Abstract

Background: quality of life is an individual's perception of his or her position in life in the context of culture, value systems, and its relationship to life goals, expectations, standards, and other related things. Objective: this study aims to determine the effect of Acupressure in improving the quality of life of hemodialysis patients. Methods: The research used in this study was systematic review and meta-analysis using the PICOT method, by searching through Pubmed, Embese, and Proquest databases, so that as many as 6 articles were obtained that discussed the effectiveness of Acupressure in improving the quality of hemodialysis patients. Results: studies showed that most of the studies revealed that there was an effect of acupressure on improving quality of life in patients with hemodialysis. Acupressure actions such as improving sleep quality, reducing the intensity of pain, depression and anxiety. In conclusion, this acupressure therapy can be useful in improving the quality of life experienced by hemodialysis patients.Clinical implications: The results of this study are an indication of the importance of nonpharmacological therapy in nursing intervention by providing acupresuree to improve the quality of life of patients with chronic kidney failure disease while undergoing hemodialysis and patients can independently intervene as an effort to prevent and self-manage at home so that the patient's quality of life is improved during treatment. Keywords: Acupresuree, Hemodialysis, Quality of Life

Abstract

Background Fracture is a condition of partial or complete bone discontinuity. Upper extremity fractures can be caused by direct trauma (such as accidents) or indirect trauma (such as excessive pressure). Fracture management is performed through surgery such as Open Reduction Internal Fixation (ORIF), which can cause pain and muscle stiffness. Early mobilization with isometric exercises is needed to prevent these problems. Objective This study aims to determine the effect of digital-based Isometric Exercise on pain levels and muscle strength in patients post-ORIF upper extremity fracture. Methods The study used a Quasi-Experimental design with a Pretest-Posttest Control Group approach, involving 34 respondents divided into two groups. Muscle strength was measured using a Hand Grip Dynamometer, while pain was measured using the Brief Pain Inventory Short Form (BPI-SF). Isometric exercise intervention was performed 24 hours after surgery, twice daily with a duration of 35 minutes per intervention. Results The study showed a more significant increase in muscle strength in the intervention group compared to the control group with a Mean difference increasing to 4.23 (95% CI: 3.28-5.18), t=9.062, df=32, p=0.000. Pain levels also decreased more in the intervention group with a Mean difference of 1.81 (SD=0.866), 95% CI: 1.51-2.11, t=12.2, df=33, p=0.000. Conclusion: Isometric exercise is effective as early mobilization to reduce pain and increase muscle strength in ORIF upper extremity fracture patients. Abstract

Background: Cochlear implantation in children with congenital ear malformations presents unique perioperative challenges due to anatomical variations. Comprehensive perioperative care is essential for optimal outcomes, yet literature on nursing management for these complex cases remains limited. Objective: To document the perioperative nursing approach for cochlear implantation in a pediatric patient with congenital ear malformation and establish evidence-based protocols for similar cases. Case: A 9-year-old child diagnosed with bilateral profound sensorineural hearing loss with Mondini malformation (incomplete partition) underwent left cochlear implantation. The patient had previously undergone right cochlear implantation two years prior with successful outcomes. Perioperative management included comprehensive preoperative assessment with special attention to craniofacial anomalies, intraoperative monitoring for cerebrospinal fluid leakage and facial nerve function, and postoperative care focusing on pain management, infection prevention, and family education. Despite anatomical challenges, the procedure was successfully completed with 10 of 12 electrodes functioning properly, and the patient was discharged on postoperative day two without complications. Conclusion: Successful cochlear implantation in children with congenital ear malformations requires a multidisciplinary approach with specialized perioperative nursing care. This case demonstrates that with proper preoperative planning, intraoperative vigilance, and comprehensive postoperative management, good functional outcomes can be achieved despite complex anatomical challenges. Standardized protocols for perioperative care in these cases can significantly improve patient safety and surgical outcomes.
