Abstract

ABSTRAK Ketuban pecah dini (KPD) adalah pecahnya selaput ketuban sebelum adanya tanda-tanda persalinan, dan dapat terjadi pada kehamilan aterm maupun pada hehamilan prematur. Inflamasi merupakan kondisi umum yang mendasari terjadinya KPD. Desain penelitian ini adalah penelitian cross sectional deskriptif observasional. Sampel penelitian ini adalah sampel jenuh yang diambil selama 4 bulan (bulan Agustus-November 2019). Bahan pemeriksaan berupa selaput ketuban yang diambil saat persalinan pasien-pasien yang didiagnosis PROM di RSPAL Surabaya. Kemudian dilakukan pemeriksaan patologi anatomi untuk melihat sebaran sel radang PMN. Total sampel adalah 20 pasien. Berdasarkan hasil pemeriksaan histopatologi didapatkan bahwa 11 dari 20 (55%) selaput janin mengandung sel radang polimorfonuklear dan 9 (45%) sisanya tidak mengandung sel radang polimorfonuklear. Lebih dari separuh kasus pecahnya selaput ketuban disebabkan oleh terjadinya keradangan pada selaput ketuban. Kata kunci: ketuban pecah dini, sel radang, sel inflamasi, polimorfonuklear, histopatologi. ABSTRACT Premature of membranes (PROM) is the rupture tht amniotic membranes before there are signs of labor, and can occur i8n term pregnancies as well as in premature pregnancy. Inflammation is a common condition that underlies the PROM. The design of this research is a cross sectional descriptive study. Examination material in the form of amniotic membranes taken during delivery of patients diagnosed with PROM at RSPAL Surabaya. The methods sampling is a saturated sample taken for 4 months (August – November 2019), then an anatomic pathology analysis was carried out to see the distribution of PMN inflammatory cells. The total sample is 20 patients. Based on the results histopathological examination, it was found that 55% fetal membranes contained polymorphonuclear cells and 45% without polymorphonuclear cells. More than half of the cases of rupture membranes are caused by inflammation. Keywords: premature rupture of the membranes, inflammatory cells, polymorphonuclear cell, histopathology.

Abstract

The normal birth process is determined by 3 main factors, namely maternal power, pelvic size and fetus. Normal delivery is vaginal delivery with a fetus in occiput posterior presentation that proceeds naturally without the help of tools. If there is no occiput posterior presentation, there is a possibility that labor will be obstructed even though the shape and extension of the baby are within normal limits. Thus, a careful examination must be carried out to determine the position of the head in the pelvic cavity. Here we will explain several head positions that often cause dystocia.
