Abstract

Background: Foreign body of fractured tracheal cannula in tracheobronchial tract is one of rarecomplication of tracheostomy. Symptoms appear in the form of respiratory distress and if not properlytreated may threaten patient’s life. Objective: To know the causative factors, management, and preventionof tracheal cannula foreign body in tracheobronchial tract. Case report: A 3-year-old boy reported with a foreign body of fractured tracheal cannula in his bronchus after tracheostomy. Method: Review of evidence-based literatures with keywords ”foreign body” AND ”tracheal cannula” on Google Scholar. Result: Extraction of foreign body of fractured tracheal cannula was performed by rigid bronchoscopy via the stoma, under general anesthesia, in all of the case reports in the literatures. After the procedure, the respiratory distress improved and patients could be discharged from the hospital. Conclusion: The definitive management of foreign body of fractured tracheal cannula in tracheobronchial tract by using rigid bronchoscopy transtracheostomy under general anesthesia is effective. Education and knowledge about post-tracheostomy care and follow up, are the keys to prevent such incidents. Keywords: foreign body, tracheal cannula, bronchoscopy

Abstract

Latar Belakang: Kelenjar seruminosa adalah kelenjar tubular melingkar yang mirip dengan kelenjar ekrin pada kulit dengan sifat kelenjar mengeluarkan kandungan di dalamnya menuju infundibulum folikular rambut atau secara langsung di atas permukaan kulit. Adenoma seruminosa merupakan suatu tumor kelenjar seruminosa yang bersifat jinak dan langka dengan kurang dari 1% dari keseluruhan tumor telinga luar. Tujuan: Menjelaskan diagnosis dan tatalaksana suatu kasus adenoma seruminosa. Laporan Kasus: Seorang laki-laki berusia 47 tahun mengeluhkan benjolan pada liang telinga kanan sejak 4 bulan lalu disertai penurunan pendengaran dengan hasil tuli konduksi berat pada tes audiometri. Adenoma seruminosa dikonfirmasi melalui pemeriksaan histopatologi dan imunohistokimia. Eksisi tumor dilakukan dengan pendekatan retroauricula. Metode: Telaah literatur berbasis bukti dengan kata kunci “Ceruminous” AND “Adenoma” AND ”External Ear Canal” melalui Google Scholar. Hasil: Diagnosis adenoma seruminosa dapat dibuat melalui anamnesis dan pemeriksaan fisik disertai dengan bantuan pemeriksaan penunjang. Pemeriksaan radiologi seperti CT scan dan MRI dapat membantu menentukan batas dan perluasan dari tumor. Diagnosis pasti adenoma seruminosa didapatkan melalui hasil positif pada pemeriksaan imunohistokimia CK7, S100 dan p63. Tatalaksana adenoma seruminosa ialah dengan pembedahan reseksi en bloc dengan batas yang memadai. Kesimpulan: Adenoma seruminosa merupakan salah satu tumor benigna pada kanalis akustikus eksternus. Tindakan pembedahan eksisi luas pada tumor merupakan tatalaksana utama bagi adenoma seruminosa. Prognosis kasus adenoma seruminosa termasuk baik.
