Abstract

Background: Low Back Pain (LBP) is a complaint of pain felt in the lower back which originates from the spinal region (lower back), muscles, nerves and other structures around it7. Low back pain is caused by muscle stretching or injury, sufferers with complaints of LBP experience a decrease in lumbar range of motion, therefore researchers will carry out mobilization with movement treatment with McKenzie Exercise to find out which is the best approach to increase lumbar flexibility. Purpose: To determine the effectiveness of the combination of mobilization with movement and Mckenzie exercise in increasing spinal flexibility in patients complaining of back pain due to high LDL lipid serum. Method: This research design uses a descriptive case study design with a two group pretest-posttest design with an approach through physiotherapy assessment and physiotherapy intervention which includes history taking, diagnosis, intervention and evaluation, directly with respondents at the STIKES Baitul Hikmah physiotherapy clinic. The type of data used in this research uses primary data with the implementation of this activity going through several stages, namely filling in the biodata then carrying out a pre-test and post-test with a modified Schober test. Results: The results of Mrs. N before being given the combination therapy of MWM and Mckenzie exercise had a pre-test modified Schober test value of 3.7 cm and after being given the combination therapy for 2 months twice a week had a modified Schober test value of 6.3 cm. Suggesstion: This research can be developed with more variables such as their relationship with various types of low back pain, age and gender.   Keywords: Health Education; Balance; Flexibility; Elderly   Pendahuluan: Low Back Pain (LBP) adalah keluhan rasa nyeri yang dirasakan pada bagian punggung bawah yang sumbernya dari tulang belakang daerah spinal (punggung bawah), otot, saraf dan struktur lain yang berada disekitarnya. Low back pain disebabkan adanya peregangan otot maupun cedera, penderita dengan keluhan LBP mengalami penurunan range of motion lumbal oleh karena itu peneliti akan melakukan treatment mobilisation with movement dengan McKenzie exercise untuk mengetahui mana pendekatan terbaik untuk meningkatkan fleksibilitas lumbal. Tujuan: Untuk mengetahui efektivitas kombinasi mobilization with movement dan mckenzie exercise pada peningkatan fleksibilitas lumbal dengan pasien keluhan sakit pinggang akibat serum Lipid LDL tinggi. Metode: Rancangan penelitian ini menggunakan desain deskriptif studi kasus dengan two group pretest-posttest design dengan pendekatan melalui assessment fisioterapi dan intervensi fisioterapi yang meliputi anamnesis, diagnosa, intervensi, dan evaluasi, secara langsung dengan responden di klinik fisioterapi STIKES Baitul Hikmah. Jenis data yang digunakan dalam penelitian ini menggunakan data primer dengan pelaksanaan kegiatan ini melalui beberapa tahapan yaitu mengisi biodata lalu melakukan pre-test dan post-test dengan modified schober test. Hasil: Hasil kasus Ny. N sebelum diberikan terapi kombinasi MWM dan Mckenzie exercise memiliki nilai pre-test modified schober test 3.7 cm dan setelah diberikan terapi kombinasi selama 2 bulan setiap 2 kali seminggu memiliki nilai modified schober test 6.3 cm. Simpulan: Terdapat efektivitas signifikan oleh kedua perlakuan terhadap peningkatan fleksibilitas lumbal pada pasien low back pain dengan kadar serum lipid LDL tinggi. Saran: Penelitian ini dapat dikembangkan dengan lebih banyak variable seperti hubungannya dengan berbagai jenis low back pain, usia maupun jenis kelamin.   Kata Kunci: Fleksibilitas lumbal; Mobilisation With Movement; McKenzie ; Lansia

Abstract

Background: Aging is a natural process that cannot be avoided by every individual. Aging is a process of gradual loss of the ability of tissue to repair itself or replace itself and maintain its normal structure and function. This decrease in physical capacity is caused by several factors including musculoskeletal, cardiovascular and neuromuscular factors. The occurrence of decreased physical capacity is a condition that is very common in society that can trigger pain. As much as 28-35% of elderly people over 65 years old fall at least once a year and increase at the age of over 75 years by 32-42%. So, it is very important for the elderly to maintain and maintain muscle strength. Purpose: Provide health education as an effort to increase knowledge about the importance of balance and flexibility in the elderly. Method: The implementation of this activity goes through several stages, namely asking for permission from the local primary health, namely the Way Sulan Health Center, explaining the importance of conducting education to the elderly and explaining the importance of knowledge/understanding of elderly health problems with specific discussions on balance and flexibility. In the implementation stage, all participants were given material presentations using lecture and presentation methods. This was followed by a discussion and question and answer session. Results: Based on the Time Up and Go Test examination of 70 participants, it shows that the majority of elderly people have a low risk of falling as many as 13 people (18.57%). As many as 24 elderly people (34.28%) are in the moderate risk of falling category and as many as 33 people (47.14%) have a greater risk of falling. And based on the Sit & reach test examination, it shows that the majority of elderly people have hamstring and lowerback flexibility ≥ 4 inches 16 people (22.85%). As many as 20 elderly people (28.57%) are in the flexibility category ≥ 3 inches, as many as 30 people (42.85%) with flexibility ≥ 2 inches and as many as 4 people have flexibility ≥ 1 inch (5.71%). Conclusion:  Health counseling or education for the elderly about balance and flexibility has a very good impact on the community, especially on the mechanism of managing elderly health that can be done independently or with family support. Educational programs about balance and flexibility also increase awareness of the importance of maintaining fitness, and being aware of the risk of falling in the elderly. .Keywords: Balance; Elderly; Flexibility; Health education Pendahuluan: Lanjut usia (lansia) merupakan proses alami yang tidak dapat dihindari oleh setiap individu. Penuaan (menjadi tua: aging) adalah suatu proses penghilangan secara perlahan-lahan kemampuan jaringan untuk memperbaiki diri atau mengganti diri dan mempertahankan struktur dan fungsi normalnya. Penurunan kapasitas fisik ini disebabkan oleh beberapa faktor diantaranya faktor penurunan muskuluskeletal, kardiovaskular dan neuromuskular. Kejadian penurunan kapasitas fisik merupakan kondisi yang sangat banyak dijumpai di masyarakat yang bisa memicu terjadinya rasa nyeri. Sebesar 28-35% lansia di atas 65 tahun setidaknya jatuh satu kali dalam satu tahun dan meningkat pada usia di atas 75 tahun sebesar 32-42%. Jadi, sangat penting bagi lansia untuk menjaga dan memelihara kekuatan otot. Tujuan: Memberikan penyuluhan kesehatan sebagai upaya meningkatkan pengetahuan mengenai pentingnya keseimbangan dan fleksibilitas pada lansia. Metode: Pelaksanaan kegiatan ini melalui beberapa tahapan yaitu meminta ijin kepada kesehatan primer setempat yaitu puskesmas Way Sulan menjelaskan kepentingan untuk melakukan penyuluhan kepada lansia dan menjelaskan pentingnya pengetahuan/pemahaman masalah kesehatan lansia dengan spesifik pembahasan keseimbangan dan fleksibilitas. Tahap pelaksanaan seluruh peserta diberikan pemaparan materi dengan metode ceramah dan presentasi. Selanjutnya dilakukan dengan sesi diskusi dan tanya jawab. Hasil: Berdasarkan pemeriksaan Time Up and Go Test  dari 70 peserta menunjukkan bahwa mayoritas lansia memiliki risiko jatuh kecil sebanyak 13 orang (18.57%). Sebanyak 24 orang lansia (34.28%) dalam kategori risiko jatuh sedang dan sebanyak 33 orang (47.14%) memiliki risiko jatuh lebih besar. Dan berdasarkan pemeriksaan Sit & reach test menunjukkan mayoritas lansia memiliki fleksibilitas hamstring dan lowerback ≥ 4 inch 16 orang (22.85%). Sebanyak 20 orang lansia (28.57%) dalam kategori fleksibilitas ≥ 3 inch,  sebanyak 30 orang (42.85%) dengan fleksibilitas ≥ 2 inch dan sebanyak 4 orang memiliki fleksibilitas ≥ 1 inch (5.71%). Simpulan: Penyuluhan kesehatan atau edukasi pada lansia tentang keseimbangan dan flesibilitas memberikan dampak yang sangat baik pada masyarakat, khususnya pada mekanisme pengelolaan kesehatan lansia yang dapat dilakukan secara mandiri atau oleh dukungan keluarga. Program edukasi tentang keseimbangan dan fleksibilitas juga meningkatkan kesadaran tentang pentingnya mempertahankan kebugaran, dan mewaspadai resiko jatuh pada lansia.

Abstract

The decision to undergo Total Knee Arthroplasty is not an easy one; patients need courage to undergo TKA. Chronic osteoarthritis complaints of more than grade 2 severely limit an individual's daily activities, and Joint arthroplasty is performed to enhance health-related quality of life in patients with end-stage hip or knee OA. Interestingly, no patient attained a higher level of physical activity 5 years post-surgery compared to their preoperative expectations. Possible reasons for this unmet expectation include other unrealistic expectations, deficiencies in lower extremity functional rehabilitation after TKA, and difficulties in accurately describing and reporting the amount of physical activity in daily life. The type of research used is quasi-experimental with a two-group pretest-posttest design. Sampling used an acidental This study involved 40 respondents as samples. Data analysis was conducted using Paired T-Test and Independent T-Test methods. The study results indicated that both groups had an impact on functional ability. (p<0.000) intervention treatment with the TUG test and (p<0.216) control treatment, so on the sit to stand test, intervention treatment (p<0.000) and (p<0.003) in the control group, but the intervention treatment was proven to improve the average score better also on functional ability using the TUG test compared to the control group (2 vs 1.05) also (2.2 vs 0.95). on the sit to stand test. It is recommended to physiotherapists to apply early activity training to improve functional activity in post-TKA patients with a time span of 3-6 months. Keywords: total knee arthroplasty, early physical activity, functional activity
