Abstract

Stunting remains a critical public health issue affecting the quality of human resources, particularly in Indonesia, where its prevalence remains high across various regions. According to the 2021 Indonesia Nutritional Status Survey (SSGI), the national stunting prevalence was 24.4%, with North Sumatra reporting 30.11% and Binjai City 21.7%, all of which exceed the WHO target of below 20%. These statistics underscore the urgent need for effective prevention strategies, particularly through nutritional interventions at the primary healthcare level. This study aims to evaluate the implementation of the stunting prevention program at the Regional Technical Implementation Unit of the Binjai Estate Community Health Center in Binjai City. The program components include nutrition education, regular monitoring of pregnant women and toddlers, and the provision of supplementary feeding. A qualitative descriptive approach was employed, utilizing in-depth interviews with healthcare personnel and field observations conducted in 2023. Findings indicate that although the program is generally well-implemented, several challenges persist. These include low community participation in outreach activities and persistent cultural myths that impede proper understanding of stunting. Nonetheless, there has been a noticeable increase in awareness regarding balanced nutrition among pregnant women and caregivers of toddlers. Monitoring is routinely conducted via the SIMPUS and SIP systems; however, long-term impact evaluation remains insufficient. In conclusion, while the stunting prevention program demonstrates promising outcomes, further improvements are necessary in enhancing community engagement, strengthening communication strategies, and developing a more robust evaluation system. Strengthening human resource capacity, improving inter-sectoral coordination, and expanding public access to accurate nutritional information are recommended to more effectively combat stunting Keywords: Stunting, Program Implementation, Primary Health Care, Nutrition Education, Monitoring and Evaluation

Abstract

Degenerative diseases such as coronary heart disease, diabetes, cancer, and chronic respiratory disorders represent major public health challenges in Indonesia, including in Medan City, with their prevalence continuing to rise due to modern lifestyles characterized by high intake of sugar and fat and low dietary fiber consumption. In response, herbal medicine has emerged as a popular and culturally aligned alternative treatment, particularly due to its affordability. This study aims to analyze the factors influencing the perceived effectiveness of herbal medicine in managing degenerative diseases in Medan City. A quantitative, cross-sectional design was employed, involving 369 respondents selected through stratified proportional and purposive sampling across 21 sub-districts in Medan. Data were collected using a structured, content-validated, and reliability-tested questionnaire, and analyzed using chi-square and logistic regression techniques. The majority of respondents were female (79.9%) with a secondary education level (89.7%), and most perceived the availability of herbal medicine services as adequate (93.2%). Additionally, 90% of participants considered herbal medicine effective for managing degenerative conditions. Multivariate analysis revealed that educational attainment was the most significant predictor of perceived effectiveness (p = 0.012), with those possessing higher levels of education more likely to report positive outcomes. In contrast, gender and perceived service availability did not show significant associations. These findings highlight the need to enhance health literacy through educational interventions to optimize the safe and effective use of herbal medicine. Integrating herbal medicine into the formal healthcare system and promoting collaboration between traditional and biomedical practitioners are recommended strategies for improving degenerative disease management in urban populations such as Medan. Abstract

Herbal remedies have gained increasing popularity, particularly among individuals with degenerative diseases. This study aims to explore the factors influencing the use of herbal medicine among the degenerative disease community in Medan City, with a specific focus on internal factors such as knowledge and awareness, as well as external factors including social and cultural influences. The research involved 12 informants, comprising members of the herbal medicine user community, family members, and herbal medicine vendors. Data were collected through in-depth interviews and analyzed using the Miles and Huberman qualitative analysis framework. The findings reveal that public perception regarding the use of herbal medicine for degenerative diseases in Medan City is shaped by a relatively high level of knowledge and awareness. Most respondents reported acquiring information from friends and social media, underscoring the influential role of social networks and digital platforms in disseminating health-related information. Social and cultural factors also significantly affect decision-making processes. While traditional beliefs were not identified as dominant influences, support from family, friends, and the broader community emerged as critical motivators in the choice to use herbal remedies. In conclusion, the decision to use herbal medicine among individuals with degenerative diseases in Medan City is influenced by a combination of knowledge, awareness, and socio-cultural factors. A solid understanding of the benefits and risks associated with herbal medicine, coupled with high awareness, reinforces individuals' decisions. Moreover, social support from family, peers, and the community plays a vital role in enhancing trust and confidence in herbal therapies. It is recommended that reliable and evidence-based information about herbal medicine be disseminated more widely through social media and community networks. Additionally, public education and training initiatives should be implemented to promote the safe and effective use of herbal remedies, thereby maximizing their potential benefits in community health settings. Abstract

Background: Inefficient implementation of Standard Operating Procedures (SOPs) can prolong waiting times for elective surgeries, which can lead to decreased service quality, increased risk of postoperative complications, and decreased patient satisfaction in hospitals. Objective: To evaluate the impact of Standard Operating Procedure implementation on reducing waiting times for elective surgeries in hospitals. Methods: This study used a qualitative approach with a descriptive case study design and data collection techniques through observation and in-depth interviews with one specialist doctor, one nurse, and one administrative staff member. The results were analyzed using the thematic method from Braun and Clarke (2021). Results: The study showed that the implementation of Standard Operating Procedures (SOPs) had a positive impact on managing waiting times for elective surgeries at Bhayangkara Hospital Level II Medan. SOPs helped accelarate the service process through a more structured workflow, clear division of tasks, and increased coordination and administrative efficiency. Based on the results of the study, the average waiting time for elective surgeries at Bhayangkara Hospital Level II Medan after the implementation of SOPs was less than one week from the time the patient was scheduled for surgery, demonstrating the effectiveness of SOP implementation in accelerating service. Conclusion: SOP plays an important role in reducing waiting times for elective surgery by increasing the effectiveness of medical and administrative services and strengthening communication between service units.
