Abstract

This study aims to develop and validate a module on managing diet, lactation, and emotion for postnatal mothers. This cross-sectional study involves three phases: needs assessment, module development, and module validation. About 37 mothers aged 18 to 49 years old with infants less than 6 months were recruited, and information on postnatal care practices, lactation knowledge, and emotion were obtained for module development. The guidelines from the Ministry of Health Malaysia and literature served as the foundation for the module that was developed following the requirements assessment utilizing the Health Belief Model (HBM). Content and face validity of the module is carried out to validate the developed module. The validation form used was adapted from Silveira de Castro (2007) which consists of seven criteria, two criteria related to content validity and another five criteria related to face validity. The content validity was assessed using a Content Validity Index (CVI). CVI was calculated using two different formulas: I-CVI and S-CVI. Meanwhile, a level of agreement was used in the data for the module's face. From the need assessment, most mothers with restricted seafood and nuts intake struggled to breastfeed and about 21.6% of them experienced postpartum depression. The developed module has been validated with both mean I-CVI and S-CVI more than 0.78 and 0.80, respectively. Meanwhile, five face validation components achieved 75% agreement, confirming the validity. In conclusion, the developed module named The LEDies has good validity and can serve as teaching material for postnatal mothers caring for themselves and their infants during the postpartum period.

Abstract

This study aims to determine infant feeding practices including breastfeeding initiation, exclusive breastfeeding, duration of breastfeeding and complementary feeding, feeding practices related factors among mothers with Gestational Diabetes Mellitus (GDM) history, and the association between Infant and Young Child Feeding indicators and GDM status. A cross-sectional study (n=130) was conducted from February to June 2023 in Meru and Bandar Botanic health clinics. Data on infant feeding practices were gathered from GDM mothers and children aged below two years. Findings showed that 43.8% (n=57) of the mothers had initiated breastfeeding within an hour, 57.7% (n=75) infants were exclusively breastfed, 30.0% (n=39) continued breastfeeding for one year and 81.9% (n=68) had an appropriate time of complementary feeding. Mothers who had caesarean-section delivery and GDM were more likely to initiate breastfeeding after an hour of delivery, p<0.05. Exclusive breastfeeding was highly practiced by older, multiparous, and higher education mothers. Unemployed mothers were more likely to continue breastfeeding for one year. GDM mothers were more inclined to delay breastfeeding initiation and introduced complementary feeding appropriately, p<0.05. This study's findings bridge the gap in providing information on infant feeding practices among mothers with a history of GDM and therefore might be beneficial for related personnel to figure out how to overcome this concerning issue in the future.

Abstract

This study aims to assess the rates of Gestational Weight Gain (GWG) among pregnant mothers with and without Gestational Diabetes Mellitus (GDM) and to investigate the sociodemographic and dietary intake factors associated with the rates of GWG. This cross-sectional study involved 77 pregnant mothers visiting selected government clinics. Sociodemographics, medical records, and dietary intake were obtained through a self-administered questionnaire. The mean age of the participants was 30.48±5.0 years old with 48.1% (n=37) of them were GDM diagnosed. Moreover, 27 (73.0%) of GDM mothers experienced abnormal GWG. Their mean intakes were 1,481±389 kcal/day, macronutrients such as carbohydrate, protein, and fat were 199.9±59.3 g/day, 75.5±23.9 g/day, 44.1±15.3 g/day accordingly, fiber was 6.9±7.0 g/day and followed by micronutrients such as folate, calcium, iron, and zinc were 137.4±78.9 μg/day, 738.6±335.3 mg/day, 16.8±7.1 mg/day, and 6.0±2.6 mg/day respectively. This study reported no significant association between GDM status and GWG rates but there was a significant association between pre-pregnancy BMI and zinc intake with rates of GWG among pregnant mothers (p<0.05). In conclusion, the latest guidelines of GWG rates should include GDM status among pregnant mothers according to the specific pre-pregnancy Body Mass Index (BMI) to ensure, they can adhere to the new recommendations and lower the risk of pregnancy complications that may occur due to abnormal rates of GWG.

Abstract

This study is aimed to assess the adherence status of renal diet, determine the association between sociodemographic characteristics, dietary knowledge, nutrition literacy, and perception of recommended diet and fluid, while also identifying significant factors that play a role in influencing adherence to the renal diet within this specific healthcare setting. This cross-sectional study with purposive sampling recruited 71 of Chronic Kidney Disease (CKD) patients attending private dialysis centers in Selangor. Information on sociodemographic data, the End Stage Renal Disease Adherence Questionnaire (ESRD-AQ), the Dialysis-Related Dietary Knowledge Questionnaire (DDKQ), and the Dialysis Specific Nutrition Literacy Scale (DSNLS) were collected. The findings revealed that renal diet adherence of the respondent was 50.7%. The number of comorbidities, perception of recommended diet, and difficulty in watching diet recommendations were associated with adherence to renal diet among the participants. Further analysis using multivariate logistic regression revealed that the number of comorbidities (AOR=9.22, 95% CI:2.10˗40.47) and the difficulty in watching diet recommendations (AOR=0.04, 95% CI:0.003˗0.13) were significant determinants of dietary adherence. Dialysis patients with a higher number of comorbidities have a stronger likelihood of dietary adherence, meanwhile those with difficulty in watching diet recommendations have a higher tendency to non-adherence to renal diet. Thus, the difficulty identified should be overcome to improve renal diet adherence in the future.

Abstract

The aim of this study is to assess the prevalence of physical activity among pregnant women during early pregnancy, which is during their first trimester, and to investigate the determinants of Gestational Diabetes Mellitus (GDM) related to sociodemographic and physical activity level. This cross-sectional study focused on pregnant women aged between 18–45 in their second or third trimester of pregnancy and visiting selected public healthcare facilities. Questionnaires including sociodemographic, anthropometric, medical history, and the Pregnancy Physical Activity Questionnaire (PPAQ) were distributed. About 123 mothers were enrolled in this study. About 86.2% of them are physically active during early pregnancy, whereas just 13.8% are physically inactive. There was no statistically significant difference between pregnant mothers with and without GDM in physical activity level. Women aged 30 and older are three times more likely to acquire GDM (AOR=3.363, 95% CI: 1.314‒8.609), while women with a pre-pregnancy BMI of overweight or obese are 2.622 times more likely to develop GDM (AOR=2.622, 95% CI: 1.170‒5.875). In conclusion, even though both pregnant women with and without GDM have a high level of physical activity, the length of time spent in sedentary activities is notable. However, no significant relationship was observed between physical activity and GDM development, as most individuals were found to be following the suggested physical activity duration, yet advanced age and obese pre-pregnancy BMI were found to be determinants of the GDM occurrence.

Abstract

The aim of this study is to identify food security status and its association with dietary diversity among children from low-income households (B40) during COVID-19. A cross-sectional study was conducted among 174 mothers and their children (<2 years old) residing in Selangor urban area. The data collection was collected through sociodemographic questionnaire to assess maternal and children’s sociodemographic characteristics, Household Food Insecurity Scale (HFIAS) to assess household food insecurity status, and Minimum Dietary Diversity (MDD)-Children 6‒23 months to assess dietary diversity score for children. The data was analyzed descriptively and a Chi-square test was used to find the relationship between food insecurity and dietary diversity. Findings showed that 105 participants (62.6%) was food insecure, there was 52.9% had low dietary diversity. There was no association between food insecure and dietary variety among B40 mothers living in urban area in Selangor. However, this study found that maternal education level (p=0.006), years of education (p=0.014) and the number of household income recipient (p=0.018) are significantly associated with the food insecurity. This study has provided the prevalence of food insecurity among B40 living in urban area and it is suggested that further research involving diverse household income of participants and research areas should be done to provide better insights on food insecurity and dietary diversity among children in Malaysia.

Abstract

This study is aimed to investigate the prevalence of malnutrition using Royal Free Hospital Nutrition Prioritizing Tools (RFH-NPT) and the correlation between malnutrition and RFH-NPT with clinical parameters. A total of 80 cirrhosis liver patients without liver cancer were enrolled in this prospective study. This study was conducted at Selayang hospital, which is the national tertiary referral centre for liver disease in Malaysia. We administered the nutrition screening of RFH-NPT to the patients within 24 hours after admission. The utilization of the RFH-NPT resulted in a higher incidence of nutritional risk, at 73%. The RFH-NPT revealed a higher proportion of Child Pugh C patients at risk for malnutrition (46/50 or 92%, p= 0.002) and had superior capability in identifying patients with intermediate and high risk for malnutrition (35/61 or 57.3%, p=0.012) within the subset of patients with Model for End- Stage Liver Disease (MELD) scores below 15. Hence, we propose that the utilization of the RFH-NPT enhances medical professional’s capacity for early-stage prediction of malnutrition risk in patients with cirrhosis predominantly attributed to hepatitis virus infection.

Abstract

This study aims to explore the baseline obstetric characteristics of singleton, primigravid women with Gestational Diabetes Mellitus (GDM) from an experimental study. A total of 58 subjects with GDM, who were solely on diet control, were recruited from Meru Health Clinic in Selangor. The majority of the subjects (97%) were Malay. The mean age of the subjects was 27.59±3.2 years, with a pre-pregnancy weight of 68.86±17.8 kg, a height of 1.56±0.1 m, and Body Mass Index (BMI) of 28.08±6.7 kg/m2. Most subjects in this study (93%) were diagnosed with GDM at their first Oral Glucose Tolerance Test (OGTT), conducted at a mean gestational age of 18.93±2.8 weeks. The OGTT readings at diagnosis were 5.1±0.6 mmol/L for Fasting Blood Glucose (FBG) and 7.9±1.5 mmol/L for 2-Hour Postprandial (2HPP). The Blood Sugar Profile (BSP) levels were recorded as follows: fasting at 4.8±0.4 mmol/L, pre-lunch at 5.1±0.5 mmol/L, post-lunch at 5.3±0.5 mmol/L, and post-dinner at 5.3±1.5 mmol/L. Macronutrient intake was within the recommended guidelines. The subjects delivered at a mean gestational age of 38.8±1.1 weeks, with a mean birth weight of 2.99±0.4 kg. No incidences of macrosomia were recorded in this study. In conclusion, the subjects underwent early GDM screening, had well-controlled antenatal blood glucose levels, and experienced no adverse pregnancy outcomes.
