of nurses is the workload and the feeling that human resources are still insufficient. There are no signs of pressure sores on the patient's back. In the implementation of the pressure ulcer prevention program using the skin approach, family involvement is necessary to ensure the program continues without adding to the workload of nurses in the ICU. Conclusion: Modifications to the formchart and family involvement in the program are necessary for the implementation of the pressure ulcer prevention program using formchart media in the ICU.

Abstract

Patient safety is a system that makes patient care safer, including risk assessment, identification of patient risk management, reporting and analysis of incidents, the ability to learn from incidents and their follow-up, and implementation of solutions to minimize risks and prevent injuries caused by errors. as a result of carrying out an action or not taking the action that should be taken. The hospital inpatient room is devoted to services for patients requiring more handling related to patient safety measures. This case study aims to find out the description of patient safety culture in hospital inpatient rooms. This case study uses a sample of 16 nurses in the inpatient ward, and the instrument used is the Hospital Survey of Patient Safety Culture Version 2.0, translated into Indonesian. It consists of 42 questions in 5 parts. The questionnaire results describe positive or negative values. This shows that in all aspects of the questionnaire, indicators > 70%, namely positive responses including the work unit section 70%, your supervisor 73.45%, the communication section 97.93, the incident frequency report section 91.7%, and the natural hospital section promotes patient safety 78.42%. The results of each indicator show that the picture of patient safety culture in the Hospital Inpatient Room is good, with a positive response of 85.25%. This research hopes to improve performance in patient safety culture as a health service provider

Abstract

Ventilator Associated Pneumonia (VAP) is a lung parenchymal infection that occurs in patients after using mechanical ventilation for 48-72 hours. Patients with ventilator associated pneumonia (VAP) will show new or progressive infiltrates on chest x-ray, experience systemic infections such as fever and changes in white blood cell count, changes in sputum characteristics, and indicate detection of the causative agent in patients with VAP. Preventive measures to prevent the occurrence of VAP in ICU is oral hygiene. Oral hygiene is an oral care procedure to clean the mouth, teeth and gums using 1% povidone iodine. This study aims to evaluate the effect of administering oral hygiene with 1% Povidone Iodine in preventing Ventilator Associated Pneumonia (VAP) in patient users. Mechanical Ventilator in the ICU at Dr. Haryoto Lumajang Hospital. The research method used is a case study method. The research instrument used in this research is Standard Operating Procedures (SOP). Modified Clinical Pulmonary Infection Score (MCPIS) observation sheet. The results showed that the MCPIS score from day 1 to day 8 was ?5 so that the patient was declared not diagnosed with VAP after being given oral hygiene intervention with povidone iodine 1% with a frequency of 2 times a day for 8 days. It is hoped that this research will be an effort to prevent the occurrence of VAP in patients using mechanical ventilators and be able to reduce patient mortality rates.
