Abstract

Penyakit tidak menular seperti hipertensi dan diabetes melitus merupakan penyebab utama kematian di Indonesia dan memiliki prevalensi yang terus meningkat, termasuk di wilayah Kelurahan Laikang, Kecamatan Biringkanaya, merupakan salah satu wilayah menunjukkan kecenderungan meningkatnya kasus penyakit kronis, yaitu kurangnya pengetahuan masyarakat mengenai pentingnya pengendalian tekanan darah dan gula darah sebagai upaya preventif terhadap penyakit kronis. Kegiatan dilaksanakan dalam bentuk penyuluhan kesehatan, pemeriksaan tekanan darah dan gula darah serta diskusi interaktif dan edukatif. Hasil kegiatan menunjukkan peningkatan pemahaman masyarakat tentang pentingnya pola hidup sehat, pengendalian stres, konsumsi makanan bergizi, dan pemeriksaan kesehatan secara rutin. Program ini diharapkan dapat menjadi model pemberdayaan masyarakat dalam pencegahan penyakit kronis dan memperkuat upaya promotive dan preventif di tingkat kelurahan.

Abstract

Background: Pre-anesthesia anxiety is a common phenomenon that can affect a patient's physiological and psychological state and impact the perioperative process. Identifying factors influencing patient anxiety is important for planning educational interventions and psychosocial support. Objective: This study aimed to analyze factors influencing patient anxiety before anesthesia, including demographic factors, previous experience, family support, and preoperative information. Methods: The study used a descriptive analytical design with a cross-sectional approach. The sample consisted of 100 elective patients undergoing general or regional anesthesia at Hospital X, selected using purposive sampling. The instruments used were a demographic questionnaire and the Hamilton Anxiety Rating Scale (HARS). Data were analyzed using the chi-square test and logistic regression. Results: The results showed that patient anxiety levels were associated with young age (p = 0.032), low education (p = 0.018), first-time surgical experience (p = 0.001), minimal family support (p = 0.005), and lack of preoperative information (p = 0.001). The dominant factor influencing anxiety was a lack of preoperative information, with an OR of 3.5. Conclusion: Patient anxiety levels before anesthesia are influenced by age, education, surgical experience, family support, and preoperative information. Preoperative educational interventions and family support are important strategies for reducing patient anxiety.

Abstract

Preoperative anxiety is a common psychological problem experienced by surgical patients and may affect both physical and psychological readiness prior to surgery. Various nursing interventions and forms of social support have been developed to reduce preoperative anxiety; however, the scientific evidence regarding their patterns and underlying mechanisms remains fragmented. This study aimed to map the forms of social support, nursing interventions, and mechanisms involved in reducing preoperative anxiety among surgical patients. A scoping review methodology was employed, involving a literature search across four electronic databases. Articles that met the inclusion criteria were analyzed and mapped using a charting table covering study objectives, types of interventions or social support, underlying mechanisms, and reported outcomes. A total of 20 articles were included in this review. The findings indicate that informational support through preoperative education, emotional and professional support from healthcare providers, and non-pharmacological interventions such as breathing relaxation techniques and spiritual approaches play a significant role in reducing preoperative anxiety. The identified mechanisms include reduced uncertainty, enhanced psychological coping, and regulation of physiological stress responses. This scoping review highlights the importance of a holistic nursing approach in preoperative care and provides a foundation for the development of evidence-based interventions to improve the quality of care for surgical patients.

Abstract

Background: Prolonged preoperative fasting can trigger surgical stress. Preoperative oral carbohydrate loading may reduce insulin resistance, improve patient comfort, and help preserve postoperative muscle function. Objective: This study aimed to compare changes in cortisol and blood glucose levels between patients receiving oral glucose 2 hours before surgery and those receiving placebo during laparoscopic cholecystectomy. Methods: This study used a single-blind randomized clinical trial involving patients undergoing elective laparoscopic cholecystectomy at Wahidin Sudirohusodo Hospital and its network hospitals in Makassar. Results: Mean cortisol levels at T2 were lower in the glucose group than in the placebo group, at 4.51 mcg/dl and 10.19 mcg/dl, respectively (p < 0.05). Blood glucose levels at T1 and T2 were also lower in the glucose group than in the placebo group (p < 0.05). A significant decrease in cortisol levels from T0 to T2 was also observed (p < 0.05). Conclusion: Oral glucose administration 2 hours before surgery was effective in reducing cortisol levels and maintaining blood glucose stability up to 24 hours postoperatively compared with placebo.

Abstract

Surgical procedures are medical interventions that require optimal anesthesia preparation to ensure patient safety and comfort during and after surgery. Patients' limited understanding of pre-anesthesia management often gives rise to preoperative anxiety and non-adherence to preoperative instructions, such as fasting before surgery and providing a complete medical history, which can increase the risk of anesthesia-related complications. This community service activity aimed to provide education to patients undergoing surgical procedures on pre-anesthesia management in order to improve their knowledge, compliance, and physical and psychological readiness. The activity was carried out at RSU Wisata UIT, targeting preoperative patients along with their accompanying family members, using lecture methods, question-and-answer discussions, and educational media (leaflets and PowerPoint slides), which began and ended with the administration of questionnaires (pre-test and post-test) to measure changes in participants' anxiety levels. The results showed a decrease in patients' anxiety levels after the education was provided, indicating that pre-anesthesia education intervention is effective in preparing patients for surgical procedures. This activity is expected to serve as a model for a sustainable intervention in efforts to improve the quality of anesthesia nursing care and patient safety in hospitals.
