Abstract

Community empowerment is an important approach to improving health at various social, economic, and cultural levels. This approach emphasizes the active role of communities in managing and improving their own well-being, including aspects of physical, mental, and social health. Community empowerment involves the process of developing the knowledge, skills, and capacity of individuals and groups to understand and overcome health problems. This can include health education, the promotion of healthy lifestyles, strengthening access to health services, and building community-friendly health infrastructure. Through collaboration between governments, non-governmental organizations, and the private sector, community empowerment can be an effective means to improve the accessibility and quality of health services, as well as to reduce health disparities among different groups of people. The implementation of community empowerment programs has shown a significant impact on increasing health knowledge, improving healthy living behaviors, and reducing preventable disease and mortality. The development of strategies and policies that support community empowerment is an important step in achieving sustainable development goals in the health sector

Abstract

Background: Diabetes mellitus is a chronic disease that occurs when the pancreas cannot produce enough insulin or the body cannot use the insulin produced effectively. Insulin is a hormone that regulates blood sugar. Purpose of service: To provide health education about diabetes mellitus for the community. Method: This service activity is carried out through direct observation of the community and dor to dor education. Results show that counseling can increase the knowledge of each Pretest by 48% and Posttest by 78%. Conclusion: Their knowledge is also increased by providing health education to the public. This community service activity can have an impact on improving the degree of public health.

Abstract

Introduction: Hypertension is a major public health concern among the elderly, contributing significantly to morbidity, mortality, and reduced quality of life. Home care visits represent a community-based approach to enhance disease management, promote treatment adherence, and improve well-being among older adults. This study aimed to evaluate the effect of home care visits on the quality of life (QoL) of elderly individuals with hypertension in a primary health center area. Methods: A quasi-experimental pre–post design with a control group was conducted involving 100 elderly participants diagnosed with hypertension. The intervention group received structured home care visits conducted by community nurses over a period of eight weeks, focusing on health education, medication adherence, diet modification, blood pressure monitoring, and lifestyle counseling. The control group received standard outpatient care. Quality of life was assessed using the WHOQOL-BREF instrument, and blood pressure measurements were recorded at baseline and post-intervention. Data were analyzed using paired t-tests and ANCOVA to compare changes between groups. Results: The results showed a significant improvement in the mean QoL score of the intervention group (from 56.3 ± 7.5 to 68.7 ± 6.8; p < 0.001), particularly in the physical and psychological domains. Mean systolic blood pressure decreased by 11.4 mmHg and diastolic by 6.8 mmHg (p < 0.05). No significant change was observed in the control group. Conclusion: Regular home care visits effectively enhance the quality of life and blood pressure control among elderly patients with hypertension. Integrating home-based nursing interventions within primary health services can promote holistic, continuous care and support aging-in-place initiatives.
