Abstract

Early breast milk production is an important requirement in supporting exclusive breastfeeding. Breast milk production in mothers with cesarean section is generally slower than in mothers who give birth normally. This study aimed to analyze the determinants of breast milk production in the 24 hours after cesarean section. This study applied a cross-sectional design, involving 662 postpartum mothers. The independent variables measured were type of delivery, breastfeeding position, active mobilization, joint care, oxytocin massage, early breastfeeding initiation, age and parity; while the dependent variable was breast milk production in the postpartum period. All variables were measured through documentation studies on secondary data, namely medical records of the hospital where the study was conducted. Furthermore, data analysis was carried out using logistic regression tests. The results of the analysis showed that the p value for each independent variable was type of delivery = 0.186, breastfeeding position = 0.001, active mobilization = 0.003, joint care = 0.001, oxytocin massage = 0.092, age = 0.771 and parity = 0.045. Furthermore, it was concluded that the determinants of breast milk production in the 24 hours after delivery were breastfeeding position, active mobilization, joint care and parity.Keywords: breast milk; postpartum; cesarean section; breastfeeding position; active mobilization; joint care; parity ABSTRAK Pengeluaran air susu ibu sejak dini merupakan syarat penting dalam mendukung pemberian air susu ibu secara eksklusif. Pemberian air susu ibu pada ibu dengan bedah caesar umumnya lebih lambat dibandingkan ibu yang melahirkan secara normal. Penelitian ini bertujuan untuk menganalisis determinan pengeluaran air susu ibu pada 24 jam pasca bedah caesar. Penelitian ini menerapkan rancangan cross-sectional, yang melibatkan 662 ibu postpartum. Variabel bebas yang diukur adalah jenis persalinan, posisi menyusui, mobilisasi aktif, rawat gabung, pijat oksitosin, inisiasi menyusu dini, umur dan paritas; sedangkan variabel terikat adalah pengeluaran air susu ibu pada masa postpartum. Seluruh variabel diukur melalui studi dokumentasi pada data sekunder yaitu rekam medis rumah sakit yang menjadi lokasi penelitian. Selanjutnya dilakukan analisis data dengan uji regresi logistik. Hasil analisis menunjukkan bahwa nilai p untuk masing-masing variabel bebas adalah jenis persalinan = 0,186, posisi menyusui = 0,001, mobilisasi aktif = 0,003, rawat gabung = 0,001, pijat oksitosin = 0,092, umur = 0,771 dan paritas = 0,045. Selanjutnya disimpulkan bahwa determinan pengeluaran air susu ibu pada 24 jam pasca persalinan adalah posisi menyusui, mobilisasi aktif, rawat gabung dan paritas.Kata kunci: air susu ibu; postpartum; bedah caesar; posisi menyusui; mobilisasi aktif; rawat gabung; paritas

Abstract

Background: Obesity has become an increasing public health problem, including among Indonesian university students. Lifestyle changes, particularly low physical activity and high fast-food consumption, are suspected to contribute to an increased body mass index (BMI) among students.Objective: This study aimed to analyze the factors associated with body mass index (BMI) among Universitas Muhammadiyah Aceh students.Methods: This study employed an analytical observational design with a cross-sectional approach and was conducted in August 2019. A total of 94 students were selected using proportional sampling methods. Data were collected through structured interviews and anthropometric measurements of the participants. Bivariate analysis was performed using the chi-squared test, followed by multivariate analysis using logistic regression to identify the determinant factors associated with BMI.Results: Most respondents had a normal nutritional status (62.77%), followed by overweight (17.02%) and obese (20.21%) statuses. Fast food consumption, snacking habits, screen-viewing duration, physical activity, nutritional knowledge, food promotion exposure, and use of food delivery services were significantly associated with body mass index (p<0.05). Low physical activity was the strongest determinant, increasing the risk of obesity by 12.84 times, followed by frequent fast food consumption, which increased the risk by 7.62 times.Conclusion: Low physical activity and high fast-food consumption were the main factors associated with increased BMI in university students. Campus-based interventions focusing on physical activity promotion, practical nutrition education, and improvement of the campus food environment are recommended to prevent obesity in students.
