Abstract

Adiposity assessment is a part of the evaluation in coronary artery disease (CAD) prevention. Waist to height ratio (WHtR) is one of adiposity parameter in CAD management guidelines. This cross sectional study enrolled 30 subject with stable CAD undergone elective coronary angiography. Eightyfive subjects has abnormal WHtR (≥0,5). Median WHtR is 0,54 with 0,471-0,706 range, and Duke CAD prognostic index with median 37 (range 19-82). Analysis using spearman’s rank correlation show a significant moderate positive correlation (r=0,425; p=0,019) between WHtR and Duke CAD prognostic index. Waist to height ratio could be one of valuable indicator for coronary artery stenosis severity. We conclude that there is a relationship between WHtR and Duke CAD prognostic index.

Abstract

Education is a process to educate aimed to develop knowledge and potential. Based on SUSENAS – 2018, there are only 35,7% of our Indonesian that exercise regularly. This survey proved that there are only about one third people that actually exercise actively. With so many benefits and positive impacts that we could get by exercising, it is expected to see our society to have decent knowledge and understanding about physical exercise. Therefore, this study is aimed to find out how the educated intervention impact physical exercise knowledge of Indonesians with age range from 18 until 64 years old, therefore will achieve healthy society with great health quality that will further impact on our national life expectancy. This study was done by using the experimental method (one group pretest-posttest design) towards the 110 samples of Indonesians with age from 18 until 64 years old with purposive sampling and Wilcoxon analitic study. The participants participated in the webinar and completes questionnaires before and after the intervention to asses their level of knowledge about exercise. The findings revealed a difference in the mean level of knowledge among respondents before and after the webinar, with pre-test score of 79,18 and post-test score of 96,82 out of 100 as the total score. There were significant difference in the mean level of knowledge and results showed that the intervention education increased the knowledge of participant with P<0,001. In conclusion, intervention education is proven to increase participant’s knowledge about physical exercise in Indonesian with age range from 18 until 64 years old.

Abstract

Background: Cardiac tamponade is the situation where fluid accumulates in the pericardial cavity and compresses the heart, which leads to reduced cardiac output and shock. Their prevalence in post-open heart surgery, especially Coronary Artery Bypass Graft (CABG) surgery is 24% and commonly related to Post-Pericardiotomy Syndrome (PPS). Case Summary: A 57-year-old female with a history of CABG surgery presented to emergency with chief complaints of shortness of breath in the last 3 days, especially when lying down. The patient underwent CABG surgery from CAD2VD + LM in the last month. Physical examination found a muffled first & second heart sound and distended jugular vein. The electrocardiograph of the patient showed sinus rhythm with low voltage. Echocardiography was done on the patient, and the result showed severe pericardial effusion with a D shape and collapse of the right ventricle. We performed a pericardiocentesis with an initial drainage of 737 cc of serosanguineous fluid. Post-procedural echocardiography showed reduced pericardial effusion, good LV contractility, and RV not collapsed. Pericardial fluid analysis concluded it was a transudative type. Patient were stable on admission and discharged uncomplicated. This patient developed cardiac tamponade with a history of CABG surgery. Conclusion: Cardiac tamponade occurs frequently in post-CABG surgery patients, both in early and late onset. It is important to recognize the early signs of cardiac tamponade, as the condition does not always present during the early phase. This case also highlighted the importance of recognizing and preventing PPS as a delayed cause of tamponade, even when there is no apparent sign of systemic inflammation. -- Highlight: 1. A 57-year-old post-CABG patient presented with cardiac tamponade one month after surgery, ultimately diagnosed as post-pericardiotomy syndrome (PPS) based on clinical and imaging findings. 2. Prompt echocardiography-guided pericardiocentesis and recognition of PPS as a delayed inflammatory cause of tamponade highlight the importance of vigilance for late-onset complications after cardiac surgery.
