Abstract

 Preeklamsi berat adalah hipertensi onset baru (tekanan darah sistol ? 160 mmHg atau diastol ? 110 mmHg) setelah usia kehamilan ? 20 minggu. Berdasarkan onset perjalanan penyakit preeklamsi dibagi menjadi preeklamsi awitan dini dan preeklamsi awitan lambat. Ibu dengan preeklamsi biasanya dilahirkan lebih awal untuk mencegah terjadinya komplikasi ibu dan janin. Keputusan pemilihan metode persalinan tidaklah mudah, karena sebagian besar kehamilan yang masih jauh dari aterm dan dengan kondisi serviks yang belum baik, cara persalinan pada ibu dengan preeklamsi harus dilahirkan dengan pertimbangan obstetrik rutin. Penelitian ini bertujuan untuk mengetahui apakah terdapat perbedaan luaran maternal pada pasien preeklamsi berat awitan dini dan lambat berdasarkan metode persalinan. Penelitian ini adalah penelitian deskriptif observasional desain cross sectional dengan menggunakan data sekunder dari rekam medis RS Muhammadiyah Palembang dengan besar 555 sampel yang telah memenuhi kriteria inklusi. Hasil penelitian didapatkan komplikasi ibu pada preeklamsi berat awitan dini dan preeklamsi berat awitan lambat lebih banyak ditemukan pada metode persalinan sectio caesarea.

Abstract

Severe preeclampsia (PE) is a major cause of maternal and perinatal complications. Early-onset (EOPE) and late-onset (LOPE) severe preeclampsia can impact fetal growth, leading to low birth weight (LBW). This study aims to determine the association between EOPE, LOPE, and LBW incidence at Muhammadiyah Hospital Palembang. A retrospective case-control study was conducted from October 2021 to January 2022. A total of 269 cases (10 EOPE, 259 LOPE) and 269 controls were analyzed. Data were obtained from hospital records and assessed using the chi-square test and odds ratio (OR). Results show LOPE prevalence (96.3%) was higher than EOPE (3.7%). LBW incidence was 100% in EOPE cases and 21.18% in LOPE cases. A significant association was found between EOPE, LOPE, and LBW (p = 0.001, OR = 63.75), indicating a 63.75-fold increased risk. Both EOPE and LOPE increase the risk of LBW, with EOPE posing a greater risk. Early detection and management of preeclampsia are crucial for improving perinatal outcomes.

Abstract

Background: Palembang City has the highest prevalence of anemia in South Sumatra, Indonesia. Protein contributes to hemoglobin formation and insufficient protein intake increases the risk of anemia. However, Sumatera studies on protein intake based on protein sources in pregnant women are still limited. Objective: This study aimed to examine the association between animal protein consumption and anemia prevalence in pregnant women. Methods: A cross-sectional analytic study was conducted from July to October 2024 at six public health centers and seven private midwife clinics selected to represent Palembang’s geographic diversity. A total of 62 pregnant women in their second or third trimester were recruited using consecutive sampling. Dietary intake was assessed via interviewer-administered 2×24-hour recalls on non-consecutive days. Protein intake was analyzed using modified NutriSurvey 2004 and categorized based on the Indonesian RDA. Hemoglobin levels were measured using venous blood samples and classified according to trimester-specific anemia thresholds. Bivariate analysis with Fisher’s exact test was used to examine the association between animal protein intake and anemia prevalence. Results: The Fisher Exact Test showed a statistically significant association between animal protein consumption levels and anemia in pregnant women in Palembang City (p = 0.018). The mean total protein intake was 80.40 ± 20.53 g/day, with animal and vegetable proteins contributing 40.22 ± 18.63 g/day and 40.91 ± 24.5 g/day, respectively. Mean hemoglobin level was 11.18 ± 1.17 g/dL. Conclusion: Sufficient consumption of animal protein was found to be more protective against the occurrence of anemia.
