Abstract

Background: Performance-based capitation payments are an innovative approach in the health financing system implemented by the health social security administering body. Purpose: To analyze the impact and effectiveness of performance-based capitation payments in improving the quality of health services and resource management in the national health insurance system. Method: The literature review was conducted systematically through databases such as Google Scholar, PubMed Central, Researchgate, and other sources. The keywords used in the literature search were capitation, based, performance, commitment, service, and capitation-based, performance, and commitment. Results: Performance-based capitation payments have varying impacts on the quality of social security agency for health patient services at primary health facilities (PHF). Conclusion: Achievement of performance-based capitation indicators is influenced by several factors, including the availability of human resources (medical and non-medical personnel), infrastructure, information systems, governance and organization, and financing. These factors align with the primary health care capacity domain components in the WHO primary health care monitoring framework.   Keywords: Performance-Based Capitation; Quality of Health Services; Social Health Security Administration.   Pendahuluan: Pembayaran kapitasi berbasis kinerja merupakan suatu pendekatan inovatif dalam sistem pembiayaan kesehatan yang diimplementasikan oleh Badan Penyelenggara Jaminan Sosial Kesehatan (BPJS Kesehatan). Tujuan: Untuk menganalisis dampak dan efektivitas pembayaran kapitasi berbasis kinerja dalam meningkatkan kualitas pelayanan kesehatan dan pengelolaan sumber daya di dalam sistem jaminan kesehatan nasional. Metode: Penelitian literature review dilakukan secara sistematis melalui database seperti Google Scholar, PubMed Central, Researchgate, dan sumber lainnya. Kata kunci yang digunakan dalam penelusuran literatur adalah kapitasi, berbasis, kinerja, komitmen, pelayanan dan kapitasi berbasis, kinerja, dan komitmen. Hasil: Pembayaran kapitasi berbasis kinerja (P4K) memberikan pengaruh yang bervariasi terhadap kualitas pelayanan pasien BPJS kesehatan di fasilitas kesehatan tingkat pertama (FKTP). Simpulan: Tercapainya indikator KBK dipengaruhi oleh beberapa factor, meliputi ketersediaan sumber daya manusia (tenaga medis dan non medis), ketersediaan sarana prasarana, ketersediaan sistem informasi, tata kelola dan organisasi, serta pembiayaan. Faktor-faktor ini selaras dengan komponen domain kapasitas FKTP dalam kerangka pemantauan layanan primer WHO.   Kata Kunci: BPJS Kesehatan; Kapitasi Berbasis Kinerja; Kualitas Pelayanan Kesehatan.

Abstract

Background: Performance-based capitation payments are an innovative approach in the health financing system implemented by Social Security Agency on Health (BPJS Health). Purpose: To analyze of the implementation of capitation policy based on service commitment. Method: A descriptive qualitative approach with a case study approach, which aims to analyzing the implementation of the Performance-Based Capitation policy, by taking four variables that are considered most crucial in determining the success of the implementation of the Performance-Based Capitation policy in the city of Semarang, with the policy implementation model approach from Donald Van Meter and Carl Van Horn, namely communication factors, resources, implementer disposition, and implementer characteristics. Results: This approach aims to stimulate the provision of higher quality and more efficient health services, by providing incentives to health service providers based on optimal performance and service outcomes. The implementation of performance-based capitation payments provides several benefits, including increased accountability of health service providers, more effective cost management, and stimulation of innovation in service provision. Conclusion: Performance-based capitation payments by BPJS Health are a step forward in creating a health insurance system that focuses on service quality.
