Abstract

Introduction: Elderly is the final stage of the human development cycle. Every year the number of elderly people increases in Indonesia in the Bali area based on the population census. The prevalence of the number of elderly people from 2021 to 2022 increases. According to a survey by the Indonesian Medical Gerontology Association (PERGEMI), as many as 24.6% of the elderly population (seniors) in Indonesia have a history of chronic disease. In the condition of the elderly who are unable to take care of themselves fully, they need a caregiver or companion who is often called a caregiver, the role of a caregiver is as a provider of nursing care to the patient. Objectives: This study aims to describe of the care giver's experience in caring for elderly people with palliative conditions. Methods: Data were collected using the face-to-face individual in deep interview, with semi structure interview questions. Results: The results of this study by using interviews and observations from 4 participants, 4 themes were obtained that show the Care Giver's experience in caring for the elderly with palliative conditions, namely: (1) End Stage of Life Phase (2) Routine Care (3) Psychological Support (4) Spiritual Support. Conclusions: The description of the Care Giver's experience is considering palliative conditions as conditions in the End Stage of Life Phase; Daily care focuses on Routine Care; Psychological support is really needed; Spiritual support is really needed. Based on the research results, it can be concluded that elderly people in nursing homes in the final phase of their lived experience worsening conditions between life and death and all their needs are assisted by caregivers. In palliative conditions, all daily needs are fulfilled by the caregiver. The main focus of care for elderly people with palliative conditions is providing psychological support and spiritual supported

Abstract

ntroduction: Hypertension is a high blood pressure condition often referred to as the "silent killer" as it is frequently asymptomatic but can lead to a range of serious health complications, including heart disease and stroke. The prevalence of hypertension continues to rise worldwide, with projections of reaching 29% of the adult population by 2025. To control hypertension, complementary therapies such as acupressure are emerging as a supportive complement to the management of high blood pressure, especially for the elderly, to improve their quality of life and ease the burden on the healthcare system. Objectives: to analyze the effect of acupressure therapy on blood pressure in hypertensive elderly. Methods: Pre-Experimental Design, with a One-Group Pretest-Posttest Design. Results: The results showed a decrease in average systolic blood pressure by 24.08 mmHg, from 148.08 mmHg in the pre-test to 124.00 mmHg in the post-test, with a p value of 0.002. In addition, there was a decrease in mean diastolic blood pressure by 11.83 mmHg, from 85.83 mmHg in the pre-test to 74.00 mmHg in the post-test, with a value of (p value 0.013). These results indicate that acupressure therapy has a significant effect on reducing systolic and diastolic blood pressure in elderly hypertension. Conclusions: These findings support using acupressure as a complementary therapy to assist in managing hypertension in the elderly, potentially improving their quality of life and reducing the burden of hypertension-related health services.

Abstract

Introduction: A Headache is a common complaint among individuals with hypertension and is often associated with elevated blood pressure. One form of complementary therapy that can be applied to alleviate this symptom is Slow Stroke Back Massage (SSBM). Objectives: This study aimed to examine the effect of Slow Stroke Back Massage on headache intensity in older adults with hypertension. Methods: This quantitative research employed a quasi-experimental design with a non-equivalent control group pretest-posttest approach. Thirty-four hypertensive older adults participated in the study and were selected through purposive sampling. The intervention group received SSBM therapy three times per week for 10 minutes over four weeks, while the control group received only standard antihypertensive medication. Headache intensity was measured using the Numeric Rating Scale (NRS). Results: The mean pre-test headache intensity score was 6.12 ± 1.62 in the intervention group and 5.82 ± 1.55 in the control group. After the intervention, the mean scores decreased to 2.71 ± 1.53 and 3.82 ± 1.55, respectively. The reduction in headache intensity was 3.41 points in the intervention group and 2.00 points in the control group. The Wilcoxon test showed a significant difference in the intervention group (p = 0.000), while the paired sample t-test confirmed significance in the control group (p = 0.000). The pre-test comparison between groups using the independent t-test indicated no significant difference (p = 0.592), whereas the post-test comparison using the Mann-Whitney U test showed a significant difference (p = 0.041). These findings suggest that SSBM significantly reduces headache intensity among hypertensive older adults. Conclusions: Slow Stroke Back Massage is an effective, safe, and non-pharmacological intervention for reducing headache intensity in hypertensive patients. Its application is recommended as a complementary therapy in elderly care.

Abstract

Pemberian air susu ibu secara eksklusif merupakan makanan yang terbaik untuk bayi. Air susu ibu (ASI) di berikan selama 6 bulan tanpa makanan tambahan lainnya dan di lanjutkan hingga umur 2 tahun, pemberian ASI dapat mengurangi angka kematian bayi dan dapat meningkatkan pertumbuhan dan perkembangan pada anak. Pemberian ASI perah (ASIP) merupakan salah satu upaya pendukung untuk mensukseskan pemberian ASI eksklusif. Kurangnya pengetahuan ibu tentang pemberian ASI perah memerlukan dukungan dari berbagai pihak, baik keluarga maupun lingkungan sekitar. Peran kader kesehatan dapat membantu meningkatkan upaya pemberian ASI eksklusif melalui pemberian ASI Perah (ASIP) sehingga ibu menyusui tidak perlu menggantikan ASI dengan susu formula saat ada kegiatan diluar rumah maupun bekerja. Pada saat ini masih banyak ditemukan ibu-ibu yang tidak mmberikan asi eksklusif kepada bayinya karena alasan bekerja. perlu tindak lanjut berupa penguatan keterampilan penyampaian informasi pengelolaan ASIP. Maka dari itu, beberapa pihak yang memiliki kompetensi dan kewenangan dapat berperan dalam kegiatan penguatan keterampilan pengelolaan ASIP pada kader setempat.

Abstract

Kehamilan usia dini merupakan permasalahan kesehatan reproduksi yang sering terjadi pada remaja dan dapat menimbulkan dampak negatif secara fisik, psikologis, maupun sosial. Rendahnya pengetahuan mengenai kesehatan reproduksi serta paparan informasi yang tidak tepat dari lingkungan sosial menjadi faktor yang berkontribusi terhadap terjadinya kehamilan usia dini. Penelitian ini bertujuan untuk mengetahui gambaran tingkat pengetahuan remaja putri mengenai pencegahan kehamilan usia dini di SMA Saraswati 1 Denpasar. Penelitian menggunakan desain deskriptif dengan pendekatan kuantitatif, dengan sampel sebanyak 115 responden yang dipilih melalui teknik purposive sampling. Instrumen penelitian berupa kuesioner pengetahuan yang telah diuji validitas dan reliabilitasnya. Hasil penelitian menunjukkan bahwa sebagian besar responden memiliki tingkat pengetahuan tinggi yaitu 108 orang (93,9%), pengetahuan cukup sebanyak 7 orang (6,1%), dan tidak terdapat responden dengan pengetahuan rendah. Temuan ini menggambarkan bahwa mayoritas remaja putri telah memiliki pemahaman yang baik mengenai upaya pencegahan kehamilan usia dini. Kesimpulan penelitian menegaskan pentingnya edukasi kesehatan reproduksi yang berkelanjutan melalui sekolah, tenaga kesehatan, dan orang tua untuk memperkuat pengetahuan remaja serta mencegah terjadinya kehamilan pada usia muda

Abstract

Hipertensi atau the silent killer masih menjadi tantangan kesehatan global yang memicu komplikasi fatal. Pengelolaan hipertensi memerlukan kombinasi terapi farmakologi dan non-farmakologi, salah satunya melalui terapi komplementer. Keberhasilan integrasi terapi ini sangat bergantung pada perilaku pasien yang didasari oleh pengetahuan memadai. Penelitian ini bertujuan untuk mengetahui hubungan antara tingkat pengetahuan dengan perilaku terhadap terapi komplementer pada penderita hipertensi di Puskesmas Mengwi 1. Penelitian kuantitatif ini menggunakan desain deskriptif korelasional dengan pendekatan cross-sectional. Teknik pengambilan sampel menggunakan metode non-probability sampling jenis purposive sampling, dengan jumlah sampel sebanyak 55 responden di wilayah kerja Puskesmas Mengwi 1. Data dikumpulkan melalui kuesioner pengetahuan dan kuesioner perilaku terapi komplementer yang telah teruji validitasnya. Analisis data menggunakan uji statistik Rank Spearman dengan tingkat signifikansi α = 0,05.Hasil penelitian menunjukkan mayoritas responden memiliki tingkat pengetahuan kategori baik sebanyak 30 orang (54,5%) dan perilaku kategori baik sebanyak 27 orang (49,1%). Uji Rank Spearman menunjukkan nilai p-value = 0,001 dengan koefisien korelasi (r) sebesar 0,676. Hal ini membuktikan adanya hubungan signifikan dengan kekuatan hubungan yang kuat dan arah positif antara pengetahuan dengan perilaku terhadap terapi komplementer pada pasien hipertensi. Semakin baik pengetahuan pasien mengenai jenis dan manfaat terapi komplementer, maka semakin baik pula perilaku mereka dalam mengaplikasikannya. Pengetahuan memadai menjadi landasan kognitif bagi pasien dalam mengambil keputusan kesehatan yang aman. Tenaga kesehatan diharapkan meningkatkan edukasi mengenai terapi komplementer guna mendukung optimalisasi kontrol tekanan darah pasien.

Abstract

Ibu hamil merupakan kelompok rentan mengalami kecemasan selama tindakan flebotomi pada pemeriksaan Triple Eliminasi. Kualitas komunikasi terapeutik tenaga kesehatan berperan penting dalam menurunkan kecemasan, meningkatkan rasa aman, serta mempengaruhi kepuasan terhadap pelayanan. Penelitian ini bertujuan untuk mengetahui hubungan antara komunikasi terapeutik dengan kepuasan ibu hamil pada proses flebotomi pemeriksaan Triple Eliminasi di Puskesmas Buleleng III Kabupaten Buleleng. Penelitian menggunakan desain deskriptif korelasional dengan pendekatan cross-sectional. Sampel sebanyak 52 ibu hamil dipilih menggunakan teknik purposive sampling. Instrumen penelitian berupa lembar observasi komunikasi terapeutik dan kuesioner kepuasan berbasis lima dimensi SERVQUAL. Analisis data dilakukan menggunakan uji korelasi Spearman Rank. Hasil penelitian menunjukkan gambaran komunikasi terapeutik berada pada kategori sangat baik jumlah responden sebanyak 30 orang dengan persentase 57,7%, sedangkan kepuasan ibu hamil berada pada kategori puas jumlah respondem 25 orang dengan persentase 48,1 %. Terdapat hubungan positif yang signifikan antara komunikasi terapeutik dan kepuasan ibu hamil (r = 0,781; p-value = 0,001 < 0,05), artinya semakin baik komunikasi terapeutik, semakin tinggi tingkat kepuasan pasien terhadap pelayanan flebotomi. Hubungan yang signifikan tersebut menunjukkan bahwa komunikasi terapeutik mampu menciptakan rasa aman, mengurangi kecemasan, dan meningkatkan persepsi positif ibu hamil terhadap pelayanan laboratorium. Oleh karena itu, peningkatan kompetensi komunikasi terapeutik tenaga kesehatan perlu dioptimalkan, terutama pada layanan pemeriksaan Triple Eliminasi

Abstract

Hypertension is a major global health problem and a leading risk factor for cardiovascular diseases, especially among the elderly, due to decreased vascular elasticity, increased peripheral resistance, and autonomic dysfunction. Despite pharmacological therapy, poor adherence and side effects highlight the need for complementary non-pharmacological interventions. Electroacupuncture has emerged as a potential therapy to support blood pressure control in elderly hypertensive patients. This study aimed to evaluate the effectiveness of electroacupuncture in reducing blood pressure among elderly patients with hypertension. A quantitative pre-experimental one-group pretest–posttest design was conducted on 20 elderly hypertensive patients. Electroacupuncture was applied to GV20, ExHN3, ExHN5, LI4, LI11, PC6, ST36, LR3, and KI3 points for 30 minutes per session over six sessions in three weeks. Blood pressure was measured before and after the intervention using a calibrated sphygmomanometer. Data analysis was performed using the Wilcoxon Signed Rank Test for systolic blood pressure and the Paired Sample T-Test for diastolic blood pressure with α = 0.05. Systolic blood pressure decreased significantly from 133.27 ±16.59 mmHg to 128.85 ±15.97 mmHg (p=0.011), while diastolic pressure decreased from 76.04 ±9.08 mmHg to 74.90 ±7.43 mmHg without statistical significance (p=0.240). Electroacupuncture is effective, safe, and economical in reducing systolic blood pressure in elderly hypertensive patients. It is recommended as a complementary therapy integrated with conventional management for holistic hypertension control and improving quality of life among the elderly.
