Abstract

Diphtheria is an acute infectious disease caused by the bacterium Corynebacterium diphtheriae. Accurate and prompt diagnosis is essential for effective case management, predicting disease prognosis, preventing complications, and ensuring cost-effective medical intervention. This study aimed to assess the variety of clinical symptoms exhibited by pediatric diphtheria cases during an outbreak. An observational cross-sectional study was conducted using data from the medical records of pediatric diphtheria cases at Sulianti Saroso Infectious Disease Hospital from November 1, 2017, to February 28, 2018. The study involved 202 cases, and statistical analysis was performed using the chi-square test. Out of the 202 cases, 58.4% were male. Age distribution was <1 year: 7.4%, 1–2 years: 3.5%, >2–5 years: 24.8%, >5–12 years: 45.5%, and >12 years: 18.8%. Anamnestic findings revealed the presence of fever in 88.1% of patients, pain upon swallowing in 73.3%, and cough in 55.4%. Clinically, every patient exhibited pseudomembrane formations. Other findings included bilateral tonsillar involvement in 53%, lymphadenopathy in 40.1%, bullneck in 17.8%, and snoring in 7.9%. Four significant variables were associated with the diphtheria diagnosis: fever, snoring, bullneck, and snoring (p<0.05) respectively. Clinical signs and symptoms are pivotal in establishing a diphtheria diagnosis in pediatric cases.

Abstract

Background: Children with HIV undergoing treatment cannot be separated from the provision of Antiretroviral Drugs (ARVs). ARV adherence is a very important treatment for HIV sufferers, especially for children because it can improve their quality of life. There are several interventions that can be done to improve ARV treatment adherence, one of which is by providing Short Message Service (SMS). The SMS method has been proven to help improve children's adherence in undergoing ARV treatment. Purpose: To provide an overview of the effect of short message service (SMS) interventions on improving ARV treatment adherence in children with HIV. Method: This study used a systematic review to obtain the latest scientific evidence. The research method was carried out by following the guidelines according to the recommendations of the PRISMA statement. Results: Five articles that have been reviewed reported that SMS interventions can improve treatment adherence in children with HIV. Conclusion: An SMS-based approach can be an effective and feasible method to improve treatment adherence in children with HIV.   Keywords: Adherence; Antiretroviral (ARV); Children with HIV; Short Message Service (SMS).   Pendahuluan: Anak dengan HIV yang menjalani perawatan tak terlepas dari pemberian Antiretroviral (ARV). Kepatuhan ARV merupakan pengobatan yang sangat penting dilakukan pada penderita HIV, khususnya bagi anak karena dapat meningkatkan kualitas hidupnya. Ada beberapa intervensi yang dapat dilakukan untuk meningkatkan kepatuhan pengobatan ARV, salah satunya adalah dengan pemberian Short Message Service (SMS). Metode SMS terbukti dapat membantu meningkatkan kepatuhan anak dalam menjalani pengobatan ARV. Tujuan: Untuk memberikan gambaran pengaruh intervensi pesan singkat (SMS) terhadap peningkatan kepatuhan pengobatan ARV pada anak dengan HIV. Metode: Penelitian ini menggunakan tinjauan sistematis (systematic review) untuk mendapatkan bukti-bukti ilmiah terkini. Metode penelitian dilakukan dengan mengikuti panduan sesuai rekomendasi dari pernyataan  dalam  PRISMA. Hasil: Lima artikel yang telah di review melaporkan bahwa intervensi SMS dapat meningkatkan kepatuhan pengobatan pada anak dengan HIV. Simpulan: Pendekatan berbasis SMS dapat menjadi metode yang efektif dan layak untuk meningkatkan kepatuhan terhadap pengobatan di kalangan anak yang hidup dengan HIV.   Kata Kunci: Anak dengan HIV; Antiretroviral (ARV); Kepatuhan; Pesan Singkat (SMS).

Abstract

Background: Antiretroviral treatment is the treatment for people infected with human immunodeficiency virus (HIV) using anti-HIV combinations that are used for life. The main factors determining the success of ARV treatment are sustained and optimal adherence to therapy. Adherence to ARV treatment can be challenging for children and especially adolescents with HIV because ARVs must be taken for life. There are several ways to improve adherence to ARV medication directly, namely through comprehensive educational pressure, other ways can be done by utilizing technology that uses medication reminder applications such as the use of smartphones, android-based application development, electronic monitoring, Short Message Service (SMS), social media-based support, and mass media campaigns. This paper reviews the scientific evidence on technology-based ARV treatment adherence. Methods: This article is based on scientific journal searches conducted through online databases, namely Scopus, Proquest, PubMed, and Scholar, published in 2019-2023. Results: showed that using technology as a reminder to take medication can improve adherence of children with HIV in taking ARV drugs. Conclusion: The use of technology can improve adherence in children with HIV, can also reduce viral load rates, and increase CD4 levels

Abstract

Latar Belakang: Penggunaan kateter intravena perifer (PIVC) adalah praktik invasif yang umum dalam pengaturan perawatan kesehatan. Diperkirakan sekitar 70% orang dengan PIVC akan mengalami komplikasi seperti infeksi. Infeksi pada area kateter intravena yang sering terjadi adalah phlebitis, dimana terjadinya peradangan pada tunika intima vena, yang dapat terjadi akibat kateterisasi vena. Untuk mengurangi adanya oklusi pada kateter instravena yang menyebabkan phlebitis adalah dengan dilakukannya flushing. Tujuan: meninjau bukti ilmiah (evidence base) tindakan flushing. Metode: Artikel ini dibuat berdasarkan telaah  jurnal yang berkaitan dengan flushing yaitu 2017-2022 dengan strategi pencarian melibatkan basis data Science Direct, ProQuest, Springer link. Kriteria inklusi dalam penelusuran literatur ini adalah artikel yang terkait dengan flushin, artikel penelitian kuantitatif, artikel fulltext dan berbahasa Inggris yang publish tahun 2015-2022. Sementara kriteria eksklusi dalam penelusuran literatur adalah jenis artikel literature review atau sistematik review. Hasil: Flushing  dengan menggunakan NaCl 0,9% terbukti dapat mempertahankan kepatenan kateter intravena perifer. Kesimpulan: Flushing dapat mencegah terjadinya phlebitis pada pasien yang dipasang infus perifer.
